Contexto: A extração de terceiros molares está fortemente associada a sequelas pós-operatórias que, para além de dor, podem incluir uma exacerbada resposta inflamatória que acarreta consigo consequências sistémicas. Os concentrados plaquetários possuem várias propriedades anti-inflamatórias e imunológicas que visam auxiliar o processo de cicatrização.
Objetivo: Avaliar se a utilização da fibrina rica em leucócitos e plaquetas reduz a resposta inflamatória aguda após a remoção de terceiros molares, através da colocação de membranas de PRF no interior do alvéolo dentário.
Materiais e Métodos: Participantes a requererem a remoção unilateral de terceiros molares inclusos foram incluídos num ensaio clínico randomizado. Os participantes foram alocados em dois grupos: controlo (sem colocação de membranas) e experimental (com a colocação de membranas). A resposta inflamatória foi analisada através de análise sanguínea para a determinação da contagem de glóbulos brancos (WBC) e velocidade de sedimentação eritrocitária (VSE). Foi também avaliada a dor, edema, trismus e cicatrização dos tecidos moles.
Resultados: Não se verificaram diferenças estatisticamente significativas entre os grupos, para a contagem da WBC, VSE e para os parâmetros da dor, edema, trismus e cicatrização dos tecidos moles.
Conclusões: O PRF parece não ter efeito na diminuição da resposta inflamatória aguda, após a extração superior e inferior unilateral de terceiros molares inclusos.
Background: Third molar extraction is strongly associated with postoperative sequelae that, besides pain, may include an exacerbated inflammatory response that brings with it systemic consequences. Platelet concentrates have several anti-inflammatory and immunological properties that aim to support the healing process. Aim: To assess whether the use of leukocyte- and platelet-rich fibrin reduces the acute inflammatory response seen after third molar removal by placing PRF membranes inside the extraction socket. Material and Methods: Participants requiring unilateral removal of impacted third molars were included in a randomized clinical trial. The participants were allocated into two groups: control (no membranes placement) and experimental (with membranes placement). The inflammatory response was assessed by blood analysis for white blood cell count (WBC) and erythrocyte sedimentation rate (VSE). Pain, edema, trismus and soft tissue healing were also evaluated. Results: There were no statistically significant differences between the groups for WBC, VSE, and for the parameters of pain, oedema, trismus and soft tissue healing. Conclusions: PRF seems to have no effect on decreasing the acute inflammatory response seen after unilateral maxillary and mandibular extraction of impacted wisdom teeth.
