Abstract(s)
A elaboração deste Relatório de Estágio pretende demonstrar todo o percurso de
aprendizagem desenvolvido durante a Unidade Curricular de Estágio com Relatório,
integrado no Curso de Mestrado em Enfermagem de Saúde Materna e Obstetrícia.
No processo de aprendizagem para o desenvolvimento de competências; científica,
técnica e humana para o cuidar de enfermagem especializado, priviligiei a prática
baseada na evidência e a reflexão crítica da prática clínica baseada na Teoria dos
Cuidados de Kristen Swanson.
O conceito de alta precoce após o parto não é consensual, mas é considerada benéfica
para a tríade. Perante a tendência em reduzir o tempo de internamento após o
nascimento, acentuado pelo contexto pandémico, surge uma maior necessidade de
cuidados especializados nos primeiros dias após a alta hospitalar, atendendo que nesta
fase é importante avaliar o estado de saúde da mulher, do recém-nascido, bem como a
adaptação da díade/tríade ao seu novo papel, de modo a facilitar o desenvolvimento de
competências parentais.
Como metodologia de eleição foi desenvolvida uma Scoping Review, segundo as
orientações do Joanna Briggs Institute, com a finalidade de análisar a evidência
científica disponível sobre “O Cuidar do Enfermeiro Obstetra à Puérpera e Recém nascido após a Alta Precoce”. A pesquisa foi realizada na plataforma EBSCO, nas
bases de dados MEDLINE Complete, CINAHL Complete e Cochrane Database of
Systematic Reviews.
Realizaram-se entrevistas telefónicas, após 24 horas da alta hospitalar, a 26 puérperas
com alta precoce no sentido de compreender as dificuldades e a satisfação da mulher e
família após a alta.
Conclui-se que a alta precoce é motivo de satisfação para as mulheres/famílias, tendo se considerado ser benéfico regressar ao ambiente familiar, mais tranquilo, confortável,
com maior interação do pai e outros apoios familiares. O hospital teve um ganho médio
de 20 horas por puérpera, com implicações na gestão de camas e nos custos
relacionados com o internamento. Importa investir na alta precoce e na articulação e
continuidade de cuidados.
The elaboration of this Internship Report intends to demonstrate the entire learning path developed during the Internship with Report Curricular Unit, which is part of the Master's Degree Course in Maternal Health and Midwifery. The learning process for the development of scientific, technical, and human competences for specialized nursing care, I privileged evidence-based practice and critical reflection of clinical practice based on Kristen Swanson's Theory of Care. Although early discharge after delivery is not consensual, but it is considered beneficial for the triad. In view of the tendency to reduce the length of hospital stay after birth, accentuated by the pandemic context, there is a greater need for specialized care in the first days after hospital discharge, with special focus to women’s and newborn’s health assessment, as well as the adaptation of the dyad/triad to its new role, in order to facilitate parenting skills development. As a methodology of choice, a Scoping Review was developed, according to the guidelines of the Joanna Briggs Institute, with the purpose of analyzing the available scientific evidence on “"The care provided by Midwives to post-partum women and newborns after early discharge”. The research was carried out on the EBSCO platform, in the MEDLINE Complete, CINAHL Complete and Cochrane Database of Systematic Reviews databases. Telephone interviews were carried out 24 hours after hospital discharge with 26 postpartum women with early discharge in order to understand the difficulties and satisfaction of the woman and family after discharge. We can concluded that early discharge promotes women/families’ satisfaction, and it was considered beneficial to return to the family environment, more peaceful, comfortable, with greater interaction from the father and other family support. Hospital wise, early discharge was also beneficial, with an average gain of 20 hours per post-partum woman. The release of hospital resources has direct implications in bed management and hospitalization-related costs. Therefore, this should be taken in account to invest more attention in early discharge, continuity of care and link with community services.
