Abstract(s)
O presente relatório, apresenta-se no âmbito do Curso de Mestrado em
Enfermagem na Área de Especialização à Pessoa em Situação Crítica (PSC), e vem
desta forma explanar o percurso efetuado nos contextos de estágio quer de Serviço
de urgência, quer de Unidade de Cuidados Intensivos, com foco especial no meu tema
de projeto “Intervenção Especializada de Enfermagem à Pessoa em Situação Crítica
com Hemorragia Digestiva Alta”. Tem assim em vista a minha aquisição e
desenvolvimento de competências na intervenção à PSC e respetiva família, tendo
como cerne as competências inerentes ao grau de mestre (definidas pelos descritores
de Dublin e Decreto de lei n.º65/2018), o documento orientador do Mestrado da PSC
da Escola Superior de Enfermagem de Lisboa (ESEL), e ainda as competências
comuns e específicas do enfermeiro especialista na área de intervenção à PSC.
A Hemorragia Digestiva Alta (HDA) em fase aguda, quer de origem varicosa,
quer de origem não varicosa, ainda se apresenta nos dias de hoje como uma grande
ameaça para os clientes, podendo levar a situações de falência multiorgânica e até
mesmo de morte, caso não haja uma vigilância e intervenção adequadas. Apesar de
todos os avanços na terapêutica tanto sistémica (via endovenosa), como ao nível da
evolução na tecnologia endoscópica, todos estes riscos se mantêm elevados, pelo
que se torna num enorme desafio para todos os profissionais de saúde que destes
clientes cuidam, onde se inclui o enfermeiro como um pilar fundamental dos cuidados,
e em que urge a necessidade de uma intervenção cada vez mais especializada.
Assim, o foco dos cuidados à PSC com HDA, alude à prevenção de desfechos
potencialmente fatais, através de uma vigilância constante dos diversos sinais e
sintomas de hemorragia, antecipando cuidados fundamentais, intervindo
precocemente perante a deteção de sinais de instabilidade, tendo em vista assim o
controlo da hemorragia e todas as lesões secundárias que daí derivam para o cliente.
Todo este percurso de desenvolvimento de competências, veio inevitavelmente
traduzir-se numa evolução no cuidar em enfermagem a este tipo de clientes e não só,
promovendo sem dúvida uma melhoria contínua da qualidade dos cuidados
prestados, baseados em conhecimento científico, que por sua vez se traduzem em
melhores outcomes na saúde e bem-estar finais.
This report is presented within the scope of the Master's Degree in Nursing in the Area of Specialization to The Person in Critical Situation (PSC), and thus explains the course taken in the contexts of internship both emergency service and intensive care unit, with special focus on my project theme "Specialized Nursing Intervention to People in Critical Situation with Upper Gastrointestinal Bleeding". It thus has in mind my acquisition and development of competencies in the intervention of the PSC and its family, having as its core the competencies inherent to the master's degree (defined by the dublin descriptors and decree of law no. 65/2018), the guiding document of the Master's degree of the PSC of the Higher School of Nursing of Lisbon (ESEL), and also the common and specific competencies of the specialist nurse in the area of intervention to the PSC. The acute Upper Gastrointestinal Bleeding (UGIB), whether of variceal or nonvariceal origin, still presents itself nowadays as a major threat to clients, and can lead to situations of multiorganic failure and even death, if there is no adequate surveillance and intervention. Despite all the advances in therapeutics both systemic (intravenously), as well as in the level of evolution in endoscopic technology, all these risks remain high, so it becomes a huge challenge for all health professionals who care for these clients, where nurses are included as a fundamental pillar of care, and in which the need for an increasingly specialized intervention is urgently needed. Thus, the focus of PSC care with UGIB, alludes to the prevention of potentially fatal outcomes, through constant surveillance of the various signs and symptoms of hemorrhage, anticipating fundamental care, intervening early in the presence of signs of instability, thus in view of the hemorrhage control and all secondary lesions that derive to the client. This whole path of skills development inevitably resulted in an evolution in nursing care to this type of clients and not only, undoubtedly promoting a continuous improvement in the quality of care provided, based on scientific knowledge, which in turn translate into better outcomes in final health and well-being
