  • Análise do uso de suportes tecnológicos e conteúdos informacionais pelos pacientes com doença pulmonar obstrutiva crónica

    Objetivo: Identificar tipos de suportes tecnológicos e conteúdos informacionais utilizados na promoção da gestão do regime terapêutico em pacientes com DPOC. Métodos: Estudo de abordagem quantitativa, exploratório transversal e descritivo, tendo como recurso um questionário baseado no modelo de aceitação da tecnologia, no modelo teórico dos determinantes da percepção da facilidade de utilização da mesma e na revisão da literatura. Resultados: Os pacientes com DPOC referiram maior necessidade de informação relacionada com as atividades do dia-a-dia e com os exercícios respiratórios. Pacientes com maior literacia e maior literacia tecnológica referem maior utilidade das tecnologias Web-based; pacientes com menor literacia e menor literacia tecnológica referem maior utilidade, intenção e facilidade para a utilização do telemóvel, livros e vídeos no acesso à informação. Conclusão: Os recursos informacionais podem adotar diferentes suportes tecnológicos, desde que disponibilizados em função da literacia e literacia tecnológica e adequados às necessidades informacionais dos pacientes.
  • Instrumentos de avaliação da dor em pessoas com alteração da consciência: uma revisão sistemática

    In patients with impaired consciousness, pain self-assessment is impracticable and communication is compromised, therefore challenging assessment by health care professionals. This causes the use of valid and trustful scales to become fundamental. The present study aims to evaluate the clinical potential of the existent scales for the assessment of pain in patients with impaired consciousness. The literature review comprehends the time frame from January of 2005 to June of 2011 based on two search engines and three databases. A total of 654 abstracts and titles were analyzed, 16 papers were selected for a full body revision of which 8 comprise within the present review. Three university archives were visualized and only 1 paper was included herein. Seven distinct assessments of pain scales were identified in patients presenting impaired consciousness. Only one of the scales took into account physiological and behavioral indicators, whereas the remaining others included solely behavioral indicators. The BPS scale obtained the highest rating, thus turning its implementation possible in these patients. More research, concerning the effects of the use of assessment of pain tools in clinics and their implications, is required.

  • Representação do conhecimento em enfermagem do trabalho na ontologia de enfermagem

    Introduction: By analyzing the national parameterization of the Nursing Practice Support System, it was found that occupational health nursing has no representation in the current information systems in use, being urgent to structure the existing knowledge in this area. Objectives: Having as client the person in the work context, the intention was to formally represent the knowledge currently available in three classes of nursing information items that represent the critical elements of the decision-making process: i) data resulting from the assessment of the client, ii) diagnoses and iii) interventions. Methodology: This work originates from a global project developed by the Centre for Information Systems Research and Development of Porto Nursing School (CIDESI-ESEP), in collaboration with the Ordem dos Enfermeiros, which was consolidated in the construction of a Nursing Ontology (NursingOntos). For its conception, a qualitative study of inferential nature was carried out, with content analysis of the national parameterization and literature review. For content validation, a focus group was used. Results: Sixty-four items of information were identified, organized by domains, referring to work activity, occupational exposure and health-seeking behaviors: twenty-one data items, with about fifty specifications; ten nursing diagnoses; and thirty-three interventions. The care conception is centered on aspects that are part of the adaptive process of the client in a work environment, namely the awareness as a property, and knowledge and meaning as facilitating or inhibiting conditions of his transition. Conclusions: The Nursing Ontology is a fundamental resource in the backend of the development of nursing information systems, supporting the description of care and the decision-making process. Having as client the person in the work context, it was verified the centrality in the identification of data characterizing the physical intensity and exposure to risk factors of the work activity, and in the adaptive process related to areas that will influence the choice of behaviors that promote his health.
  • Estratégias de supervisão clínica: análise crítico-reflexiva das práticas

    Introdução: A supervisão clínica em enfermagem é um processo formal de acompanhamento da prática clínica, que visa promover o desenvolvimento profissional, a proteção dos clientes e a segurança dos cuidados, através de processos de reflexão e análise das práticas. Objetivo: Identificar estratégias de supervisão clínica em uso. Métodos: Estudo qualitativo, exploratório. Os participantes foram 42 Enfermeiros de três Centros de Saúde da região norte de Portugal. Recolha dos dados através de entrevista semiestruturada e análise efetuada segundo os princípios do método da grounded theory. Resultados: Da análise emergiu a categoria “Análise crítico-reflexiva das práticas” e respetivas dimensões e subcategorias. As dimensões identificadas foram: “Dimensão intrapessoal”, da qual emergiram duas subcategorias; e a “Dimensão interpessoal”, da qual emergiram dez subcategorias. Conclusão: Concluiu-se que existia um conjunto de estratégias de supervisão em uso, promotoras do desenvolvimento profissional, nomeadamente, a análise crítico-reflexiva das práticas. Contudo, constatou-se que havia um conjunto de constrangimentos de natureza individual e contextual que podiam interferir com a operacionalização desta estratégia, que exige serem refletidos.
  • Self‐Care Dependency Evaluation Form: Psychometric properties of the revised version with 27 items

    Abstract Introduction: The Self-Care Dependency Evaluation Form assesses dependency in performing self-care activities, but its original version is extensive and provides redundant information. The present study aims to scrutinise the items of the scale with the purpose of creating a revised version and to evaluate its psychometric properties. Methods: The study was conducted in two phases. In the first phase, an exploratory and correctional analysis of the items of the original form was performed from a database with 282 participants, followed by a review by a panel of experts who analysed the discriminatory ability and the contribution and relevance of each item, which resulted in the revised version. In the second phase, a new study with a sample comprising 150 participants was conducted to test the psychometric properties of the revised version. All ethical aspects and matters of confidentiality and privacy were assured. Results: The scale with 27 items shows good internal consistency, ranging from 0.67 (taking medication) to 0.96 (walking). It was moderately correlated with the Barthel Index and the Lawton and Brody Scale, proven to be a discriminatory measurement instrument. Discussion/Conclusion: This measure will enable health professionals to better evaluate self-care activities and provide more efficient, simple and effective prescriptions.
  • Estudo das propriedades psicométricas da Escala de Bem-Estar Psicológico de 42 itens

    CONTEXTO: O bem-estar psicológico é um constructo multidimensional que tem merecido atenção dos investigadores no âmbito da saúde mental positiva, na medida em que se relaciona com o desenvolvimento do potencial humano. OBJETIVO: Validar, para a população portuguesa, a versão reduzida de 42 itens da Escala de Bem-Estar Psicológico numa amostra de adultos e jovens adultos. METODOLOGIA: Recorremos à análise fatorial confirmatória e à validade de constructo numa amostra de 252 participantes recrutados a partir de uma instituição de ensino superior, sendo 48% estudantes e 52% docentes e não docentes, a maioria do sexo feminino com média de idade de 31,4 anos e uma escolaridade média de 15,5 anos. Os participantes preencheram um questionário através de uma plataforma informática. A Escala de Bem-Estar Psicológico é constituída por seis constructos teóricos medidos numa escala de resposta do tipo likert. RESULTADOS: A análise fatorial confirmatória mostra índices de ajustamento sofrível tendo, no entanto, melhorado com a eliminação de dois itens. As dimensões da escala apresentaram valores de coeficiente alfa de Cronbach com variação entre 0,71 e 0,82 e uma matriz de correlação que varia entre 0,31 a 0,73, com valores significativos ao nível de 0,01. Apenas a Autonomia se correlacionou com a dimensão Saúde Mental. CONCLUSÃO: A escala em estudo tem mostrado adequação para avaliar o bem-estar psicológico. Esta versão mostra ser discriminativa para a idade, género, escolaridade e condição de saúde mental e uma boa consistência interna. A estrutura fatorial da escala não mostra ser tão clara quanto o quadro teórico sugerido, requerendo novas investigações.
  • Nursing clinical practice changes to improve self-management in chronic obstructive pulmonary disease

    Aim: To propose nursing clinical practice changes to improve the development of patient self-management. Background: Chronic obstructive pulmonary disease is one of the main causes of chronic morbidity, loss of quality of life and high mortality rates. Introduction: Control of the disease’s progression, the preservation of autonomy in self-care and maintenance of quality of life are extremely challenging for patients to execute in their daily living. However, there is still little evidence to support nursing clinical practice changes to improve the development of self-management. Methods: A participatory action research study was performed in a medicine inpatient department and the outpatient unit of a Portuguese hospital. The sample comprised 52 nurses and 99 patients. For data collection, we used interviews, participant observation and content analysis. Results: The main elements of nursing clinical practice that were identified as a focus for improvement measures were the healthcare model, the organization of healthcare and the documentation of a support decision-making process. The specific guidelines, the provision of material to support decision-making and the optimization of information sharing between professionals positively influenced the change process. This change improved the development of self-management skills related to the awareness of the need for ‘change’, hope, involvement, knowledge and abilities. Discussion: The implemented changes have improved health-related behaviours and clinical outcomes. Conclusion: To support self-management development skills, an effective nursing clinical practice change is needed. This study has demonstrated the relevance of a portfolio of techniques and tools to help patients adopt healthy behaviours.
  • Nursing knowledge on skin ulcer healing: a living scoping review protocol

    Objective: This review aims to continuously map the nursing knowledge on skin ulcer healing in any context of care. Introduction: Chronic wounds are an increasing concern for society and health care providers. Pressure ulcers and venous ulcers, among others, have devastating effects on morbidity and quality of life and require a systematic approach. The nursing process is an important method that allows a better organization and overall care quality for a systematic and continuous professional approach to nursing management of skin ulcers. The integration of this nursing knowledge in informatics systems creates an opportunity to embed decision-support models in clinical activity, promoting evidence-based practice. Inclusion criteria: This scoping review will consider articles on nursing data, diagnosis, interventions, and outcomes focused on people with skin ulcers in all contexts of care. This review will include quantitative, qualitative, and mixed methods study designs as well as systematic reviews and dissertations. Methods: JBI’s scoping review guidance, as well as the Cochrane Collaboration’s guidance on living reviews, will be followed to meet the review’s objective. Screening of new literature will be performed regularly, with the review updated according to new findings. The search strategy will map published and unpublished studies. The databases to be searched include MEDLINE, CINAHL, Scopus, JBI Evidence Synthesis, Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, and PEDro. Searches for unpublished studies will include OpenGrey and Reposito´ rios Cientı´ficos de Acesso Aberto de Portugal. Studies published in English and Portuguese since 2010 will be considered for inclusion.
