Introdução: A enfermagem especializada desempenha um papel essencial nas equipas
de cuidados paliativos, impulsionada pelo aumento da incidência de doenças incuráveis,
progressivas e limitantes da vida. Essas condições geram sofrimento significativo para
os clientes e suas famílias, afetando aspetos físicos, emocionais, espirituais e sociais.
Este documento aborda os cuidados paliativos e apresenta um Relatório de Estágio de
Natureza Profissional, referente ao Mestrado em Enfermagem Médico-Cirúrgica, na
área de Enfermagem à Pessoa em Situação Paliativa, da Escola Superior de
Enfermagem do Porto. O objetivo principal deste relatório é descrever de forma reflexiva
as atividades realizadas durante o estágio, destacando o impacto no desenvolvimento
das competências comuns e específicas do enfermeiro especialista, em dois dos três
contextos clínicos de cuidados paliativos especializados previstos no plano de estudos
do curso e na regulamentação da Ordem dos Enfermeiros para a atribuição do título de
especialista. Metodologia: Descritiva e reflexiva, fundamentada na evidência científica
disponível, com objetivo de justificar os processos, percursos e intervenções adotadas.
Apresentam-se dois estudos de caso com base na Nursing Ontos e na estrutura
E4Nursing desenvolvida pela Escola Superior de Enfermagem do Porto, no sentido de
aumentar a capacidade reflexiva e raciocínio para a tomada de decisão clinica de
enfermagem. Resultados: Descritas as características dos contextos clínicos onde os
estágios foram realizados (EIHSCP e uma UCP), inclui uma revisão de literatura sobre
os cuidados em últimos dias hora de vida (UDHV). Enfatiza-se uma visão holística do
cliente, considerando as suas diversas dimensões e orientando para as intervenções de
enfermagem em fase de UDHV. O relatório apresenta uma narrativa reflexiva sobre o
desenvolvimento das competências comuns e específicas, em conformidade com os
Regulamentos nº 429/2018, de 16 de julho, e o nº 140/2019, de 6 de fevereiro,
publicados no Diário da República pela Ordem dos Enfermeiros. Promove-se uma
reflexão sobre a prática para o desenvolvimento de um perfil de competências clínicas
que caracterizam a área profissional especializada, assegurando a prestação de
cuidados adequados às necessidades multidimensionais das pessoas em situação
paliativa, bem como da sua Família e referentes diretos.
Introduction: Specialized nursing plays an essential role in palliative care teams, driven by the increased incidence of severe, progressive, and life-limiting illnesses. These conditions generate significant suffering for clients and their families, affecting physical, emotional, spiritual, and social aspects. This document addresses palliative care and presents a Professional Internship Report related to the Master’s in Medical-Surgical Nursing, in the area of Nursing for Persons in a Palliative Situation, at the School of Nursing of Porto. The main objective of this report is to reflectively describe the activities carried out during the internship, highlighting the impact on the development of both the common and specific competencies of the specialist nurse, in two of the three clinical contexts of specialized palliative care provided for in the course curriculum and in the regulations of the Order of Nurses for the attribution of the specialist title. Methodology: The approach is descriptive and reflective, based on the best available scientific evidence, aiming to justify the processes, pathways, and interventions adopted. Two case studies are presented, based on nursing Ontos and the E4Nursing framework developed by the School of Nursing of Porto, in order to develop reflective capacity and reasoning for nursing decision-making. Results: The report describes the characteristics of the clinical contexts in which the internships were conducted, including a literature review on end-of-life care in the final days/hours of life (UDHV). A holistic view of the client is emphasized, considering the various dimensions of their well-being and guiding nursing interventions in UDHV. The report offers a reflective narrative on the development of both common and specific competencies, in accordance with Regulations no. 429/2018, of July 16, and no. 140/2019, of February 6, published in the Official Gazette. It encourages reflection on practice for the development of a clinical competency profile that characterizes the specialized professional area, ensuring the provision of care that adequately meets the multidimensional needs of people in palliative situations, as well as their families and direct relatives.
