A pessoa é um ser social com direito a autodeterminar-se e a ter uma posição ativa na procura de melhores níveis de saúde. O enfermeiro atua na relação terapêutica com a pessoa e família, desenvolvendo os seus conhecimentos e as suas intervenções com base no projeto de saúde da pessoa.
A Insuficiência Cardíaca é a primeira causa de hospitalização em pessoas com mais de 65 anos, nos países desenvolvidos, associando-se um aumento do número de portadores desta patologia ao envelhecimento da população. Apesar dos avanços científicos ao longo das últimas três décadas, a Insuficiência Cardíaca representa elevadas taxas de mortalidade e de internamentos hospitalares, constituindo um grande impacto no sistema de saúde nacional.
O enfermeiro especialista tem uma ação fundamental na prevenção de complicações e na implementação de uma intervenção eficiente a um problema identificado. A resposta adequada de acordo com o problema reconhecido deve ser a área de atuação do enfermeiro especialista promovendo cuidados de saúde com vista à recuperação da pessoa. Desta forma, foi realizada uma pesquisa com base na evidência científica aliada à teoria das transições de Afaf Meleis.
A intervenção de enfermagem deve ser estabelecida com base na pessoa alvo dos cuidados e, por isto
o enfermeiro especialista deve ter a capacidade de adequar e direcionar a sua ação numa perspetiva centrada na pessoa e na sua família.
Associado ao estudo da problemática, o presente relatório demonstra os objetivos delineados para o estágio e o percurso de desenvolvimento de competências especializadas em enfermagem, assim como a descrição e análise das atividades, e como foram fundamentais para o desenvolvimento de competências.
The person is a social being with the right to self-determination and an active role in the search for better levels of health. The nurse acts in a therapeutic relationship with the person and their family, developing their knowledge and interventions based on the person's health project. Heart failure is the leading cause of hospitalization in people over the age of 65 in developed countries, and an increase in the number of people with this condition is associated with an ageing population. Despite scientific advances over the last three decades, heart failure represents high rates of mortality and hospital admissions, and has a major impact on the national health system. The specialised nurse has a fundamental role to play in preventing complications and implementing an efficient intervention for an identified problem. The adequacy of the response according to the recognised problem should be the specialised nurse's area of action, promoting health care with a view to the person's recovery. In this way, I carried out research based on scientific evidence combined with Afaf Meleis' theory of transitions. Nursing intervention must be based on the person being cared for and, for this reason, the specialist nurse must have the ability to adapt and direct their action from a perspective centred on the person and their family. Linked to the study of the problem, this report demonstrates the development of specialised nursing skills, by describing and analysing the activities carried out during the internship, as well as the objectives outlined and how they were fundamental to the development of skills.
