Este relatório emerge da Unidade Curricular “Estágio de Natureza Profissional com Relatório”,
Módulo I e Módulo II, correspondendo à finalização do percurso académico no Mestrado em
Enfermagem Médico-Cirúrgica, na área da Enfermagem à Pessoa em Situação Crítica, da Escola
Superior de Enfermagem do Porto. Seguindo uma metodologia descritiva e de análise críticoreflexiva,
procuro apresentar o processo de desenvolvimento de competências, comuns e
específicas, do Enfermeiro Especialista, em conformidade com os Regulamentos n.º 140/2019 e
n.º 429/2018, da Ordem dos Enfermeiros.
O estágio concretizou-se em três contextos distintos, Serviço de Medicina Intensiva Polivalente,
Unidade Local do Programa de Prevenção e Controlo de Infeções e de Resistência aos
Antimicrobianos (UL-PPCIRA) e Serviço de Urgência. Nos contextos de estágio tive oportunidade
de aplicar e integrar os conhecimentos previamente adquiridos em contexto teórico, na
assistência à pessoa em situação crítica. A fundamentação da prática clínica de enfermagem
sustentou-se na plataforma de aprendizagem E4Nursing, com base na ontologia de Enfermagem
NursingOntos. O tema central que norteou o processo de desenvolvimento de competências foi
a prevenção da infeção urinária associada ao cateter vesical, com particular enfoque em
atividades para melhorar a aplicação da norma n.º 019/2015 atualizada a 29/08/2022, da
Direção-Geral da Saúde, na prática clínica dos enfermeiros.
A infeção associada aos cuidados de saúde (IACS) é um problema grave e transversal a vários
países e instituições de saúde, com implicações na morbilidade e mortalidade dos doentes, nos
custos e no consumo de recursos na saúde. De acordo com o European Center for Disease
Prevention and Control (ECDC) a infeção urinária associada ao cateter vesical tem uma
prevalência elevada tanto em termos globais como em contexto de cuidados intensivos. Nesta
conjuntura, o enfermeiro especialista, enquanto profissional dotado de conhecimentos e
competências diferenciadas, é um elemento essencial, não só no âmbito da prática clínica, mas
também no diagnóstico de necessidades/problemas, planeamento e implementação de
intervenções concertadas para mitigar a infeção e garantir a melhoria da qualidade dos
cuidados de enfermagem.
This report emerges from the Curricular Unit “Estágio de Natureza Profissional com Relatório”, Module I and Module II, corresponding to the conclusion of the Master's Degree in Medical- Surgical Nursing, in the area of critical care;, at the Escola Superior de Enfermagem do Porto. Following a descriptive methodology and critical-reflective analysis, I seek to present the process of developing common and specific skills of the Specialist Nurse, in accordance with Regulations no. 140/2019 and no. 429/2018 of the Ordem dos Enfermeiros. The internship took place in three different contexts, Multipurpose Intensive Medicine Service, Local Unit of the Infection Prevention and Control and Antimicrobial Resistance Program (ULPPCIRA) and Emergency Service. In internship contexts, I had the opportunity to apply and integrate the knowledge previously acquired in a theoretical context, in assisting people in critical situations. The foundation of clinical nursing practice was supported by the E4Nursing learning platform, based on the nursing ontology NursingOntos. The central theme that guided the skills development process was the prevention of urinary infection associated with the urinary catheter, with particular focus on activities to improve the application of standard no. 019/2015 of Direção Geral de Saúde, in the clinical practice of nurses. Healthcare-associated infection (HAI) is a serious problem that crosses several countries and healthcare institutions, with implications for patient morbidity and mortality, costs and consumption of healthcare resources. According to the European Center for Disease Prevention and Control (ECDC), urinary tract infections associated with urinary catheters have a high prevalence both globally and in the context of intensive care. At this juncture, the specialist nurse, as a professional endowed with differentiated knowledge and skills, is an essential element, not only in the scope of clinical practice, but also in diagnosing needs/problems, planning and implementing concerted interventions to mitigate infection and ensure improving the quality of nursing care. The report follows a structural sequence that includes the characterization of the internship contexts; the presentation of two clinical cases supporting the design of care using the E4Nursing platform; the analysis and reflection of the contributions of different activities to the development of common and specific skills of a nurse specializing in critical care; and the presentation of a final synthesis, with a global reflection on my skills development journey.
