Abstract(s)
do processo saúde-doença.
Background: Population aging is a globally observed phenomenon that represents a major challenge for the healthcare sector, associated with chronic diseases, decreased functionality, and quality of life. Through the empowerment and training of the individual and family, rehabilitation nursing presents itself as an area of practice to promote health and quality of life during the transition process of the health-disease continuum. Objective: To demonstrate the development of rehabilitation nursing skills. To present results consequent to the implementation of the intervention project, evidencing the impact through the health gains achieved. Methodology: A methodology of multiple case studies was used for reflection and analysis of data obtained from the implementation of the intervention project. The sample was conveniently selected following previously established criteria. The care was contextualized by the theoretical framework of Afaf Meleis, following the structure of the nursing process. Results: A rehabilitation program organized and directed to participants resulted in a decrease in functional dependence, allowing greater autonomy and independence of the participants, health gains, indicating that the intervention and empowerment were effective. Conclusion: It was proven the importance of rehabilitation interventions in the functional improvement of the individual, at the same time as empowering the patient and their family during the transition process, making it safer.
