Com a transição demográfica em curso, o número de idosos está a aumentar, o que implica uma maior dependência no autocuidado e mudanças no quotidiano familiar, sendo necessário planear e implementar intervenções adequadas às especificidades e complexidades do sistema familiar, para que o funcionamento familiar e a saúde dos seus membros, não sejam comprometidos.
No âmbito da componente clínica do ciclo de estudos do Mestrado em Enfermagem Comunitária na área de Enfermagem de Saúde Familiar, da Escola Superior de Enfermagem do Porto, este relatório teve como objetivo apresentar e descrever as atividades desenvolvidas, baseadas num modelo teórico de Enfermagem, evidenciando o processo de desenvolvimento de competências comuns do enfermeiro especialista (Regulamento n.º 140/2019 da Ordem dos Enfermeiros ) e específicas do enfermeiro especialista em enfermagem comunitária na área de enfermagem de saúde familiar (Regulamento n.º 428/2018 da Ordem dos Enfermeiros). Para a concretização do desenvolvimento de competências foram definidos os seguintes objetivos: prestar cuidados a famílias com membros idosos dependentes, enquanto unidade de cuidados bem como dos seus membros, ao longo do ciclo vital e nos diferentes níveis de prevenção e, promover a liderança e a colaboração em processos de intervenção de saúde familiar.
A importância de modelos teóricos, particularmente o Modelo Dinâmico de Avaliação e Intervenção Familiar (MDAIF), que direcionam e sustentam a conceção de cuidados de enfermagem às famílias e aos seus membros, ficou evidente através da realização deste estágio, enfatizando a importância de uma prática baseada em evidência e voltada para ganhos em saúde, permitindo uma perspetiva sistémica dinâmica e flexível na avaliação e intervenção em famílias com membros idosos dependentes. Outros contributos significativos surgiram por meio do processo formativo facultado aos enfermeiros relativo à ESF, bem como, o trabalho desenvolvido com a equipa parametrizadora para a otimização do suporte documental no SClínico CSP, a introdução de objetivos para a avaliação de desempenho, a realização de um protocolo de Scoping Review, e a participação em eventos científicos.
Contribuiu-se assim, para a demonstração científica da prestação de cuidados às famílias com membros idosos dependentes, no contexto dos cuidados de saúde primários, suportada pela evidência científica atual, concomitantemente com o desenvolvimento pessoal e profissional.
Algumas implicações e recomendações importantes foram identificadas, como a necessidade de aprofundar a investigação sobre a abordagem sistémica às famílias e a utilização do MDAIF, sendo crucial para uma atuação cada vez mais embasada em evidências efetivas na enfermagem de saúde familiar.
With the ongoing demographic transition, the number of elderly individuals is increasing, implying a greater reliance on self-care and changes in family daily life. It is necessary to plan and implement interventions tailored to the specificities and complexities of the family system, so that family functioning and the health of its members are not compromised. Within the clinical component of the Master's degree program in Community Nursing in the area of Family Health Nursing at the Porto School of Nursing, this report aimed to present and describe the activities carried out, based on a theoretical Nursing model, highlighting the process of developing common skills of the specialist nurse (Ordinance No. 140/2019 of the Order of Nurses) and specific skills of the specialist nurse in community nursing in the area of family health nursing (Ordinance No. 428/2018 of the Order of Nurses). The following objectives were defined for the development of skills: providing care to families with dependent elderly members, as a unit of care, as well as to its individual members, throughout the life cycle and at different levels of prevention, and promoting leadership and collaboration in family health intervention processes. The importance of theoretical models, particularly the Dynamic Model for Family Assessment and Intervention (DMFAI), which guide and support the conception of nursing care for families and their members, became evident through the completion of this internship, emphasizing the importance of evidence-based practice focused on health gains, allowing for a dynamic and flexible systemic perspective in the assessment and intervention of families with dependent elderly members. Other significant contributions arose from the training process provided to nurses regarding Family Health Nursing, as well as the work carried out with the standardizing team to optimize the documentary support in the SClinico CSP, the introduction of performance evaluation objectives, the conduct of a Scoping Review protocol, and participation in scientific events. Thus, a scientific demonstration was made of the provision of care to families with dependent elderly members in the context of primary health care, supported by current scientific evidence, simultaneously contributing to personal and professional development. Some important implications and recommendations were identified, such as the need to further research the systemic approach to families and the use of DMFAI, which is crucial for an increasingly evidence-based practice in family health nursing.
