Sinónimo do culminar de um caminho de aprendizagens emerge o presente Relatório de Estágio, uma reflexão pessoal sobre o percurso formativo concretizado. A Doença Pulmonar Obstrutiva Crónica é, atualmente a terceira causa de morte no mundo, prevendo-se uma crescente prevalência nos próximos anos. Com implicações pulmonares e extrapulmonares, é pautada por exacerbações, eventos agudos de agravamento de sintomas com elevado impacto pessoal, familiar e social, responsável por uma maior procura de cuidados de saúde urgentes, elevadas taxas de internamento e readmissões hospitalares associadas, representando uma importante carga na sociedade e sistemas de saúde.
Capacitar a pessoa com vista à prevenção de exacerbações, dotando-a de competências que facilitem a autogestão da sua condição de saúde a longo prazo constituiu a motivação da temática foco da intervenção de enfermagem de reabilitação e da revisão da literatura realizada, com o intuito de sintetizar a evidência científica disponível através de pesquisa em bases de dados.
O enfermeiro especialista em enfermagem de reabilitação desenvolve uma intervenção individualizada, centrada na pessoa e família com o objetivo de promover readaptação funcional, capacidade de autocuidado e prevenção de complicações, resultante em ganhos em saúde. O Modelo de Adaptação de Roy expressa a capacidade de adaptação constante da pessoa resultante da interação com o ambiente que a rodeia, norteando o ponto de vista do conhecimento de enfermagem essencial para a operacionalização da sua intervenção.
O estágio clínico realizado em dois contextos distintos, Hospital e Cuidados de Saúde Primários, proporcionou múltiplas aprendizagens resultantes da articulação de conhecimentos teóricos com a prática clínica. A metodologia descritiva, reflexiva e crítica utilizada na descrição de um conjunto de atividades implementadas, cruciais ao alcance dos objetivos definidos, ostenta o desenvolvimento de competências comuns e específicas do Enfermeiro Especialista em Enfermagem de Reabilitação.

Chronic Obstructive Pulmonary Disease is currently the third leading cause of death in the world and is expected to increase in prevalence in the coming years. With pulmonary and extrapulmonary implications, it is marked by exacerbations, acute events of worsening of symptoms with high personal, family and social impact, responsible for a greater demand for urgent health care, with high rates of hospitalizations and associated hospital readmissions, representing an important burden on society and health systems. Empowering the person to prevent exacerbations and providing them skills that facilitate the self-management of their health condition in the long term, was the motivation for the focus theme of the rehabilitation nursing intervention and the literature review carried out to synthesize the scientific evidence available through database research. The nurse specialist in rehabilitation nursing develops an individualized intervention, centred on the person and family, intending to promote functional readaptation, self-care capacity and preventing complications, resulting in health gains. Roy's Adaptation Model expresses the person's ability to constantly adapt due to the interaction with the environment that surrounds him/her, guiding the point of view of nursing knowledge that is essential for the operationalization of this intervention. The clinical internship fulfilled in two distinct contexts, Hospital and Primary Health Care, provided multiple learnings resulting from the articulation of theoretical knowledge with clinical practice. The descriptive, reflexive and critical methodology used in describing a set of implemented activities, crucial to the achieving the defined objectives, shows the development of common and specific competencies of the Specialist Nurse in Rehabilitation Nursing.
