cuidador informal.
Context: Informal caregivers play a significant role, but they face numerous challenges and needs resulting from the complexity of caring for a dependent person. This responsibility involves performing multiple tasks and providing specialized care in different areas. When the available support is insufficient and the needs of informal caregivers are not met, they end up experiencing an overload that makes them individuals in need of health care. Objective: To promote the empowerment of informal caregivers of patients admitted to the Integrated Continuous Care Team of a Community Care Unit in the Alentejo Region. Methodology: Anchored in the health planning methodology and with the theoretical framework of Orem's Theory of Self-Care Deficit Nursing theory. Results: After the situation diagnosis, it was found that the main needs/difficulties of informal caregivers are the risk of compromised capacity to care, in terms of physical demands, risk of social isolation, risk of compromised coping in caregiving, lack of knowledge/lack of social support, relational problems, financial problems, and lack of family support. Conclusion: The community and public health specialist nurse plays an essential role in empowering groups and communities, including informal caregivers. Based on the Health Planning methodology, the project "Empowerment of Informal Caregivers - Learn to Take Care of Yourself while Learning to Take Care of Others" was developed. Through the situation diagnosis, it was possible to identify the needs/difficulties experienced by informal caregivers in the act of caregiving. Thus, various empowerment tools for informal caregivers emerged.
