  • Representação do conhecimento em enfermagem do trabalho na ontologia de enfermagem

    Introduction: By analyzing the national parameterization of the Nursing Practice Support System, it was found that occupational health nursing has no representation in the current information systems in use, being urgent to structure the existing knowledge in this area. Objectives: Having as client the person in the work context, the intention was to formally represent the knowledge currently available in three classes of nursing information items that represent the critical elements of the decision-making process: i) data resulting from the assessment of the client, ii) diagnoses and iii) interventions. Methodology: This work originates from a global project developed by the Centre for Information Systems Research and Development of Porto Nursing School (CIDESI-ESEP), in collaboration with the Ordem dos Enfermeiros, which was consolidated in the construction of a Nursing Ontology (NursingOntos). For its conception, a qualitative study of inferential nature was carried out, with content analysis of the national parameterization and literature review. For content validation, a focus group was used. Results: Sixty-four items of information were identified, organized by domains, referring to work activity, occupational exposure and health-seeking behaviors: twenty-one data items, with about fifty specifications; ten nursing diagnoses; and thirty-three interventions. The care conception is centered on aspects that are part of the adaptive process of the client in a work environment, namely the awareness as a property, and knowledge and meaning as facilitating or inhibiting conditions of his transition. Conclusions: The Nursing Ontology is a fundamental resource in the backend of the development of nursing information systems, supporting the description of care and the decision-making process. Having as client the person in the work context, it was verified the centrality in the identification of data characterizing the physical intensity and exposure to risk factors of the work activity, and in the adaptive process related to areas that will influence the choice of behaviors that promote his health.

    Introduction: Occupational health presents itself as a whole area to ensure healthy work environments and a better quality of life for workers. In the information systems in use, it was found that this area was missing, and a representation of existing knowledge in occupational nursing is urgent, particularly concerning the data that allow the characterization of health needs in a work environment. Objectives: Formally represent the available knowledge related to data relating to the assessment of the client under the influence of the work environment, which represent the first element of the planning and description of the nursing care process, thus constituting a fundamental resource for the design of clinical decision support systems. Methodology: This work derives from a global project developed by the Center for Research and Development of Information Systems at the Porto School of Nursing, in collaboration with the Ordem dos Enfermeiros, which consolidated itself in the construction of a Nursing Ontology. A qualitative study of inferential nature was carried out for its conception, with content analysis to national parameterization and literature review. For content validation, a focus group was used. Results: Nineteen assessment data were identified, most of which could generate sensitive indicators for nursing care, organized into three domains: i) data related to the characterization of the work activity, its context and physical intensity; ii) data related to occupational exposure potentially leading to changes in bodily and psychological processes; and iii) data that characterize aspects related to health-seeking behaviours related to the adaptive process (transition) of the client in the work context, such as knowledge and awareness. Conclusions: The identification of data is decisive for the diagnostic process, with centrality in the characterization of the data centred on physical intensity and exposure to risk factors in the work activity and the adaptive process related to areas that will influence the choice of health-promoting behaviours.
  • Representação do conhecimento em enfermagem – a família como cliente

    Enquadramento: a família é o contexto privilegiado de cuidados de suporte à vida e saúde dos indivíduos. A abordagem sistémica da família tem, conceptualmente vindo a ganhar projeção. A Ontologia de Enfermagem, propõe uma estrutura onde os conceitos da disciplina e as suas relações são especificados, descrevendo uma representação formal do conhecimento de enfermagem. Objetivos: representar o conhecimento em enfermagem sobre a família como cliente dos cuidados, em três classes de itens de informação: dados, diagnósticos e intervenções. Metodologia: estudo qualitativo inferencial; realizada análise documental à parametrização do Sistema de Apoio à Prática de Enfermagem - SAPE®, revisão da literatura e focus group. Resultados: na parametrização nacional constatou-se pouca visibilidade da família como unidade de cuidados bem como a ausência de representação dos modelos concetuais que suportam o conhecimento disciplinar. Na Ontologia de Enfermagem, o processo familiar engloba: organização do funcionamento da casa e edifício residencial; preparação da família para integrar um familiar dependente no autocuidado; preparação da família para a chegada do recém-nascido; e, planeamento familiar. Para cada domínio foram especificadas três classes de informações de enfermagem. Conclusão: Este estudo apresenta-se como um contributo para a formalização do conhecimento de enfermagem no domínio do Processo Familiar.
  • O Empowerment na doença cónica: revisão integrativa da literatura

    A intencionalidade da intervenção de Enfermagem na doença crónica é capacitar a pessoa a tomar decisões informadas e gerir a sua doença crónica. O Empowerment poderá ser um dos resultados desejáveis no processo de autogestão da doença crónica. Com o intuito de aprofundar a problemática optou-se por elaborar a revisão integrativa da literatura. Objetivo: Caracterizar e sintetizar a produção científica relativamente ao conceito de Empowerment e sua aplicação às situações crónicas de saúde no período de 2007-2013, no cuidado de Enfermagem. Metodologia: Revisão integrativa da literatura. Resultados: Identificação de 27 artigos que corresponderam aos critérios previamente estabelecidos, os mesmos caracterizam o Empowerment , no contexto da doença crónica, como sendo um processo, um resultado e uma intervenção educativa participativa. Conclusões: A evidência científica nesta área aponta para a necessidade de desenvolver e aprofundar o conhecimento da mesma, sobretudo da sua aplicação à Enfermagem, como disciplina, profissão e área do conhecimento. A avaliação do Empowerment como resultado permite identificar estratégias e intervenções de Enfermagem inovadoras que contribuam para a capacitação em saúde.
  • Empowerment comunitário em saúde escolar – adolescente com diabetes mellitus tipo 1

    Type 1 Diabetes Mellitus (DM1) is one of the most prevalent chronic diseases in school age. Health policies point to a greater intervention and accountability of the community towards the health of its members and their full integration in society, promoting the development of health literacy based on an empowerment philosophy. Since the school is a favorable context for intervention, this study, through a review of the literature and based on the Empowerment Nursing-User Model and Laverack Community Empowerment Model, aims to elaborate a Nursing Care Plan Model aimed at training the school community with adolescents with DM1, using ICNP 2017. Agglutinating the first phase of the Laverack Community Empowerment Model (personal action) with the examples of Empowerment Nursing-User Model empowering behaviors (access to information, support, resources, opportunities to learn and grow, informal power and formal power), we identified diagnoses as: potential to raise awareness of the relationship between the therapeutic regimen and DM1 control and potentiality to improve the problematic meaning attributed to the therapeutic regime or illness by the adolescent. According to the phases of approach of small community groups and development of community organizations, we identified diagnoses aimed at the school community and the family like the potential to improve awareness of their role towards the adolescent with DM1. Community empowerment is an instrument to be used in the development of the process of school inclusion and training adolescents with DM1 as well as the whole school community.
  • Nursing knowledge on skin ulcer healing: a living scoping review protocol

    Objective: This review aims to continuously map the nursing knowledge on skin ulcer healing in any context of care. Introduction: Chronic wounds are an increasing concern for society and health care providers. Pressure ulcers and venous ulcers, among others, have devastating effects on morbidity and quality of life and require a systematic approach. The nursing process is an important method that allows a better organization and overall care quality for a systematic and continuous professional approach to nursing management of skin ulcers. The integration of this nursing knowledge in informatics systems creates an opportunity to embed decision-support models in clinical activity, promoting evidence-based practice. Inclusion criteria: This scoping review will consider articles on nursing data, diagnosis, interventions, and outcomes focused on people with skin ulcers in all contexts of care. This review will include quantitative, qualitative, and mixed methods study designs as well as systematic reviews and dissertations. Methods: JBI’s scoping review guidance, as well as the Cochrane Collaboration’s guidance on living reviews, will be followed to meet the review’s objective. Screening of new literature will be performed regularly, with the review updated according to new findings. The search strategy will map published and unpublished studies. The databases to be searched include MEDLINE, CINAHL, Scopus, JBI Evidence Synthesis, Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, and PEDro. Searches for unpublished studies will include OpenGrey and Reposito´ rios Cientı´ficos de Acesso Aberto de Portugal. Studies published in English and Portuguese since 2010 will be considered for inclusion.

  • Construção e validação da Escala de Empowerment Individual no contexto da doença crónica

    Resumo: Enquadramento: A avaliação do empowerment poderá ser um indicador determinante para a obtenção de ganhos em saúde. Objetivos: Construir e validar uma escala de empowerment individual no contexto da pessoa com doença crónica. Metodologia: O instrumento foi construído e aplicado a uma amostra de conveniência de 271 pessoas com doença crónica, com idades compreendidas entre os 18 e os 65 anos residentes em Lisboa. Para avaliar as propriedades métricas da escala recorreu-se à validação da fiabilidade do instrumento. Resultados: Obteve-se uma escala de 25 itens com 7 dimensões: Autoperceção; Participação nas decisões em saúde; Mestria; Determinação; Identidade; Autonomia e poder; Relação com os profissionais de saúde. A análise fatorial (componentes principais- Varimax) revelou que os que os 25 itens se organizaram em 7 fatores com uma variância explicada total de 65,28% e alfa de Cronbach total de 0,803. Conclusão: A criação de uma medida válida e fiável de empowerment no contexto da doença crónica poderá ajudará os profissionais de saúde a explorar o impacto deste resultado na autogestão dos regimes terapêuticos.
