Abstract(s)
As mudanças verificadas nas organizações de saúde são uma constante, derivadas de fatores de variada ordem. Estas mudanças têm impacte no ambiente de trabalho dos profissionais que, por sua vez, influenciam a satisfação profissional, a forma como se prestam os cuidados de saúde, a satisfação dos clientes e a qualidade dos resultados obtidos com os cuidados prestados pela organização. A mudança organizacional é um conceito vasto, usados em várias áreas das ciências humanas, que envolve questões como a adaptação, renovação e revitalização. Já o ambiente de trabalho pode considerar-se o equilíbrio da relação entre o ser humano e o ambiente no contexto de trabalho.
Com base nestes pressupostos e integrado no projeto de investigação “Qualidade e Segurança nos Cuidados de Saúde (QSCare)”, ESEP/CINTESIS@RISE, avançou-se para um estudo qualitativo, descritivo e transversal, que teve como finalidade contribuir para a melhoria da gestão em enfermagem, a partir do conhecimento obtido sobre a perceção dos enfermeiros sobre o impacte de uma mudança organizacional, ocorrida numa instituição hospitalar, no ambiente de trabalho.
Os objetivos que orientaram a investigação foram: Descrever a perceção dos enfermeiros sobre o conceito de mudança organizacional; Descrever a mudança organizacional ocorrida, numa instituição hospitalar da região Norte de Portugal; Descrever a forma como a mudança organizacional ocorrida foi gerida; Descrever a perceção dos enfermeiros sobre o conceito de ambiente de trabalho; Caracterizar o ambiente de trabalho prévio e após a mudança organizacional; Explorar o impacte da mudança organizacional ocorrida no ambiente de trabalho dos enfermeiros.
Participaram no estudo 11 enfermeiros, selecionados através de uma amostra intencional não probabilística, que vivenciaram a mudança organizacional ocorrida e que aceitaram participar no estudo. Para a recolha da informação optou-se pela entrevista semiestruturada. A análise dos dados foi realizada através da técnica de análise de conteúdo de Bardin (2020).
A partir dos discursos dos participantes emergiram 19 subcategorias e 7 categorias. As categorias emergentes são: Conceito de mudança organizacional; Perceção dos enfermeiros sobre a mudança organizacional ocorrida; Perspetivas dos enfermeiros sobre a gestão da mudança organizacional; Conceito de ambiente de trabalho de enfermagem; Caracterização do ambiente de trabalho prévio à mudança organizacional; Caracterização do ambiente de trabalho após a mudança organizacional; Perceção dos enfermeiros sobre o impacte da mudança organizacional vivenciada no ambiente de trabalho atual.
A informação gerada com este estudo pode constituir um ponto de reflexão em torno da gestão dos processos de mudança, particularmente centrado nas estratégias de manutenção e promoção de ambientes de trabalho saudáveis. Assim, revela-se importante desenvolver dinâmicas de formação dos enfermeiros em torno dos conceitos de mudança organizacional e ambiente de trabalho, inclusivamente no que concerne ao papel determinante do enfermeiro gestor nestes processos.
The changes that occur in healthcare organizations are constant and are derived from various factors. These changes have an impact on the work environment of professionals, which in turn influences job satisfaction, the way healthcare is provided, customer satisfaction, and the quality of results obtained from the care provided by the organization. Organizational change is a broad concept used in various areas of human sciences, involving issues such as adaptation, renewal, and revitalization. The work environment can be considered the balance of the relationship between the human being and the environment in the context of work. Based on these assumptions and integrated into the research project "Quality and Safety in Healthcare (QSCare)", ESEP/CINTESIS@RISE, a qualitative, descriptive, and cross-sectional study was carried out with the aim of contributing to the improvement of nursing management through knowledge obtained about nurses' perception of the impact of an organizational change that occurred in a hospital institution on the work environment. The objectives that guided the research were: to describe nurses' perception of the concept of organizational change; to describe the organizational change that occurred in a hospital institution in the Northern region of Portugal; to describe how the organizational change was managed; to describe nurses' perception of the concept of work environment; to characterize the pre-existing work environment and after the organizational change; to explore the impact of the organizational change on nurses' work environment. Eleven nurses participated in the study, selected through a non-probabilistic intentional sample, who experienced the organizational change and agreed to participate in the study. The semi-structured interview was chosen for data collection. The data analysis was performed using the content analysis technique of Bardin (2020). From the participants speeches, 19 subcategories and 7 categories emerged. The emerging categories are: Concept of Organizational Change; Nurses' Perception of Organizational Change that Occurred; Nurses' Perspectives on Managing Organizational Change; Concept of Nursing Work Environment; Characterization of Pre-Organizational Change Work Environment; Characterization of Work Environment After Organizational Change; Nurses' Perception of the Impact of Organizational Change Experienced in the Current Work Environment. The information generated by this study can be a point of reflection around managing change processes, particularly focused on strategies for maintaining and promoting healthy work environments. Therefore, it is important to develop training dynamics for nurses around the concepts of organizational change and work environment, including the determining role of nurse managers in these processes.
