Estratégias do enfermeiro gestor para a segurança do doente no bloco operatório: revisão scoping

Introdução: O bloco operatório constitui-se como um dos locais mais complexos de prestação de cuidados e onde ocorre o maior número de eventos adversos. Desta forma, é imprescindível posicionar o enfermeiro gestor como um profissional de destaque na promoção da segurança do doente, que tem sido também um foco de atenção da Organização Mundial de Saúde. Objetivo: mapear a evidência sobre as estratégias do enfermeiro gestor para a segurança do doente no bloco operatório. Métodos: realizada revisão scoping (ScR), segundo o método do Joanna Briggs Institute (JBI), onde foram considerados estudos quantitativos, qualitativos, mistos, nos idiomas português, inglês e espanhol, sem limite temporal. A pesquisa foi efetuada nas bases de dados Medline®, CINAHL®, MedicLatina®, Academic Search Complete® via EBSCO, SCOPUS® e Web of Science, Portal da Biblioteca Virtual em Saúde, OpenAir e RCAAP (Repositório Científico de Acesso Aberto de Portugal). A seleção da fonte, triagem por título, resumo e texto completo foi conduzida por dois revisores independentes. As divergências foram resolvidas por consenso ou por decisão do terceiro revisor. Os resultados foram organizados seguindo as orientações do Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Foram incluídos estudos sobre estratégias utilizadas pelo enfermeiro gestor que promovam a segurança do doente no bloco operatório. Na mnemónica PCC, foram considerados como participantes os enfermeiros gestores, o conceito enquadrou as estratégias para a segurança do doente e o contexto foi o de bloco operatório. Foram excluídos todos os artigos que não cumpriram os critérios referidos. Resultados: A revisão efetuada permitiu identificar algumas estratégias do enfermeiro gestor para a segurança do doente no bloco operatório, entre as quais se destacam: o apoio do enfermeiro gestor e da gestão de topo, a garantia de equipas adequadas e ambientes de trabalho saudáveis, uma cultura que não penaliza erros, uma comunicação clara, programas de formação, a liderança do enfermeiro gestor e uma abordagem centrada no doente. Conclusão: os resultados encontrados pretendem ser um contributo para melhorar a segurança do doente no bloco operatório. É essencial a prevenção de eventos adversos e promoção de um ambiente de trabalho eficiente e satisfatório para os profissionais de saúde A intervenção do enfermeiro gestor é determinante na implementação de estratégias.
Introduction: The operating theatre is one of the most complex places in which care is provided and where the greatest number of adverse events occur. It is therefore essential to position the nurse manager as a leading professional in promoting patient safety, which has also been a focus of attention for the World Health Organisation. Objective: To map the evidence on nurse managers' strategies for patient safety in the operating theatre. Methods: A scoping review (ScR) was carried out using the Joanna Briggs Institute (JBI) method, which considered quantitative, qualitative and mixed studies in Portuguese, English and Spanish, with no time limit. The search was carried out in the Medline®, CINAHL®, MedicLatina®, Academic Search Complete® via EBSCO, SCOPUS® and Web of Science databases, the Virtual Health Library Portal, OpenAir and RCAAP (Portuguese Open Access Scientific Repository). Source selection, screening by title, abstract and full text was conducted by two independent reviewers. Disagreements were resolved by consensus or by decision of the third reviewer. The results were organised following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Studies on strategies used by nurse managers to promote patient safety in the operating theatre were included. In the PCC mnemonic, nurse managers were considered as participants, the concept included strategies for patient safety and the context was the operating theatre. All articles that did not fulfil the above criteria were excluded. Results: The review made it possible to identify some of the nurse manager's strategies for patient safety in the operating theatre, among which the following stand out: support from the nurse manager and top management, ensuring adequate teams and healthy working environments, a culture that does not penalise errors, clear communication, training programmes, the nurse manager's leadership and a patient-centred approach. Conclusion: The results found are intended as a contribution to improving patient safety in the operating theatre. It is essential to prevent adverse events and promote an efficient and satisfactory working environment for healthcare professionals.
