- Accuracy of acoustic evaluation of swallowing as a diagnostic method of dysphagia in individuals affected by stroke: preliminary analysisPublication . Enz, Vivian; Vaz, Amanda; Nunes, Maria Cristina; Rosa, Marcelo; Nunes, Janaína; Marques, Jair; Santos, Rosane SampaioAfter a stroke, more than half of the patients have some kind of disability, and dysphagia is frequently found. Cervical auscultation by Doppler sonar is an innovative technique with gain of credibility in the clinical evaluation of swallowing. To verify the diagnostic accuracy of Doppler sonar along with the DeglutiSom® software as an auxiliary method in the evaluation of oropharyngeal dysphagia in patients after stroke. The research is a cross-sectional, uncontrolled, blind, quantitative study with systematic random sampling. Patients from inpatient and outpatient units of a reference hospital with a stroke care unit were concomitantly submitted to both Doppler sonar and Fiberoptic Endoscopic Evaluation of Swallowing (FEES®). Seventy-three audio files collected from 26 patients through Doppler sonar were analyzed using DeglutiSom® software and confronted with the FEES® report, regarding three food consistencies offered to them during the exam. The study showed that the Doppler sonar correctly identified, among all the analyzed files, those that actually presented tracheal aspiration as well as it effectively identified patients who did not aspirate. The Youden index of 0.91 corroborates this information, showing a promising accuracy in detecting tracheal aspiration in the studied sample. The study evaluates the diagnostic accuracy of Doppler sonar, showing that it can be used as a valuable tool in the diagnosis of tracheal aspiration in patients after stroke. It is important to emphasize that the identification of residue by this method requires further studies. Also, larger sample size and more than one blind evaluator should be considered in future researches to increase the reliability of the proposed method.
- Acoustic analysis of swallowing as an auxiliary method for assessing dysphagia in Parkinson's diseasePublication . Silva, Glauce; Figueiredo, Ana Clara; Zeigelboim, Bianca; Araújo, Cristiano; Neto, José; Santos, Rosane SampaioObjective: To measure the accuracy of DeglutiSom as an auxiliary method to assess swallowing in patients with Parkinson's disease (PD). Methodology: Accuracy study. Among 248 individuals, 91 participants met the inclusion criteria, with a mean age of 64.9 years (SD 7.7), 53.8% male and 46.2% female, with a mean disease duration of 12.6 years (SD 3.8 years). Two instrumental studies were performed: An acoustic analysis with Sonar Doppler via the DeglutiSom Software, and a videofluoroscopic swallow study. Three judges analyzed the swallowing sounds in the DeglutiSom software and the instrumental examination findings. Results: The inter-rater reliability was 90.1%, with 4.4% degree of partial agreement and 5.5% disagreement. Sensitivity resulted in 90.0% and specificity in 90.0%. A sensitivity of 97.0% and specificity of 91.0% are indicators of a high validity for the dysphagia screening method, with predictive value (+) of 97.0% and predictive value (-) of 91.0%, with an accuracy method of 96.0%. The validity indicator values for screening aspiration were also high, with a sensitivity of 90.0%, specificity of 90.0%, predictive value (+) of 82.0%, predictive value (-) of 95% , 0% and 90.0% accuracy. Conclusion: The method proposed is considered appropriate for oropharyngeal dysphagia and tracheal aspiration screening in patients with Parkinson's Disease.
- Acoustic analysis of swallowing sounds with the use of Sonar Doppler in premature babies during food transitionPublication . Vieira Cunha, Talita; Figueiredo, Ana Clara; Guidotti Lemos, Raquel; Pereira, Karina; Zeigelboim, Bianca Simone; Stechman-Neto, José; Sampaio Santos, RosaneObjective: To define the acoustic parameters and the feasibility of using swallowing acoustic analysis as an auxiliary method for the transition from nasogastric or orogastric tube feeding to oral breastfeeding. Methodology: A cross-sectional study, which consisted of the following steps: 1. Data collection: Thirty-two newborns participated in this study (16 preterm and 16 full-term); 2. Clinical speech-language pathology evaluation: Clinical data were collected using Sonar Doppler used to assess the readiness of preterm infants to start oral feeding and swallowing sounds. Swallowing sounds were captured during breastfeeding and analyzed according to the frequency, intensity, and time of swallowing, as well as the pause time between swallows and the number of swallows. Results: The control and study groups presented significant differences in the mean wave time and swallowing per minute variables. Conclusion: Sonar Doppler, as an instrument used to capture sounds, is a viable auxiliary resource to evaluate the transition from nasogastric or orogastric tube to oral breastfeeding.
- Action verbal fluency in Parkinson’s patientsPublication . Tello Rodrigues, Inês; Ferreira, Joaquim; Coelho, Miguel; Rosa, Mário; Castro-Caldas, AlexandreWe compared the performance of 31 non-demented Parkinson´s disease (PD) patients to 61 healthy controls in an action verbal fluency task. Semantic and phonemic fluencies, cognitive impairment and behavioural dysfunction were also assessed. The mean disease duration of PD was 9.8 years (standard deviation (SD) = 6.13). There were no age (U = 899.5, p = 0.616), gender(chi-square = 0.00, p = 1.00) or literacy (U = 956, p = 0.96) differences between the two groups. A significant difference was observed between the two groups in the action verbal fluency task (U = 406.5, p < 0.01) that was not found in the other fluency tasks. The education level was the only biographical variable that influenced the action (verb) fluency outcomes, irrespective of disease duration. Our findings suggest a correlation between the disease mechanisms in PD and a specific verb deficit, support the validity of the action (verb) fluency as an executive function measure and suggest that this task provides unique information not captured with traditional executive function tasks.
- Acupuncture and expression of neurotransmitters in temporomandibular disorders: systematic reviewPublication . Vilela, Maria Cristina; Ramos, Carina; Silva, Hilton JustinoTemporomandibular disorders (TMD) is a common condition of chronic orofacial pain. Acupuncture is a therapeutic method of traditional Chinese medicine (TCM) and some studies to get understand the mechanisms of action and molecular basis involved in this approach therapeutic. We propose to verify in the current literature on the integration of neurotransmitters expression in treat temporomandibular joint disorders by acupuncture in adulthood. A predefined protocol was used and registered on PROSPERO. This qualitative review of the literature analysed international scientific publications in several databases that used the following keywords: Temporomandibular Joint Disorders OR Acupuncture OR Neurotransmitter. Then, in the study selection phase, the eligible texts were read in full to determine their final inclusion or exclusion. The pre-established selection criteria in the protocol determined the inclusion or exclusion of studies during the selection phase. Although to know the diverse effects of acupuncture are intrinsically linked at the cellular and molecular level there is not yet any paper about this in adulthood, but there is a publication about the relationship between TMD and Acupuncture Treatment. There are no studies in the literature that correlate the temporomandibular disorders, acupuncture therapy and neurotransmitters in humans.
- An investigation of the voice handicap index with speakers of portuguese: preliminary dataPublication . Guimarães, Isabel; Abberton, EvelynAlthough the Voice Handicap Index (VHI) has been recognized as a useful tool for quantifying American English speakers’ perceived consequences of voice disorder, it has not yet been widely applied in the clinic to study dysphonic patient populations, or tested with a normally speaking population. The purpose of this investigation was to obtain information about the VHI in a group of Portuguese speakers with voice complaints compared with an age- and gender-matched group of speakers without voice complaints. The results demonstrate that speakers with voice complaints have overall VHI total scores significantly higher than speakers without voice complaints (p < 0.001). This is also true for allVHIsubscores in the emotional (p < 0.001), functional (p < 0.05), and physical (p < 0.001) domains. So, the assumption that a group with voice complaints has higher voice handicap impact than the matched control comparison group is justified for Portuguese speakers.
- Aphasia assessment: impact of material on naming performancePublication . Fonseca, José; Miranda, Filipa; Leal, Gabriela; Melo, Teresa Pinho e; Martins, Isabel PavãoBackground: Naming and lexical retrieval difficulties are common symptoms of aphasia. Naming abilities are usually evaluated by means of real objects or pictures or line drawings that are printed. Objective: The aim of this study was to investigate whether the ability to name objects among individuals with aphasia is influenced by the dimensions of the visual stimuli and to understand whether the order of presentation of the stimuli, number of years of education and length of time post-onset are involved in the success of naming. Methods: The naming abilities of healthy controls and patients with acute or chronic aphasia due to stroke were assessed through presentation of two sets of 24 stimuli consisting of real objects and color photographs of the same objects presented on a screen. The results obtained under these two conditions were compared by means of within-subject paired ANOVA, controlling for presentation order. Results: 40 patients (62.4 ± 17.3 years old; 70% females; mean education level of 8.5 ± 5.3 years; and 60% evaluated within the first eight days after stroke) and 50 controls that were age, gender and education-matched were included. Object naming was significantly better than naming color photographs among the patients (p = 0.001), but no differences were observed among the controls. Age, education, length of time postonset and presentation sequence did not have any impact on naming performance. Conclusion: These results suggest that use of digital color photographs for naming objects in assessment of aphasia may be associated with lower naming performance, compared with use of real objects. This needs to be taken into account when different forms of stimuli are presented in sequential aphasia evaluations.
- Auditory processing disorder test battery in european portuguese: development and normative data for pediatric populationPublication . Martins, Jorge Humberto; Alves, Marisa; Andrade, Susana; Falé, Isabel; Teixeira, AntónioThere is an increasing need for state-of-the-art Central Auditory Processing assessment for Portuguese native speakers, applicable as early as possible. As a contribution to answering this need, this paper presents a new battery for Central Auditory Processing assessment for European Portuguese applicable to children aged 5 and above, named BAPA-PE, providing information regarding test selection and development. The battery consists of six behavioral tests: Staggered Spondaic Words (SSW) for European Portuguese, Filtered Speech, Speech in Noise, Detection Interval in Noise, Duration, and Frequency Pattern. The normative data for children aged 5 to 12 are also reported. A sample was obtained of 217 subjects without ear pathology and with typical development. Each age group was composed of at least 30 children. All children were evaluated using pure tone audiometry, speech audiometry, impedance, and otoacoustic emissions. Normative scores are reported for each of the six auditory processing tests. The assessment is applicable to young children (aged 5 and 6). The statistical analyses showed significant effects in scores of Age for all tests and of Ear for several tests. The main result from the work presented, the Auditory Processing Assessment Battery—European Portuguese (BAPA-PE), is available for clinical use with normative data. This battery is a new tool for behaviorism assessment of European Portuguese speakers with suspected central auditory pathology and for monitoring the results of auditory training
