  • Translation, cross-cultural adaptation, and validation of measurement instruments :

    Cross-cultural validation of self-reported measurement instruments for research is a long and complex process, which involves specific risks of bias that could affect the research process and results. Furthermore, it requires researchers to have a wide range of technical knowledge about the translation, adaptation and pre-test aspects, their purposes and options, about the different psychometric properties, and the required evidence for their assessment and knowledge about the quantitative data processing and analysis using statistical software. This article aimed: 1) identify all guidelines and recommendations for translation, cross-cultural adaptation, and validation within the healthcare sciences; 2) describe the methodological approaches established in these guidelines for conducting translation, adaptation, and cross-cultural validation; and 3) provide a practical guideline featuring various methodological options for novice researchers involved in translating, adapting, and validating measurement instruments. Forty-two guidelines on translation, adaptation, or cross-cultural validation of measurement instruments were obtained from “CINAHL with Full Text” (via EBSCO) and “MEDLINE with Full Text”. A content analysis was conducted to identify the similarities and differences in the methodological approaches recommended. Bases on these similarities and differences, we proposed an eight-step guideline that includes: a) forward translation; 2) synthesis of translations; 3) back translation; 4) harmonization; 5) pre-testing; 6) field testing; 7) psychometric validation, and 8) analysis of psychometric properties. It is a practical guideline because it provides extensive and comprehensive information on the methodological approaches available to researchers. This is the first methodological literature review carried out in the healthcare sciences regarding the methodological approaches recommended by existing guidelines.
  • Translation, Adaptation, and Validation of the L’Échelle d’Interactions Infirmière-Patient-23 for the Portuguese Culture:

    Nursing care is based on the interaction between nurse and patient. The L’Échelled’Interactions Infirmière-Patient-23 (EIIP-23) is used to evaluate and understand the perceptionof nurses about their interventions in the practice of care, to reach better health results. The presentstudy aims to validate the questionnaire EIIP-23 to Portuguese, evaluating its psychometric prop-erties. Methods: This is methodological research for the process of cross-cultural translation andadaptation. Results: The process of cross-cultural translation and adaptation were satisfactory. Thecommittee of experts reached an agreement of more than 90% in the first evaluation for all theitems. The internal consistency of the nurse-patient interaction scale 22-PT (NPIS-22-PT) was 0.864.Exploratory and confirmatory factor analyses were carried out in the NPIS-22-PT model, with threefactors. The results show that the final factorial solution presents acceptable goodness of fit indexesand adequate convergent validity. Conclusion: The translated version produced a good qualitypsychometric evaluation, and can be considered a valid, trustworthy, and useful instrument toevaluate the nurse-patient interactions in Portugal.   • Evaluating the Methodological Approaches of Cross-Cultural Adaptation of the Bedside Handover Attitudes and Behaviours Questionnaire into Portuguese

    Abstract: Nurse managers need culturally adapted assessment instruments to support the implementation of change to Nursing Bedside Handover (NBH) in healthcare institutions. This study aimed to cross-culturally adapt the Bedside Handover Attitudes and Behaviours (BHAB) questionnaire to the Portuguese context and evaluate the methodological approaches used for this purpose. To guide this study, we followed a guideline for cross-cultural translation and adaptation measurement instruments in healthcare. The results of the content validity testing suggested that the BHAB questionnaire is a valid instrument for use in the Portuguese context. To obtain these results we showed 1) using of a new methodological approach, the dual focus, to resolve the divergences and ambiguities in the translators’ committee and the multi-professional committee; 2) the lack of a conceptual definition of the construct of the instrument as a requirement to retain items with I-CVI <0.70 after validity relevance pretesting and 3) the cognitive debriefing and relevance pretesting as methodological approaches which can be used alone or together to reinforce the evaluation of cultural relevance of the items. We concluded there is a need for guidelines to support the decision-making process of healthcare researchers with comprehensive information about the different methodological approaches they can follow.
  • The Integration of Information Technology in the Management and Organization of Nursing Care in a Hospital Environment: A Scoping Review

    The adoption of health technologies is occurring on an unprecedented scale, with enormous potential to improve the health of populations. In this context, information technology (IT) in nursing has emerged with a focus on quality and safety of care for the benefit of the patient. There is insufficient reliable evidence demonstrating how the integration of IT in nursing care influences methodologies for managing and organizing care in terms of structure and care practice, justifying a scoping review that synthesizes the knowledge produced so far. Online databases were used to identify papers published in 2012–2023, from which we selected nine publications that used information technology in the nursing care processes. The participants were hospital nurses and nurse managers. The results show that the integration of IT in healthcare organizations impacts the management and organization of nursing care, and changes in structure, process reorganization, management, training, and the development of nurses’ skills. To minimize this impact, the organizational structure must be prepared for a cultural change, with well-defined and communicated policies and procedures, and strong leadership. Within the teams, the importance of process reorganization, continuous training, and skill development emerges, thus enhancing the integration of IT into practice environments in conjunction with care.
  • How the Nursing Practice Environment Influences Retention and Turnover Intention: An Umbrella Review Protocol

    Currently, there is a global shortage of nurses, which has a negative impact on health institutions, mainly on the quality of care provided. The quality of nursing care depends on several factors, including the nursing practice environment, which has been stated as a fundamental element for the successful functioning of health systems. Scientific evidence shows that favorable practice environments contribute to nurse retention, and unfavorable environments increase nurses’ turnover intention. Retaining nurses is essential to ensure the sustainability of healthcare organizations and the quality of nurse care. In the current shortage scenario, this issue has become even more significant. Objective: This review aims to identify systematic literature reviews that describe the characteristics of nursing practice environments that contribute to nurse retention or turnover intention in hospitals. Methods: The umbrella review will follow the Joanna Briggs Institute methodology, the Preferred Reporting Items for Systematic Review and Meta-Analysis reporting guidelines, and the Preferred Reporting Items for Overviews of Reviews guidelines. A search with indexed terms will be performed in various databases, and two reviewers will identify, select, and extract studies, while a third reviewer will resolve any disagreements. Results: This protocol provides a structured approach for conducting the review, and the initial searches have identified 440 articles, with no previous protocol or umbrella review identified, underscoring the importance of this research. Conclusions: This study will enhance the dissemination of scientific evidence regarding nursing practice environments, thereby contributing to an improved understanding of factors affecting nurse retention and turnover intention.
  • Instruments for Patient Safety Assessment: A Scoping Review

    Background: Patient safety is an important component of healthcare service quality, and there are numerous instruments in the literature that measure patient safety. This scoping reviewaims to map the instruments/scales for assessing patient safety in healthcare services. Method: This scoping review follows the JBI methodology. The protocol was registered on the Open Science Framework. Eligibility criteria were defined based on studies that include instruments or scales for assessing patient safety in healthcare services, in any language, and without temporal restrictions. It adhered to all scoping review checklist items [PRISMA-ScR], with searches in the Embase, Lilacs, MedLine, and Scopus databases, as well as the repository of the Brazilian Digital Library of Theses and Dissertations. Two independent reviewers performed selection and data extraction in July 2023. Results: Of the 4019 potential titles, 63 studies reported on a total of 47 instruments/scales and 71 dimensions for patient safety assessment. The most-described dimensions were teamwork, professional satisfaction, safety climate, communication, and working conditions. Conclusion: The diversity of instruments and dimensions for patient safety assessment characterizes the multidimensionality and scope of patient safety. However, it hinders benchmarking between institutions and healthcare units.
  • Cultural Competence and Nursing Work Environment: Impact on Culturally Congruent Care in Portuguese Multicultural Healthcare Units

    Cultural competence is central to ensuring effective culturally congruent care to patients and fostering positive work environments, particularly in multicultural settings. Objective: This study aimed to analyse the relationship between cultural competence, the nursing work environment, and the delivery of culturally congruent care in multicultural units of a healthcare organisation in Portugal. Method: This was a quantitative, descriptive, and cross-sectional study, targeting nurses from multicultural units. Data were collected using both online and paper-based questionnaires, which included the Cultural Competence Questionnaire for Help Professionals, the Nursing Work Index—Revised Scale (NWI-R-PT), and a single question assessing nurses’ perceptions of the adequacy of the culturally congruent care they provide. Results: A moderate, positive correlation was identified between cultural competence and the Fundamentals for Nursing, while the nursing work environment was influenced by organisational support, professional category, and unit type. Discussion: The findings suggest that enhancing cultural knowledge and technical skills and reinforcing management support may positively impact culturally congruent care delivery in multicultural settings. Conclusion: This study contributes to nursing knowledge by highlighting the complex interactions between cultural competence and the work environment in Portuguese multicultural healthcare units. Future research should explore the impact of transcultural nursing leadership on multicultural work environments and in the delivery of culturally congruent care.
  • Impact of Nurse Manager’s Attributes on Multi-Cultural Nursing Teams: A Scoping Review

    As global migration increases, nurse managers’ effectiveness in multi-cultural nursing work environments is crucial due to the rising cultural diversity within healthcare teams. Despite the increasing international recruitment of qualified nurses to address the worldwide nursing shortage, no studies have synthesised the impact of nurse managers’ attributes on nurses in multi-cultural nursing teams. Therefore, it was conducted a literature review aimed to synthesise the available literature on how nurse managers’ personality traits, competencies, behaviours, and leadership styles influence nurse outcomes in multi-cultural nursing teams. Methods: Scoping review conducted according to the Joanna Briggs Institute guidelines to map the relationship or influence of nurse managers’ personality traits, competencies, behaviours, and leadership styles on the outcomes of nurses in multi-cultural settings across various clinical environments. Searches were conducted across electronic databases such as CINAHL and MEDLINE, along with grey literature. Results: This review included 39 studies, highlighting 29 personality traits, 9 competencies, 115 behaviours, and 5 leadership styles that impact nurses’ outcomes. Key findings emphasise the importance of nurse managers being supportive, culturally competent, and effective communicators, with transformational leadership style being particularly beneficial. Conclusions: These findings provide insights for planning and developing training programmes to equip current and future nurse managers with skills to effectively lead in multi-cultural care settings.
  • Escala de perceção individual da carga de trabalho

    A Individual Workload Perception Scale-Revised (IWPS-R) foi desenvolvida para ser uma ferramenta importante no fornecimento de informações necessárias para formular intervenções que visam melhorar a perceção dos enfermeiros sobre a sua carga de trabalho e, consequentemente, o ambiente da prática de enfermagem, promovendo, assim, a segurança e a qualidade dos cuidados. Em Portugal não existia uma escala válida e fiável que avaliasse a satisfação dos enfermeiros com a sua carga de trabalho. Consequentemente, foi decidido adaptar culturalmente/linguisticamente e validar a IWPS-R, dando origem à versão portuguesa da mesma. Assim, a versão portuguesa IWPS-R tem 5 dimensões, com um total de 21 itens. Integra cinco dimensões: o Suporte do Enfermeiro Gestor, o Suporte da Equipa, os Recursos da Organização, a Carga de Trabalho e a Intenção de Permanecer. A EPICT apresenta um Alfa de Cronbach de 0,88. É um instrumento de recolha de dados pertinente para o contexto da gestão e da investigação em enfermagem.

    A escala NWI-R-PT tem como finalidade conhecer os atributos organizacionais que caracterizam o ambiente de prática de enfermagem. É composta por 6 dimensões: “apoio da gestão”; “desenvolvimento profissional”; “fundamentos de enfermagem”; “relação enfermeiro-médico”; “dotações” e “organização dos cuidados de enfermagem”. Apresenta um Alfa de Cronbach para o instrumento total de 0,91. Em relação às dimensões, o Alpha é de 0,86 para a dimensão “apoio da gestão”; 0,81 para o “desenvolvimento profissional”; 0,69 para “fundamentos de enfermagem”; 0,77 para a “relação enfermeiro-médico”; 0,65 para a “dimensão dotações” e por fim 0,59 para a dimensão “organização dos cuidados de enfermagem”. A escala é válida e pode ser utilizada em todos os contextos na prática clínica, na gestão em enfermagem e na investigação. A sua utilização pode ter impacto na melhoria da qualidade e segurança dos cuidados e no desenvolvimento profissional dos enfermeiros.
