O Relatório de Estágio versa sobre o tema, Gestão do Delirium na Pessoa em Situação Paliativa nos Últimos Dias e Horas de Vida e Família em contexto hospitalar, estando inserido na unidade curricular de Estágio com Relatório do 1º curso de Mestrado em Enfermagem Médico-Cirúrgica na área de Enfermagem à Pessoa em Situação Paliativa. O tema deve-se à grande prevalência de utentes com delirium no meu contexto profissional, sendo o desenvolvimento do delirium terminal particularmente prevalente (44% a 88%) e considerado sinal de mau prognóstico. O relatório tem como objetivos demonstrar capacidade de orientar o meu processo de aprendizagem, desenvolvimento profissional e prática clínica, revelando pensamento crítico, reflexivo e raciocínio clínico e relacionando-a com a Teoria do Fim de Vida Tranquilo. Para tal utilizarei a Metodologia de Trabalho por Projeto e aprendizagem experiencial. O percurso de formação levou-me a crescer enquanto profissional, dando primazia aos desejos e vontades do utente e família/cuidadores. Modifiquei a minha comunicação, demonstrando maior compaixão e disponibilidade, conseguindo construir a minha autonomia e o trabalho em equipa. Consegui identificar as necessidades e sintomas do utente e família/cuidadores, intervindo para identificar e antecipar situações de agudização. Desenvolvi competências relativas à participação no processo de tomada de decisão garantindo desejos e preferências, incentivando a participação dos utentes e família/cuidadores, promovendo a sua privacidade e dignidade. Desenvolvi o meu autoconhecimento, permitindo-me prestar cuidados centrados na pessoa. Promovi a divulgação da evidência científica e prática baseada na evidência. No que concerne a Teoria do Fim de Vida Tranquilo, a sua mobilização para a conceção de cuidados à pessoa em situação paliativa e família, permitiu-me promover junto dos utentes e família/cuidadores, o conforto, dignidade, respeito e paz e a proximidade com pessoas significativas ou cuidadoras. Reconheço que o percurso foi fundamental para a aquisição de competências na identificação do delirium, sua gestão e apoio aos utentes e família/cuidadores.
The Internship Report is about the subject of Managing Delirium in Palliative Care in the Last Days and Hours of Life and the Family in a hospital setting. It’s part of the Internship with Report course of the 1st Master's Degree in Medical-Surgical Nursing in the area of Nursing for Palliative Care. The topic arose from the high prevalence of patients with delirium in my professional context, with the development of terminal delirium being particularly prevalent (44% to 88%) and considered a sign of poor prognosis. The report aims to demonstrate my ability to guide my learning process, professional development and clinical practice, revealing critical, reflective thinking and clinical reasoning and relating it to the Peaceful End of Life Theory. To this end, I will use the Project Work Methodology and experiential learning. The training course led me to grow as a professional, giving priority to the wishes and desires of the patient and family/caregivers. I changed my communication, showing greater compassion and availability, managing to build my autonomy and teamwork. I was able to identify the needs and symptoms of the patient and family/caregivers, intervening to identify and anticipate worsening situations. I developed skills relating to participation in the decision-making process, guaranteeing wishes and preferences, encouraging the participation of patients and their families/caregivers, promoting their privacy and dignity. I developed my self-knowledge, enabling me to provide person-centered care. I promoted the dissemination of scientific evidence and evidence-based practice. Regarding the Peaceful End of Life Theory, its mobilization for the conception of care for people in a palliative situation and their families, allowed me to promote with patients and their families/caregivers, comfort, dignity, respect and peace, and proximity to significant people or caregivers. I recognize that the course was fundamental for acquiring skills in the identification of delirium, its management and support for patients and their families/caregivers.
