A dor crónica crânio-cérvico-facial é um problema de saúde pública no que tange à incapacidade laboral, redução de produtividade, assim como causa para o excesso no consumo de medicamentos, adoecimento, e gasto de demasiado recurso financeiro. É um contribuinte significativo para a morbilidade, incapacidade e perda financeira em todo o mundo.
Diferente da dor aguda, a dor crónica normalmente não tem solução pelo próprio organismo, e ao encará-la como uma doença, a medicina tem a tendência em focar as suas expectativas no controlo dos sintomas, não alcançando por vezes, a erradicação da causa. E assim o doente acaba entrando num mundo sem volta da doença crónica. O futuro é de limitações de vida em todos os sentidos.
A terapia neural é uma abordagem de tratamento que visa regular o sistema nervoso autónomo alterando o potencial de repouso das células nervosas através da injeção de um anestésico local específico e em baixa concentração. Possui efeito anti-inflamatório que modula sistematicamente os processos neuroplásticos de sensibilização periférica, potencialização pós-sináptica de longo prazo e disfunções patológicas do sistema nervoso central e sistema nervoso autónomo, encerrando o ciclo da dor, encerrando a libertação de citocinas e reestabelecendo a homeostase.
O Objetivo desta revisão narrativa é analisar e descrever o panorama das dores crónicas da cabeça e do pescoço, que podem ser detalhadas regionalmente em crânio-cérvico-facial, e a sugestão da terapia neural, avaliando o seu efeito a curto e longo prazo, utilizando injeções de procaína a baixas concentrações, de 0,5% a 1%, para o tratamento e erradicação da causa quando possível.
Este trabalho visa demonstrar e sugerir a terapia neural como uma grande possibilidade aplicada no âmbito da medicina dentária, clarificando o entendimento e o tratamento destes sintomas, principalmente da causa neurológica da dor.
Chronic cranio-cervical-facial pain is a public health problem in terms of work incapacity, reduced productivity, as well as a cause for over-consumption of medication, illness, and expenditure of excessive financial resources. It is a significant contributor to morbidity, disability, and financial loss worldwide. In contrast to acute pain, chronic pain doesn´t find a solution by the organism, and when facing it as a disease, medicine tends to focus its expectations on controlling the symptoms, sometimes not reaching the eradication of the cause. And so the patient ends up entering a world with no return of chronic illness. The future is one of life limitations in every way. Neural therapy is a treatment approach that aims to regulate the autonomic nervous system by altering the resting potential of nerve cells through the injection of a specific, low-concentration local anesthetic. It has an anti-inflammatory effect that systematically modulates neuroplastic processes of peripheral sensitization, long-term postsynaptic potentiation and pathological dysfunctions of the central nervous system and autonomic nervous system, ending the pain cycle, ending the release of cytokines and reestablishing homeostasis. The objective of this narrative review is to analyze and describe the panorama of chronic head and neck pain, which can be regionally detailed in cranio-cervico-facial, and the suggestion of neural therapy, evaluating its short and long-term effect, using procaine injections at low concentrations, from 0.5% to 1%, for treatment and eradication of the cause when possible. This work aims to demonstrate and suggest neural therapy as a great possibility applied in the field of dentistry, clarifying the understanding and treatment of these symptoms, especially the neurological cause of pain.
