- Contributions of mindfulness in high-risk pregnancy: a mixed methods systematic reviewPublication . Dutra, Irina Neves; Seixinho, Sandra; Baixinho, Cristina; Presado, Maria Helena; García-Fernández, RubénIntroduction Experiencing a high-risk pregnancy entails pathological situations with repercussions that can interfere with maternal-fetal well-being and increase maternal anxiety. Mindfulness emerges as one of the techniques used to improve the quality of care in the transition period resulting from pregnancy, which, through meditative practice, seeks to understand observation and the nature of the lived experience, resulting in a progressive state of clarity and awareness. Objective This review assessed the contributions of applying mindfulness in high-risk pregnancies. The review considered experimental, quasi-experimental studies, randomized controlled trials and non-randomized controlled trials. Observational studies were also considered. Evidence published in the last 5 years was considered. Methods This systematic review was conducted in accordance with the Joanna Briggs Institute methodology for systematic reviews of mixed methods using a convergent integrated approach to synthesis and integration. We searched for published and unpublished English, Portuguese and Spanish-language studies and grey literature. CINAHL Ultimate, MEDLINE (via Pubmed), Web of Science, Academic Search Complete, EBSCO Host Open Dissertations and Open Access Theses and Dissertations were searched in April–May 2024. Two authors screened titles and abstracts before full-text screening and data extraction. Two authors reviewed the extracted data. Results Ten articles were included in the review: 5 randomized controlled trials, 3 quasi-experimental studies and 2 mixed-methods studies. 9 studies measured the effectiveness of the application of mindfulness-based interventions on high-risk pregnant women and 1 assessed the acceptance of mindfulness intervention. Six studies measured the effects of the application of mindfulness in high-risk pregnant women in depressive symptoms, two reported the outcomes in stress, five on anxiety, two on psychological well-being, one on gestational weight gain, one in prenatal attachment and another in sleep quality. Conclusions Long term reduction of depressive symptoms, anxiety and stress and increased psychological well-being are contributions of mindfulness on high-risk pregnant women. The existing evidence on other contributions of the application of mindfulness is limited, further research being necessary.
- Conversational agents for health and well-being across the life course: protocol for an evidence mapPublication . Guerreiro, Mara Pereira; Angelini, Leonardo; Henriques, Helga Rafael; Kamali, Mira El; Baixinho, Cristina; Balsa, João; Félix, Isa brito; Khaled, Omar Abou; Carmo, Maria Beatriz; Cláudio, Ana Paula; Caon, Maurizio; Daher, Karl; Alexandre, Bruno; Padinha, Mafalda; Mugellini, ElenaBackground: Conversational agents, which we defined as computer programs that are designed to simulate two-way human conversation by using language and are potentially supplemented with nonlanguage modalities, offer promising avenues for health interventions for different populations across the life course. There is a lack of open-access and user-friendly resources for identifying research trends and gaps and pinpointing expertise across international centers. Objective: Our aim is to provide an overview of all relevant evidence on conversational agents for health and well-being across the life course. Specifically, our objectives are to identify, categorize, and synthesize—through visual formats and a searchable database—primary studies and reviews in this research field. Methods: An evidence map was selected as the type of literature review to be conducted, as it optimally corresponded to our aim. We systematically searched 8 databases (MEDLINE; CINAHL; Web of Science; Scopus; the Cochrane, ACM, IEEE, and Joanna Briggs Institute databases; and Google Scholar). We will perform backward citation searching on all included studies. The first stage of a double-stage screening procedure, which was based on abstracts and titles only, was conducted by using predetermined eligibility criteria for primary studies and reviews. An operational screening procedure was developed for streamlined and consistent screening across the team. Double data extraction will be performed with previously piloted data collection forms. We will appraise systematic reviews by using A Measurement Tool to Assess Systematic Reviews (AMSTAR) 2. Primary studies and reviews will be assessed separately in the analysis. Data will be synthesized through descriptive statistics, bivariate statistics, and subgroup analysis (if appropriate) and through high-level maps such as scatter and bubble charts. The development of the searchable database will be informed by the research questions and data extraction forms. Results:As of April 2021, the literature search in the eight databases was concluded, yielding a total of 16,351 records. The first stage of screening, which was based on abstracts and titles only, resulted in the selection of 1282 records of primary studies and 151 records of reviews. These will be subjected to second-stage screening. A glossary with operational definitions for supporting the study selection and data extraction stages was drafted. The anticipated completion date is October 2021. Conclusions: Our wider definition of a conversational agent and the broad scope of our evidence map will explicate trends and gaps in this field of research. Additionally, our evidence map and searchable database of studies will help researchers to avoid fragmented research efforts and wasteful redundancies. Finally, as part of the Harnessing the Power of Conversational e-Coaches for Health and Well-being Through Swiss-Portuguese Collaboration project, our work will also inform the development of an international taxonomy on conversational agents for health and well-being, thereby contributing to terminology standardization and categorization.

- How the Nursing Practice Environment Influences Retention and Turnover Intention: An Umbrella Review ProtocolPublication . Figueiredo, Ana Rita; Gaspar, Filomena; Baixinho, Cristina; Lucas, PedroCurrently, there is a global shortage of nurses, which has a negative impact on health institutions, mainly on the quality of care provided. The quality of nursing care depends on several factors, including the nursing practice environment, which has been stated as a fundamental element for the successful functioning of health systems. Scientific evidence shows that favorable practice environments contribute to nurse retention, and unfavorable environments increase nurses’ turnover intention. Retaining nurses is essential to ensure the sustainability of healthcare organizations and the quality of nurse care. In the current shortage scenario, this issue has become even more significant. Objective: This review aims to identify systematic literature reviews that describe the characteristics of nursing practice environments that contribute to nurse retention or turnover intention in hospitals. Methods: The umbrella review will follow the Joanna Briggs Institute methodology, the Preferred Reporting Items for Systematic Review and Meta-Analysis reporting guidelines, and the Preferred Reporting Items for Overviews of Reviews guidelines. A search with indexed terms will be performed in various databases, and two reviewers will identify, select, and extract studies, while a third reviewer will resolve any disagreements. Results: This protocol provides a structured approach for conducting the review, and the initial searches have identified 440 articles, with no previous protocol or umbrella review identified, underscoring the importance of this research. Conclusions: This study will enhance the dissemination of scientific evidence regarding nursing practice environments, thereby contributing to an improved understanding of factors affecting nurse retention and turnover intention.

A Organização Mundial de Saúde consciente da necessidade de se desenvolverem ecossistemas de evidência clínicos aponta uma série de recomendações; Objetivos: Este artigo pretende responder à questão: ‘Quais os contributos da pesquisa qualitativa para o desenvolvimento de ecossistemas de evidência na clínica em saúde? Métodos: A partir da revisão da literatura e da experiência dos autores (com a investigação qualitativa e a translação do conhecimento) pretende-se explorar esta temática, quais as vantagens da investigação qualitativa, a importância da sua aplicação no cuidado centrado na pessoa e no desenvolvimento dos ecossistemas de evidência. Resultados. Uma visão diferenciadora sobre a investigação e os modelos de translação de conhecimento contribui para o desenvolvimento de ecossistemas de evidência, onde desde o desenho do estudo ao uso dos resultados de investigação há coconstrução e pelos diferentes atores do processo: cidadãos, profissionais de saúde, gestores e decisores políticos. Conclusões. A investigação qualitativa permite o cuidado centrado, o desenho de intervenções que tenham em conta as preferências e crenças da pessoa, contribuindo para uma prática baseada na evidência e para o desenvolvimento dos ecossistemas de evidência.
- A primeira experiência clínica do estudante de enfermagemPublication . Cabete, Dulce; Alves, Patricia; Baixinho, Cristina; Rafael, Helga; Viegas, LauraIntrodução: O ensino clínico, parte essencial da formação em enfermagem, expõe desde cedo o estudante à realidade dos cuidados e da profissão. Reconhecendo que, num contexto de mudanças sociais e profissionais com reflexo, também, na estrutura e organização do curso de enfermagem, é fundamental analisar e adequar a prática pedagógica para maximizar aprendizagens. Objetivo: Pretendemos, com este estudo, descrever a estrutura essencial da primeira experiência clínica do estudante de enfermagem. Método: Trata-se de um estudo descritivo, retrospetivo, de abordagem fenomenológica. Após a primeira incursão em ensino clínico, realizada no primeiro ano do curso, com a duração de uma semana, os estudantes foram convidados a participar no estudo. Dos quarenta estudantes matriculados, onze compareceram para marcação de entrevista individual, constituindo, assim, o grupo em estudo. A recolha de dados foi feita através de entrevista não estruturada. As entrevistas foram gravadas em áudio e transcritas integralmente, dando origem a onze verbatins. Foram salvaguardados os aspetos da confidencialidade e da participação livre e esclarecida. As entrevistas foram conduzidas por uma docente e decorreram após a avaliação e classificação daqueles estudantes na Unidade Curricular. Resultados: A análise fenomenológica conduziu à descrição da estrutura da primeira experiência clínica como um processo de tomada de consciência da identidade profissional, das competências desenvolvidas e a desenvolver, do processo de aprendizagem e das emoções e sentimentos envolvendo a experiência. Conclusão: Salienta-se a natureza confirmatória desta experiência para a confirmação da escolha do curso, a necessidade de o estudante se sentir acolhido e reconhecido pelo cliente bem como uma componente fortemente emocional acompanhando toda a experiência. Importa aprofundar estes aspetos com outros estudos que possam determinar se as dimensões identificadas são específicas do primeiro ensino clínico, bem como integrar os resultados na melhoria do processo educativo.
