O presente documento formaliza o percurso académico final, que aqui se descreve a fim de
demonstrar o desenvolvimento de competências para obtenção do grau de mestre em
Enfermagem e do título profissional de enfermeiro especialista em Enfermagem Médico-
Cirúrgica na área de Enfermagem à Pessoa em Situação Crítica.
Ao longo do documento serão apresentadas as diferentes atividades que me capacitam para,
enquanto enfermeiro especialista nesta área de Enfermagem, prestar cuidados diferenciados,
assumir responsabilidades na gestão dos cuidados de enfermagem, supervisão e formação de
outros enfermeiros e profissionais de saúde.
O enfoque na pessoa em situação crítica portadora de cateter venoso central foi o mote para o
desenvolvimento de competências clínicas e onde se incluem aquelas que se relacionam com a
prevenção e controlo da infeção.
A segurança do doente depende da qualidade dos serviços de saúde. As infeções associadas aos
cuidados de saúde comprometem esta segurança, aumentam os custos com a saúde e
contribuem para a resistência antimicrobiana. O Plano Nacional para a Segurança dos Doentes
2021–2026, destaca as atividades fundamentais do Programa de Prevenção e Controlo de
Infeções e de Resistência aos Antimicrobianos, nomeadamente na promoção e implementação
de feixes de intervenções, um dos quais tem como objetivo reduzir a infeção da corrente
sanguínea relacionada com o cateter venoso central. Um enfermeiro especialista com domínio
da melhor evidência disponível acerca do assunto pode fazer toda a diferença no contexto
clínico. Isto porque, são os enfermeiros, os profissionais responsáveis pela manutenção e
otimização do cateter venoso central.
A narrativa do percurso desenvolvido inclui a caraterização dos contextos da prática clínica; a
apresentação de dois casos clínicos, procurando fundamentar o processo de tomada de decisão
clínica e sistematizar os cuidados de enfermagem com base na plataforma E4Nursing, que
deriva da ontologia NursingOntos; e a explanação do contributo das diferentes atividades para o
desenvolvimento das competências comuns do Enfermeiro Especialista e das competências
específicas em Enfermagem Médico-Cirúrgica na área de Enfermagem à Pessoa em Situação
Crítica.
This document formalises the final academic pathway, which is described here in order to demonstrate the development of competences to obtain the Master's degree in Nursing and the professional title of specialist nurse in Medical-Surgical Nursing in the area of Critical Care Nursing. Throughout the document, I will present the different activities that enable me, as a nurse specialising in this area of nursing, to provide differentiated care, take responsibility for the management of nursing care, supervision and training of other nurses and health professionals. The focus on the critically ill person with a central venous catheter was the motto for developing clinical skills, including those related to infection prevention and control. Patient safety depends on the quality of healthcare services. Healthcare-associated infections jeopardise this safety, increase healthcare costs and contribute to antimicrobial resistance. The National Patient Safety Plan 2021-2026 highlights the fundamental activities of the Programme for the Prevention and Control of Infections and Antimicrobial Resistance, namely the promotion and implementation of bundles of interventions, one of which aims to reduce central venous catheter-related bloodstream infections. A specialised nurse with a command of the best available evidence on the subject can make all the difference in the clinical context. This is because nurses are the professionals responsible for maintaining and optimising the central venous cateter. The narrative of the path developed includes the characterisation of the clinical practice contexts; the presentation of two clinical cases, seeking to substantiate the clinical decisionmaking process and systematise nursing care based on the E4Nursing platform, which derives from the NursingOntos ontology; and the explanation of the contribution of the different activities to the development of the common competences of the Specialist Nurse and the specific competences in Medical-Surgical Nursing in the area of Nursing the Critically Ill Person
