Impacto económico da doença oncológica na família - uma revisão sistemática da literatura

Abstract(s)
O aumento da sobrevida dos doentes oncológicos deve-se aos seguintes fatores: às novas técnicas de diagnóstico,
que permitem a identificação da doença em estádios cada vez mais iniciais; à abordagem multiprofissional; ao
aparecimento de novos medicamentos com menos efeitos colaterais e às novas técnicas cirúrgicas. A literatura
revela que todos estes aspetos tornaram a doença oncológica mais onerosa para os sistemas de saúde. Por outro
lado, a conjetura social e económica da atualidade, determina que se emagreçam os custos com a saúde. Assim,
importa conhecer os custos suportados pelas famílias relacionados com a doença oncológica.
Método: Revisão sistemática da literatura, durante o mês de janeiro de 2012, nas seguintes bases de dados
eletrónicas: CINHAL Plus with Full text; MEDLINE with Full Text; MedicLatina; Nursing & Allied Health
Collection; Academic Search Complete; Psychology and behavior. Utilizou-se como descritor ((Cost* AND
Ill*) OR (Cost* AND Disease*) OR (Cost* AND care)) AND famil* AND (oncolog* OR cancer) AND nursing.
Foram incluídos artigos que versassem estudos de investigaçao, em que os participantes fossem doentes oncológicos
submetidos a tratamento curativo ou paliativo, em contexto hospitalar ou domiciliar.
Resultados: Na pesquisa foram encontrados 88 artigos, dos quais 2 se encontravam repetidos; 50 foram rejeitados
pelo título; 31 foram rejeitados pela leitura do resumo e 2 pela leitura do texto integral. Em síntese, foram
incluídos 3 artigos: 2 estudos primários e 1 de revisão sistemática da literatura.
Conclusões: Os custos suportados pelos doentes com cancro e seus familiares relacionam-se com as visitas regulares
às instituições de saúde, com as necessidades decorrentes do percurso terapêutico da doença e com o impacto
desta ao nível laboral. Este tipo de custos infere sobrecarga para as famílias, podendo ter diversas implicações
nas suas vidas, com repercussão na gestão e adesão ao regime terapêutico.
The increase of 5 years in the longevity of cancer patients is due to: new diagnostic techniques which allow identifying the disease in an earlier stage; the multi-professional approach; new medicines with fewer side effects, and new surgical techniques. The literature states that these aspects have made the oncological disease more expensive to healthcare systems. On the other hand, current social and economic context determines cuttings in health costs. Results: Eighty eight articles were found in the search. Two were repeated articles, fifty were rejected because of the title, thirty one were rejected after reading the abstract, and two after reading the full text. In summary, three articles were included: two primary studies and one systematic literature review. Conclusions: Costs supported by cancer patients and their relatives arise from regular visits to healthcare institutions, the therapeutic regimen of the disease and the impact of the disease in labor. These costs result in an overload to families, which in turn has implications in management and adherence of the therapeutic regimen.
