A supervisão clínica alicerça a gestão da Dor no doente crítico, através da capacitação e empoderamento dos enfermeiros na tomada de decisão, permitindo assegurar um padrão de qualidade de cuidados prestados ao doente crítico. O presente estudo refere o diagnóstico de situação efetuado ao Indicador Sensível à Supervisão Clínica em Enfermagem Dor no contexto de uma Unidade de Cuidados Intensivos, baseando-se no modelo SafeCare.
O estudo tem como objetivos: identificar como é feita a avaliação da Dor no doente crítico numa Unidade de Cuidados Intensivos de um hospital do distrito do Porto; identificar as necessidades formativas a integrar nas sessões de supervisão clínica dos enfermeiros relativamente ao Indicador Sensível à Supervisão Clínica em Enfermagem Dor; e comparar a avaliação da Dor no doente crítico numa Unidade de Cuidados Intensivos num hospital do distrito do Porto antes e após as sessões de supervisão clínica.
É um estudo de investigação quantitativa, do tipo quase-experimental (pré e pós-intervenção) e transversal. A população-alvo é constituída por 31 enfermeiros de uma Unidade de Cuidados Intensivos de um hospital do distrito do Porto. A recolha de dados consistiu em auditorias ao Indicador Sensível à Supervisão Clínica em Enfermagem Dor, efetuadas durante o mês de novembro de 2022 e o mês de maio de 2023, em 92 processos clínicos de doentes críticos numa Unidade de Cuidados Intensivos, através do programa de registos clínicos B-Simple (B-ICU).
As áreas identificadas com necessidade de intervenção supervisiva, foram a “Avaliação da Dor” e a “Gestão da Dor”, sendo à posteriori incluídas nas sessões supervisivas, sob a forma de formação e acompanhamento. Foi salientada a importância da avaliação da Dor, a importância do tratamento não farmacológico, e o conhecimento sobre as complicações da ineficaz gestão da Dor.
Da análise dos resultados verificou-se uma melhoria no Indicador Sensível à Supervisão Clínica em Enfermagem Dor na pós-intervenção, demonstrando melhoria da segurança e qualidade de cuidados prestados ao doente crítico. Este aspeto veio demonstrar que a adoção de um processo de Supervisão Clínica em Enfermagem estruturado contribui para a melhoria da gestão da Dor numa Unidade de Cuidados Intensivos.
Clinical supervision underpins the pain management in the critically ill patient by enabling and empowering nurses to make decisions and ensuring a standard of quality care for the critically ill patient. This study refers to the situation diagnosis regarding the Sensitive Indicator to Clinical Supervision in Nursing Pain in the Intensive Care Unit context, based on the SafeCare model. The study aims to: identify how pain is assessed in critically ill patients in an Intensive Care Unit in a hospital in Porto district; identify the training needs to be included in nurses' clinical supervision sessions in relation to the Sensitive Indicator to Clinical Supervision in Nursing Pain; and compare the assessment of pain in critically ill patients in an Intensive Care Unit in a hospital in Porto district before and after clinical supervision sessions. This is a quantitative, quasi-experimental (pre- and post-intervention), cross-sectional research study.
