Abstract(s)
A doença renal crónica (DRC) é um distúrbio que afeta a estrutura e a função dos
rins. É reconhecida como um problema de saúde pública mundial que deve ser
gerido desde o seu estágio inicial. A doença e o tratamento são avaliados a partir
da taxa de filtração glomerular (TFG) e albuminúria, e o diagnóstico clínico (causa
e patologia) é classificado de acordo com 5 estágios de gravidade, sendo o primeiro
classificado como inicial e o quinto classificado como grave e com necessidade de
Terapia de Substituição da Função Renal (TSFR), tendo como opções a
Hemodiálise (HD), a Diálise Peritoneal (DP), o Tratamento Médico Conservador
(TMC) e o Transplante Renal (TR).
A estratégia de gestão da DRC é complexa e representa custos elevados para o
Serviço Nacional de Saúde (SNS) pois permeia por vários segmentos, desde a
saúde primária, através da prevenção e promoção da saúde, até ao atendimento
terciário, no qual prevalecem as modalidades terapêuticas para TSFR. Neste
sentido, a qualidade é o ponto fulcral para o equilíbrio entre a eficácia e a eficiência,
a equidade e a acessibilidade, a segurança e a satisfação.
A scoping review realizada propôs responder à seguinte questão: “Quais os
critérios que influenciam a qualidade dos cuidados de enfermagem do doente renal
crónico em hemodiálise?” Identificaram-se critérios relacionados com a segurança
do doente, liderança, ambiente de cuidados, interações de equipas
multidisciplinares, políticas e protocolos, supervisão, auditorias, desenvolvimento
profissional e valor em saúde.
Chronic kidney disease (CKD) is a disorder that affects the structure and function of the kidneys. It is recognized as a global public health problem that must be managed from an early stage. The disease and its management are evaluated using the are glomerular filtration rate, and the clinical diagnosis (cause and pathology) is classified in 5 stages of severity, the first one being initial and the fifth being severe and in need of Replacement Therapy for Renal Function (RTRF), with options for Hemodialysis (HD), Peritoneal Dialysis (PD), Conservative Medical Treatment (CMT) and Renal Transplantation (RT). The management strategy of CKD is complex and represents high costs for the National Health System (NHS), since it triggers several segments, from primary, through health promotion, to the therapies for RTRF. In this sense, quality is the focal point for the balance between effectiveness and efficiency, equity and accessibility, safety, and satisfaction. This scoping review aims to answer: “Which criteria influence the quality of nursing care to patients with chronic kidney disease on hemodialysis” Patient’s security, leadership, health care environment, interactions of multidisciplinary teams, professional policies and protocols, supervision, audits, professional development and health value were defined as criteria.
