Abstract(s)
Objetivo: O objetivo desta revisão da literatura (revisão narrativa) é abordar a lesão oral potencialmente maligna (LOPM) mais prevalente que é a leucoplasia oral de forma a que seja um instrumento de trabalho simples para os médicos dentistas que com a cobertura nacional que fazem da saúde oral, possam ser também os primeiros a identificar estas lesões potencialmente malignas, permitindo o seu tratamento, levando à vigilância de longo prazo destes doentes e participando assim no esforço global da saúde para a diminuição da taxa de mortalidade por cancro oral em Portugal. Pretende-se com a elaboração desta revisão narrativa elucidar sobre a prevalência da leucoplasia oral, possíveis fatores etiológicos, apresentação clínica, diferentes características e localizações desta LOPM, diagnóstico clínico e caracterização histológica, taxa de transformação maligna descrita nos diferentes estudos internacionais, diagnóstico diferencial e opções terapêuticas. Será ainda discutida a leucoplasia proliferativa verrucosa como entidade independente.
Materiais e Métodos: A presente revisão da literatura será realizada com recurso às mais recentes publicações científicas sobre o tema, utilizando as plataformas como a Embase, Scopus e Web of Science , PubMed e COCHRANE (Cochrane Libraly), esta pesquisa ocorreu entre os meses de janeiro de 2024 e de abril 2024. Conclusão: Esta revisão narrativa resumiu os resultados e conclusões dos artigos que abordam as lesões potencialmente malignas e o cancro oral apos a leitura e revisão aprofundada das mesmas. Não existe acordo sobre qual o tratamento indicado para a leucoplasia quando há somente hiperqueratose sem displasia, estando indicada a monitorização ou a remoção cirúrgica consoante os autores. No caso das leucoplasias com displasia e a leucoplasia verrucosa proliferativa, é recomendado a excisão das mesmas com margens sem lesão. São necessários mais estudos prospectivos, multicêntricos e sequenciamento genómico para o estudo da leucoplasia e investigar os potenciais biomarcadores, promover a importância de vigiar ou tratar qualquer tipo de lesões potencialmente malignas de forma evitar a transformação maligna.
Objective: The aim of this literature review (narrative review) is to address the most prevalent potentially malignant oral lesion (PMOL), which is oral leukoplakia, to provide a simple working tool for dentists. Given their national coverage in oral healthcare, dentists can also be the first to identify these potentially malignant lesions, enabling their treatment and leading to long-term monitoring of these patients. In this way, they contribute to the global health effort to reduce the mortality rate from oral cancer in Portugal. This narrative review intended to clarify the prevalence of oral leukoplakia, possible etiological factors, clinical presentation, different characteristics and locations of this PMOL, clinical diagnosis and histological characterization, the malignant transformation rate reported in different international studies, differential diagnosis, and therapeutic options. Verrucous proliferative leukoplakia will also be discussed as an independent entity. Conclusion: This narrative review summarized the results and conclusions of articles dealing with potentially malignant lesions and oral cancer after reading and thorough review of them. There is no agreement on what treatment is indicated for leukoplasia when there is only hyperkeratosis without dysplasia, and monitoring or surgical removal is indicated according to the authors. In the case of dysplasia leukoplasia and proliferative verrucose leukoplasia, excision of the same is recommended with margins without injury. More prospective, multicentre and genomic sequencing studies are needed to study leukoplasia and investigate potential biomarkers, promote the importance of monitoring or treating any potentially malignant lesions in order to avoid malignant transformation.
