Introdução: A peri-implantite é uma condição inflamatória que compromete os tecidos de suporte dos implantes dentårios e representa uma das principais causas de insucesso a longo prazo. A compreensão dos fatores associados ao seu desenvolvimento é essencial para melhorar o prognóstico dos tratamentos reabilitadores com implantes.
Objetivo: Investigar a influĂȘncia dos fatores protĂ©ticos e de variĂĄveis associadas Ă colocação de implantes na peri-implantite.
MĂ©todos: Foi realizado um estudo retrospetivo, incluindo 50 pacientes diagnosticados com peri-implantite, totalizando 209 implantes avaliados. Foram analisadas variĂĄveis relacionadas com o tempo em função, a localização e o posicionamento 3D, o tipo de conexĂŁo, o tipo de prĂłtese e o seu contorno, a presença de ponto de contacto e o espaço para a higienização, o tipo de reabilitação (unitĂĄria,parcial, total), o tipo de fixação, a presença de platform switching, a superfĂcie do implante e outros fatores como a quantidade de gengiva queratinizada, a presença de colapso horizontal e a regeneração Ăłssea na colocação .A presença e a severidade da peri-implantite foram determinadas de acordo com parĂąmetros clĂnicos e radiogrĂĄficos, e as associaçÔes entre variĂĄveis foram avaliadas por testes estatĂsticos de qui-quadrado.
Resultados: A localização dos implantes, a largura de gengiva queratinizada, o tempo em função, o tipo de conexĂŁo, a existĂȘncia de platform switching, o tipo de prĂłtese e a presença de colapso horizontal demonstraram uma associação significativa com a ocorrĂȘncia da doença. A cirurgia de regeneração Ăłssea nĂŁo se associou Ă presença de peri-implantite, mas relacionou-se com a severidade da mesma.
ConclusĂŁo: A peri-implantite apresenta uma etiologia multifatorial, sendo influenciada por diversos fatores. Os resultados reforçam a importĂąncia de protocolos de prevenção individualizados, considerando caracterĂsticas protĂ©ticas e locais especĂficas, bem como a necessidade de estratĂ©gias de tratamento periodontal de suporte adaptadas ao perfil de risco do paciente.

Understanding the factors associated with its development is essential to improve the prognosis of implant-supported rehabilitations. Objective: To investigate the influence of prosthetic factors and variables related to implant placement on peri-implantitis. Methods: A retrospective study was conducted, including 50 patients diagnosed with peri-implantitis, totaling 209 implants evaluated. Variables analyzed included time in function, implant location and 3D positioning, type of connection, type of prosthesis and its contour, presence of proximal contact and space for hygiene, type of rehabilitation (single, partial, full), type of retention, presence of platform switching, implant surface, and other factors such as the width of keratinized mucosa, presence of horizontal collapse, and bone regeneration at placement. The presence and severity of peri-implantitis were determined according to clinical and radiographic parameters, and associations between variables were assessed using chi-square statistical tests. Results: Implant location, width of keratinized mucosa, time in function, type of connection, presence of platform switching, type of prosthesis, and presence of horizontal collapse showed significant association with disease occurrence. Bone regeneration surgery was not associated with the presence of peri-implantitis, but was related to its severity. Conclusion: Peri-implantitis has a multifactorial etiology and is influenced by several factors. The results highlight the importance of individualized prevention protocols, taking into account prosthetic and site-specific characteristics, as well as the need for supportive periodontal treatment strategies adapted to each patientâs risk profile.
