Abstract(s)
As infeções associadas aos cuidados de saúde comprometem a segurança e a qualidade dos
serviços de saúde. Globalmente, o impacto das infeções associadas aos cuidados de saúde está
a ser priorizado nas decisões políticas pois contribuem para a ameaça global da resistência
antimicrobiana.
O cateter venoso central é um dispositivo muito utilizado na pessoa em situação crítica e apesar
de apresentar imensas vantagens, a sua manutenção associa-se a infeções locais ou sistémicas,
aumentando a probabilidade de infeção, principalmente por agentes multirresistentes. Na
prática clínica, a manutenção do cateter venoso central é uma das intervenções
frequentemente realizadas pelos enfermeiros, assim, estes profissionais podem contribuir para
minimizar as infeções associadas aos cuidados de saúde através do planeamento e prestação
de cuidados baseados num “Feixe de Intervenções” de Prevenção de Infeção Relacionada com
Cateter Venoso Central.
Esta apreensão com a infeção é manifestada no Plano Nacional para a Segurança do Doente
2021-2026, que tem como objetivo reduzir em 30% a incidência de infeções da corrente
sanguínea associadas ao cateter venoso central. Assim, a Direção-Geral da Saúde e o Ministério
da Saúde consideram a participação da instituição no "STOP-Infeção Hospitalar 2.0" um
indicador do Índice de Qualidade do Programa de Prevenção e Controlo de Infeção e de
Resistências aos Antimicrobianos, e parte integrante do processo de contratualização das
instituições prestadoras de saúde do Sistema Nacional de Saúde.
Nesta lógica, existe cada vez mais responsabilidade a nível das organizações de saúde na qual
as Unidades Locais do Programa de Prevenção e Controlo de Infeções e de Resistência aos
Antimicrobianos estão inseridas, sendo estas unidades responsáveis pela divulgação e formação
de toda a equipa multidisciplinar na adoção de práticas seguras. Sendo também um dever ético
deontológico, a par da consciencialização da responsabilidade de cada profissional de saúde.
O presente relatório é o resultado formal da conclusão do Mestrado em Enfermagem Médico-
Cirúrgica na área de Enfermagem à Pessoa em Situação Crítica. Através deste relatório
pretende-se demonstrar o percurso formativo de construção em diferentes serviços de
prestação de cuidados de saúde de um Enfermeiro Especialista, e sintetizar o conhecimento que
este possui apresentando a reflexão contínua como um processo dinâmico e imprescindível à
tomada de decisão.
Healthcare-associated infections jeopardise the safety and quality of healthcare services. Worldwide, the impact of healthcare-associated infections is being prioritised in policy-making, as they contribute to the global threat of antimicrobial resistance. The central venous catheter is a device widely used by critically ill patients and, despite having numerous advantages, its maintenance is associated with infections, both local and systemic, increasing the likelihood of infection, especially by multidrug-resistant agents. In clinical practice, the maintenance of the central venous catheter is one of the interventions frequently carried out by nurses, so they can contribute to minimising healthcare-associated infections by planning and providing care based on a "Bundle of Interventions" for the Prevention of Central Venous Catheter-Related Infection. This concern about infection is foreseen in the National Patient Safety Plan 2021-2026, which aims to reduce the occurrence of central venous catheter-related bloodstream infections by 30%. The Directorate-General for Health and the Ministry of Health consider participation in the "STOP-Infection Hospital 2.0" programme as an indicator of the Quality Index of the Infection Prevention and Control and Antimicrobial Resistance Programme and as part of the process of selecting institutions for the National Health System. There is thus a growing responsibility for healthcare organisations that are Local Units of the Infection Prevention and Control and Antimicrobial Resistance Programme, and it is up to these units to disseminate and train all multidisciplinary teams in the adoption of safe practices. This is an ethical and deontological duty, as well as raising awareness of the responsibility of each healthcare professional. This report is the formal conclusion of the Master's Degree in Medical-Surgical Nursing in the area of Critical Care Nursing. This report will demonstrate the formative journey of the different health services provided by a specialist nurse, summarise her knowledge and present continuous reflection as a dynamic and fundamental process for decision-making.
