Abstract(s)
Este relatório de trabalho de projeto surge no âmbito do Curso do 2º Mestrado em
Enfermagem Médico-Cirúrgica da Escola Superior de Saúde do Instituto Politécnico de
Setúbal e visa descrever e expor uma reflexão crítica sobre: as Competências Comuns e
Específicas em Enfermagem à Pessoa em Situação Crítica, as Competências Específicas em
Enfermagem em Pessoa em Situação Crónica e Paliativa e as Competências do Mestre em
Enfermagem Médico-Cirúrgica, desenvolvidas durante os estágios realizados no âmbito do
Curso supracitado, bem como o trabalho elaborado no decorrer dos mesmos.
Os estágios, acima mencionados decorreram na Unidade de Cuidados Paliativos
(UCP) do hospital Esperança, onde desenvolvemos um Projeto de Intervenção em Serviço
(PIS) e um Projeto de Aprendizagem Clínica (PAC) de acordo com a metodologia de
projeto.
No âmbito do PIS identificamos como problemática: inexistência de uniformização
na avaliação/registo do delírium em clientes internados na Unidade supramencionada, bem
como a inexistência de uma escala de avaliação instituída, para esse fim. Assim, planeámos,
executámos e avaliámos atividades que permitiram obter os objetivos inicialmente traçados,
que consistiam em: elaborar uma norma de procedimento de enfermagem a clientes com
delírium, internados na UCP do hospital Esperança; dotar a equipa de formação na área do
delírium em Cuidados Paliativos e da escala Método de Avaliação da Confusão (escala
CAM) e implementar a escala CAM na Unidade, acima mencionada.
A elaboração do PAC permitiu-mos adquirir/desenvolver as competências
específicas do enfermeiro especialista em enfermagem à pessoa em situação crítica: Cuida
da pessoa a vivenciar processos complexos de doença crítica e/ou falência orgânica,
Dinamiza a resposta a situações de catástrofe ou emergência multi-vítimas, da conceção à
ação e Maximiza a intervenção na prevenção e controlo da infeção perante a pessoa em
situação crítica e/ou falência orgânica face à complexidade da situação e à necessidade de
respostas em tempo útil e adequadas.
Abstract: This report project work comes within the course of the 2nd Master in Medical- Surgical Nursing, School of Health, and Polytechnic Institute of Setúbal. This report project work aims to describe and present a critical reflection about the Common and Specific Competencies, the Nursing Person in Critical Situation, Specific Competencies in nursing in Person in chronic and palliative Situation and Competencies of the Master in Medical- Surgical Nursing, developed during courses conducted under the above course, as well as the work done in the course thereof. The stages were held above the Palliative Care Unit (UCP) Hope Hospital, where he developed an Project Intervention in Service (PIS) and Clinical Learning Project (PAC). Under the PIS identified as problematic: lack of uniformity in the evaluation / registration of delirium in hospitalized clients in unit mentioned above and the absence of a rating scale established for this purpose. Thus, we planned executed and evaluated activities which achieved the initial goals, which were to: develop a standard procedure nursing clients with delirium admitted to the hospital UCP Hope; giving the team training in the area of delirium in palliative care and scale of the Confusion Assessment Method (CAM scale ) and the scale CAM implement in Unity, above. The preparation of the PAC allowed we acquire / develop the specific competencies of the nurse specialist in nursing the person in critical condition: “Care of the person that experience complex process of the critic disease and / or organic failure and streamline the response to catastrophe situation or emergency multi-victim, form the idea to action and maximize the intervention on the prevention and control of infection towards the person in critic situation and/or organic failure taking into consideration the complexity of the situation and the need of getting adequate responses in useful time”.
