Abstract(s)
O serviço de urgência é, muitas das vezes, a linha da frente no que diz respeito ao contato do doente com o serviço de saúde, verificando-se uma afluência crescente e um aumento das exigências de qualidade a par da necessidade da redu ção de custos.A implementação de estratégias em prol da sustentabilida de do sistema não se pode focar apenas na contenção de custos per si. Há, necessariamente, que entender o conjunto, do ponto de vista do utente que o procura, com todas as suas expetativas e emoções, de acordo com as suas condições de acessibilidade físicas e económicas; e do ponto de vista do profissional de saúde, como elemento de equipa de trabalho multidisciplinar, num domínio dinâmico em constante permuta, com ambições e frustrações, sujeito a pressões, a conflitos e à necessidade de uma comunicação permanente, universal e eficaz. Todo o sistema está ainda sujeito à política dos seus gestores, que devem ter um papel proativo e não reativo, as sumindo as suas inseguranças perante esta porta de entrada hospitalar de difícil controlo.
Qualquer intervenção neste campo tem sempre que ter em consideração a implantação física dos serviços de urgência, acessibilidade, capacidade de resposta em situações críticas e condições de trabalho, pilares que vão determinar o nível de cuidados lá prestados e consequente procura. Neste ambiente caótico, é fundamental uma boa liderança,em que o líder não se limita a ser exemplo de caráter, respeito e justiça, mas é também um gestor e um comunicador, que cria condições a uma aprendizagem adaptativa e expansiva, que
possa contribuir para a melhoria da prestação de cuidados.
The emergency department is often the front line in the pa tient’s contact with the health service, with a growing af fluence and increased quality requirements along with the need for cost reduction. The implementation of strategies for the sustainability of the system cannot focus only on cost containment itself. It is necessary to understand the big picture by the user’s point of view, with all its expectations and emotions, according to its conditions of physical and economic accessibility; and from the standpoint of health professionals as multidisci plinary work team elements, in a dynamic area in constant exchange, with ambitions and frustrations, under pressure and conflicts and with the need for constant, universal and effective communication. The entire system is subject to the policy of the managers who must have a proactive and not reactive role, accepting their insecurities towards this, diffi cult to control, hospital entryway. Any intervention in this field must always take into ac count the physical deployment of emergency services, their accessibility, responsiveness in critical situations and working conditions, pillars that will determine the level of care provided there and consequent demand. In this chaotic environment, good leadership is essential, in which the leader is not only an example of character, respect and justice, but also a manager and a communica tor, which creates conditions for an adaptive and expansive learning, which can contribute to improving care
