Abstract(s)
A alta hospitalar Ć© um momento de transição crĆtica na gestĆ£o da saĆŗde dos
doentes. A precocidade com que as altas são dadas após cirurgia tende a complicar
a sua preparação, devendo os enfermeiros serem capazes de recolher e analisar
informação pertinente para uma adequada organização da alta de enfermagem. Os
enfermeiros devem ter um papel ativo nas estratégias de transição dos cuidados a
fim de aprimorar a preparação da alta e garantir a continuidade dos cuidados no
domicĆlio.
Este projeto é desenvolvido no âmbito da unidade curricular de EstÔgio com
Relatório do 12º Curso de Mestrado na Ôrea de Especialização em Enfermagem
Médico-Cirúrgica, vertente Enfermagem Oncológica. Pretende dar visibilidade à s
intervenções autónomas do enfermeiro especialista na capacitação para o
autocuidado do doente hepato-bilio-pancreƔtico, constituindo ainda um importante
contributo na construção de estratĆ©gias de melhoria de cuidados no perĆodo
perioperatório. A sua finalidade é promover o autocuidado na preparação para a alta
do doente hepato-bilio-pancreƔtico submetido a cirurgia, implementar uma consulta
de enfermagem de follow-up telefónica e prevenir reinternamentos hospitalares.
Neste sentido, pretendo dar resposta Ć seguinte questĆ£o de investigação: āQuais as
intervenƧƵes de enfermagem que permitem capacitar o doente hepato-bilio pancreĆ”tico submetido a cirurgia, com vista a uma alta segura para a comunidade?ā
A metodologia foi sustentada pela pesquisa bibliogrÔfica e a realização dos estÔgios
onde adquiri e consolidei conhecimentos e estratƩgias adotadas para a
implementação da consulta de enfermagem follow-up telefónica.
A capacitação do doente para o autocuidado, desde a admissĆ£o, perĆodo
perioperatório e acompanhamento pós-alta revela-se uma intervenção de
enfermagem decisiva. A consulta de enfermagem de follow-up telefónica e a
avaliação do enfermeiro sobre os problemas ou necessidades identificados mostrou se eficaz na sua resolução. Ć considerada pertinente nos benefĆcios que proporciona
ao doente e famĆlia e no desempenho dos enfermeiros, que assumem um papel
dinamizador relativamente ao ensino, à promoção do autocuidado ao longo da
preparação para a alta hospitalar e continuidade dos cuidados.
Hospital discharge is a time of critical transition in patient health management. The precocity with which discharges are given after surgery tends to complicate their preparation, and nurses should be able to collect and analyse pertinent information for an adequate organization of nursing discharge. Nurses should play an active role in care transition strategies to improve discharge preparation and ensure continuity of care at home. This project is developed within the scope of the curricular unit of internship with report of the 12th Master in Medical Surgery Nursing Specialization, Oncological Nursing branch, for the Nurse Specialist and Master degree title. It intends to give visibility to the autonomous interventions of the specialist nurse in the training for the self-care of the hepatic-bile-pancreatic patient, constituting an important contribution in the construction of strategies to improve care in the perioperative period. The purpose is to promote self-care in preparation for the discharge of hepatic-bile pancreatic patients undergoing surgery, to implement a telephone follow-up nursing consultation and to prevent hospital readmissions. In this sense, I intend to answer the following research question: āWhat are the nursing interventions that allow training the hepatic- bile- pancreatic patient undergoing surgery, with a view to a safe discharge to the community?ā The methodology was supported by bibliographic research and the completion of internships where I acquired and consolidated knowledge and strategies adopted for the implementation of the telephone follow-up nursing consultation. Patient empowerment for self-care, from admission, perioperative period and post discharge follow-up, proves to be a decisive nursing intervention. The telephone follow-up nursing consultation and the nurse's assessment of the identified problems or needs proved to be effective in their resolution. It is considered relevant in the benefits it provides to the patient and family and in the performance of nurses, who assume a dynamic role in terms of teaching, promoting self-care throughout the preparation for hospital discharge and continuity of care.
