O presente relatório realizado no âmbito do Mestrado em Enfermagem Médico-Cirúrgica, na
vertente de Enfermagem à Pessoa em Situação Crítica, na Escola Superior de Enfermagem do
Porto, pretende apresentar uma descrição crítico-reflexiva das experiências vivenciadas ao
longo do Estágio de Natureza Profissional com Relatório, com especial enfoque para o
desenvolvimento de competências comuns e especificas do Enfermeiro Especialista com base
no Regulamento n.º 140/2019 e n.º 429/2018 da Ordem dos Enfermeiros assim como os saberes
adquiridos. Pretende-se de igual forma, espelhar as competências de conceção e prestação de
cuidados de enfermagem à pessoa em situação crítica; a implementação e gestão de protocolos
terapêuticos de especial complexidade e mapear as intervenções de enfermagem com
particular enfoque no controlo da hipertermia da pessoa em situação crítica.
A hipertermia constituiu ao longo do estágio um dos diagnósticos com maior incidência na
pessoa em situação critica e as intervenções de Enfermagem para controlo da hipertermia não
são totalmente evidentes. Por outro lado, a hipertermia na pessoa em situação crítica está
fortemente associada a resultados negativos para o cliente. Neste sentido torna-se crucial
mapear as intervenções de enfermagem associadas à hipertermia na pessoa em situação
crítica.
O projeto de desenvolvimento de competências clínicas especializadas, intitulado “Intervenções
de Enfermagem associadas ao compromisso da termorregulação na pessoa em situação crítica:
hipertermia”, guiou o meu percurso durante os estágios em três contextos clínicos distintos:
Unidade de Cuidados Intermédios, Serviço de Urgência e Serviço de Medicina Intensiva. O
estudo de caso apresentado ilustra a complexidade dos cuidados prestados num Serviço de
Medicina Intensiva, refletindo a tomada de decisão do enfermeiro especialista em contextos
clínicos exigentes.
Para a elaboração deste relatório utilizou-se a plataforma pedagógica "E4nursing"
disponibilizada pela Escola Superior de Enfermagem do Porto.
This report, developed within the scope of the Master's Degree in Medical-Surgical Nursing, with a focus on Nursing Care for the Critically Ill Person, at the Escola Superior de Enfermagem do Porto (Nursing School of Porto), aims to present a critical and reflective description of the experiences lived throughout the Professional Internship with Report. It places special emphasis on the development of common and specific competencies of the Specialist Nurse, based on Regulations No. 140/2019 and No. 429/2018 of the Portuguese Nursing Council (Ordem dos Enfermeiros), as well as the knowledge acquired. The report also aims to reflect the competencies in designing and delivering nursing care to critically ill individuals, the implementation and management of highly complex therapeutic protocols, and the mapping of nursing interventions with a particular focus on the control of hyperthermia in critically ill patients. During the internship, hyperthermia emerged as one of the most frequent diagnoses in critically ill patients, and nursing interventions for hyperthermia control were not always clearly defined. Furthermore, hyperthermia in critically ill patients is strongly associated with negative patient outcomes. Therefore, it is crucial to map nursing interventions related to hyperthermia in critically ill individuals. The clinical skills development project, titled “Nursing Interventions Associated with Thermoregulation Impairment in Critically Ill Patients: Hyperthermia,” guided my journey through internships in three different clinical settings: the Intermediate Care Unit, the Emergency Department, and the Intensive Care Unit. The presented case study illustrates the complexity of care provided in an Intensive Care Unit, reflecting the decision-making process of the specialist nurse in demanding clinical contexts. The "E4nursing" pedagogical platform, made available by the Escola Superior de Enfermagem do Porto, was used in the preparation of this report.
