Fevereiro-Martins, M; Santos, AC; Marques-Neves, C; Guimarães, H; Bicho, M; Afonso, C; Ferreira, J; Espírito Santo, R; Teixeira, F; Rosa, R; Carneiro, CV; Ferreira, M; Matos, T; Neiva, L; Pereira, S; Aires, S; Parreira, R; Melnik, Z; Faria, J; Teixeira, J; Barros, P; Almeida, J; Malheiro, B; Rodrigues, PC; Albuquerque, L; Freitas, A; Barros, P; Kotchekova, N; Freitas, R; Silveira, AC; Ferreira, A; Morais, B; Teixeira, S; Mota, M; Guerra, M; Coimbra, L; Gigante, J; Ferreira, M; Lapa, P; Monteiro, M; Alfaiate, M; Rodrigues, T; Pina, T; Rosário, M; Silva, R; Breda, J; Bazenga, F; Pinto, JAPurpose: To evaluate complete blood count (CBC) parameters in the first week of life as predictive biomarkers for the development of retinopathy of prematurity (ROP). Methods: Multicenter, prospective, observational study of a cohort of preterm infants born with gestational age (GA) < 32 weeks or birth weight < 1500 g in eight Portuguese neonatal intensive care units. All demographic, clinical, and laboratory data from the first week of life were collected. Univariate logistic regression was used to assess risk factors for ROP and then multivariate regression was performed. Results: A total of 455 infants were included in the study. The median GA was 29.6 weeks, and the median birth weight was 1295 g. One hundred and seventy-two infants (37.8%) developed ROP. Median values of erythrocytes (p < 0.001), hemoglobin (p < 0.001), hematocrit (p < 0.001), mean corpuscular hemoglobin concentration (p < 0.001), lymphocytes (p = 0.035), and platelets (p = 0.003) of the group of infants diagnosed with ROP any stage were lower than those without ROP. Mean corpuscular volume (MCV) (p = 0.044), red blood cell distribution width (RDW) (p < 0.001), erythroblasts (p < 0.001), neutrophils (p = 0.030), neutrophils-lymphocytes ratio (p = 0.028), and basophils (p = 0.003) were higher in the ROP group. Higher values of MCV, erythroblasts, and basophils remained significantly associated with ROP after multivariate regression. Conclusion: In our cohort, the increase in erythroblasts, MCV, and basophils in the first week of life was significantly and independently associated with the development of ROP. These CBC parameters may be early predictive biomarkers for ROP.
- Creating and implementing a system of geolocation for physiotherapy servicesPublication . Alves Lopes, António; Borges, P; Almeida, J; Correia, RThe aim of this work was to create a geolocation system for Physiotherapy services. This system will enable Physiotherapists, Physiotherapy clinics and users to locate different Physiotherapy services according to the geographical location. The specific objectives were to create a model of indexing information for Physiotherapists based on their geolocation, personal data, areas of intervention and service agreements; creation of a model of information search; and implementation of a web service that allows indexing and information search of Physiotherapy services for both users and clinicians, based on this various parameters. There are many needs among decision makers regarding the modelling and presentation of health information for better understanding of the phenomena and better management of all resources. The geolocation (Web-based mapping) stands out as an important tool that allows users through systems like Google Maps or Yahoo Maps to access, acknowledge and manage services or monitoring diseases or epidemics. In the context of health care these systems allow a better allocation of resources by health systems as well as a selection and more informed choice by the users. The system methodology involved several stages: 1º Step: the list of requirements was create with the assistance of a group of Physiotherapists, it was determined the set of information considered useful to the end user when viewing the system, including: General information - Physiotherapist personal data, including relevant contacts; Clinical Information - refers to the types of available valances within Physiotherapy services; Administrative Information - refers to agreements and contracts with external entities. The definition of data took into account the end user, the vocabulary was adapted for Physiotherapists and users. 2º Step: Design, system architecture and the creation of the system interface, in both frontend and backend, we opted for the PHP language for the management system and MySQL for the database. 3º Step: The system was set up in a subdomain with its own web access in the last phase of implementation. Users of the system were divided into two types: User looking for Physiotherapy services - this includes patients and other Physiotherapists who refer their clients to other services and users seeking to advertise their Physiotherapy services - this includes Physiotherapists and clinics wishing to register their information on the system aiming to facilitate contacts.Several benefits of using this system were described, including easy management and availability of geographical information for patients and other health professionals, that could promote a possible decrease in costs, time and resources saving, both human and economic in health services access.Today, ICT are increasingly present in the practice of the Physiotherapist, covering new areas and putting at their disposal a wide range of solutions and tools for clinical practice. This system will enable Physiotherapists, Physiotherapy clinics and users to locate different Physiotherapy services, fostering an environment of increasingly shared and equal access to care facilities.
- Images in cardiovascular medicine. Accessory mitral valve with cordal attachments to mitral and aortic valves: an unusual cause of left ventricular outflow tract obstruction and both mitral and aortic insufficienciesPublication . Gaspar, A; Almeida, J; Marinho, B; Monteiro, V; Abreu, A; Pinho, P
- Ischemic rupture of the anterolateral papillary musclePublication . Vieira, C; Gaspar, A; Álvares-Pereira, M; Salomé, N; Almeida, J; Amorim, MJWe describe the case of a 59-year-old man who presented with chest pain and ST-segment elevation in the inferior leads, R>S in V1 and ST depression in the anterior leads due to proximal occlusion of the first obtuse marginal. Primary coronary angioplasty and stenting of this artery were performed. Twelve hours later the patient became hemodynamically unstable and severe mitral regurgitation due to rupture of one of the heads of the anterolateral papillary muscle was diagnosed. Emergency surgery was performed (papillary muscle head reimplantation, mitral annuloplasty with a rigid ring, tricuspid annuloplasty and coronary artery bypass grafting). On surgical inspection, it was observed that the detached muscle head had become trapped in the left ventricle by a secondary cord attached to the other head. This case is unusual in presenting two uncommon features of ischemic papillary muscle: rupture of the anterolateral muscle in myocardial infarction involving the inferoposterior walls, and the fact that the ruptured muscle head did not prolapse because it had become trapped in the left ventricle by secondary cord attachment.
- Treatment patterns and preferences of people living with HIV starting or switching antiretroviral therapy: Real-world evidence from PortugalPublication . Pedro, L; Zagalo, A; Tavares, R; Pacheco, P; Oliveira, J; Vaz Pinto, I; Serrão, R; Tavares, S; Brito, P; Maltez, F; Neves, I; Carvalho, A; Teófilo, E; Almeida, J; Lains, IBackground: There is a lack of up-to-date real-life evidence on antiretroviral therapy (ART) strategies among people living with HIV (PLWH) in Portugal. This study aimed to describe the treatment strategy used in PLWH either initiating or switching ART. Methods: Non-interventional, cross-sectional, multicenter study carried out between December 2019 and October 2021 in Portugal. Results: A total of 237 PLWH were included in this study, 171 of whom were ART-experienced and 66 were ART-naïve. The study showed that triple regimens were the most common ART strategy and integrase strand transfer inhibitors-based therapy was the most frequently used therapeutic class in both ART-naïve and ART-experienced PLWH. Nevertheless, about a third of PLWH who started a triple regimen transitioned to a dual regimen. Patient-reported outcomes revealed high HIV literacy and similar ART preferences in both groups. Conclusions: This real-world study showed that triple regimens were the most widely used ART strategy, even after the European AIDS Clinical Society guidelines introduced the recommendation of a dual regimen for naïve patients. The cohorts of this study presented a high level of HIV literacy at the time of inclusion. Our findings highlighted that taking pills only once a day is considered a very important feature for most patients.
