Introdução: São competências do Enfermeiro Gestor a gestão da mudança, da qualidade
e segurança e do desenvolvimento profissional e organizacional. Em 2010, a DGS
estabeleceu a criação e implementação das Equipas de Emergência Médica Intra-
Hospitalares, por meio da Circular Normativa n.º 15/DQS/DQCO, com o objetivo de
proporcionar respostas rápidas em casos de PCR e descompensação fisiológica aguda. A
implementação das Equipas de Resposta Rápida tem impacto significativo na gestão de
doentes críticos, no entanto, a ativação das mesmas é influenciada por diversos fatores.
O questionário CARED é um instrumento com 22 itens, desenvolvido em inglês, que
auxilia os líderes na identificação de possíveis fatores que possam afetar a utilização dos
Sistemas de Resposta Rápida, com o objetivo de otimizar o SRR de cada hospital. Em
Portugal, não existe nenhum instrumento validado que ajude os gestores a reconhecer
os fatores que afetam a utilização dos SRR.
Objetivos: Adaptar e validar transculturalmente o questionário para a população
portuguesa, e determinar as propriedades psicométricas do instrumento obtido.
Metodologia: Foi realizado um estudo do tipo metodológico, composto por três etapas
distintas: 1) tradução e adaptação transcultural, 2) Teste de campo e 3) Validação
psicométrica. Uma amostra de 233 enfermeiros de um hospital público português,
participaram no teste de campo.
Resultados: Obteve-se um instrumento com 19 itens dividido em 3 dimensões: “Crenças
sobre o Sistema de Resposta Rápida”, “Receios em relação à resposta e encaminhamento
de cuidados” e “Perceção da confiança na resposta a doente em deterioração”, que explica
41,29% da variância total. O alfa de Cronbach demonstrou valores aceitáveis para o
questionário total (α=0,74). A análise fatorial confirmatória sustentou a estrutura trifatorial
da escala (X2 /df = 1,433; CFI = 0,951; GFI = 0,922; RMSEA = 0,043; P[rmsea] < 0,001;
MECVI = 1,350).
Conclusão: A escala CARED-PT é válido e fiável, pelo que é adequado para ser utilizado
em contexto cultural português.

In 2010, the DGS established the creation and implementation of Intra-Hospital Medical Emergency Teams, through Normative Circular No. 15/DQS/DQCO, with the aim of providing rapid responses in cases of CPR and acute physiological decompensation. The implementation of Rapid Response Teams has a significant impact on the management of critically ill patients, but their activation is influenced by several factors. The CARED questionnaire is a 22-item instrument, developed in English, which helps leaders identify possible factors that could affect the use of Rapid Response Systems (RRS), with the aim of optimizing each hospital's RRS. In Portugal, there is no validated instrument to help managers recognize the factors that affect the use of RRS. Aims: To adapt and cross-culturally validate the questionnaire for the Portuguese population and determine the psychometric properties of the instrument obtained. Methodology: A methodological study was carried out consisting of three distinct stages: 1) translation and cross-cultural adaptation, 2) field testing and 3) psychometric validation. A sample of 233 nurses from a Portuguese public hospital took part in the field test. Results: An instrument with 19 items was obtained, distributed by 3 dimensions: ‘Beliefs about the Rapid Response System’, ‘Fears about escalating care’ and ‘Perceived confidence in responding to deteriorating patients’, which explains 41.29% of the total variance. Cronbach's alpha showed acceptable values for the total scale (α=0,74). The confirmatory factor analysis supported the three-factor structure of the scale (X2 /df = 1.433; CFI = 0.951; GFI = 0.922; RMSEA = 0.043; P[rmsea] < 0.001; MECVI = 1.350). Conclusion: The CARED-PT scale is valid and reliable, so it is appropriate for use in a Portuguese cultural context.
