Abstract(s)
As doenças cardiovasculares, como o enfarte agudo do miocárdio, a insuficiência
cardíaca e as arritmias, conduzem por vezes, a situações de potencial ou total, falência das
funções vitais. Em casos específicos, a utilização adequada de pacing provisório transvenoso
pode salvar vidas. O pacemaker provisório é utilizado na presença de bradiarritmias
intermitentes, ou nas repercussões hemodinâmicas, mas também, nos casos de elevado risco
de assistolia ou bloqueio auriculoventricular. No entanto, está associado a diversas
complicações, podendo comprometer a segurança do doente, pelo que se preconiza a
uniformização dos cuidados de saúde.
No decorrer do Estágio Final, foi desenvolvida uma Intervenção Profissional Major com o
objetivo de atender à qualidade e segurança nos cuidados de enfermagem ao doente crítico
com pacemaker provisório, através da implementação de um procedimento com as
intervenções de enfermagem delineadas à uniformização dos cuidados, e que se apresenta
de forma fundamentada neste relatório.
Neste documento, procede-se também, à análise e reflexão fundamentada do processo
de aquisição e desenvolvimento das competências comuns e específicas do Enfermeiro
Especialista em Enfermagem Médico-Cirúrgica – A Pessoa em Situação Crítica e aquisição
de competências de Mestre em Enfermagem.
Cardiovascular diseases such as acute myocardial infarction, heart failure, and arrhythmias sometimes lead to situations of potential or total failure of vital functions. In specific cases, the appropriate use of transvenous pacing can save lives. The temporary pacemaker is used in the presence of intermittent bradyarrhythmias, or in hemodynamic repercussions, but also in cases of high risk of asystole or auriculoventricular block. However, it is associated with several complications, which can compromise patient safety. During the Final Traineeship, a Major Professional Intervention was developed with the purpose of addressing the quality and safety in the nursing care provided to critically ill patients with temporary pacemakers, through the implementation of a procedure with the nursing interventions designed to standardize care, which is presented in this report. This document also provides an analysis and reasoned reflection on the process of acquisition and development of the common and specific competencies of the Specialist Nurse in Medical-Surgical Nursing - The Person in Critical Condition and the acquisition of competencies of the Master in Nursing.
