Os alinhadores ortodônticos transparentes surgiram como uma alternativa estética e confortável aos aparelhos fixos convencionais, oferecendo uma solução mais discreta e funcional para o tratamento ortodôntico. Além disso, permitem um planeamento digital preciso dos movimentos dentários, o que proporciona maior previsibilidade e personalização do tratamento, adaptando-se às necessidades específicas de cada paciente.
Paralelamente, a ancoragem esquelética por meio de micro implantes, trouxe uma revolução na biomecânica ortodôntica. Ao serem inseridos diretamente no osso, esses dispositivos permitem a aplicação de forças ortodônticas sem a necessidade de apoio nos dentes adjacentes, evitando assim efeitos indesejados como a perda de ancoragem ou movimentos dentários não planeados. Os micro implantes destacam-se ainda pela sua simplicidade de instalação, segurança clínica e eficácia mesmo em espaços reduzidos da arcada dentária.
A combinação entre alinhadores e micro implantes representa, assim, um avanço significativo nas abordagens terapêuticas ortodônticas contemporâneas. Esta sinergia torna possível a execução de movimentos dentários complexos e altamente controlados, que seriam de difícil realização apenas com alinhadores. A sua associação com alinhadores permite um menor desconforto para o paciente e uma abordagem mais personalizada. Em casos clínicos que exigem movimentações extensas, retrações anteriores, intrusões, esta combinação tem-se mostrado particularmente vantajosa, contribuindo para tratamentos mais eficazes, previsíveis e satisfatórios.
O objetivo desta revisão narrativa é avaliar de que forma os mecanismos de ancoragem esquelética com recurso a micro implantes, quando associados ao tratamento ortodôntico com alinhadores transparentes, podem permitir obter resultados clínicos mais eficazes e previsíveis.
Clear orthodontic aligners emerged as an aesthetic and comfortable alternative to conventional fixed braces, offering a more discreet and functional solution for orthodontic treatment. In addition, they allow precise digital planning of tooth movements, which provides greater predictability and personalization of treatment, adapting to the specific needs of each patient. At the same time, skeletal anchorage using micro implants has revolutionized orthodontic biomechanics. By being inserted directly into the bone, these devices allow the application of orthodontic forces without the need for support from adjacent teeth, thus avoiding unwanted effects such as loss of anchorage or unplanned tooth movements. Micro implants also stand out for their ease of installation, clinical safety and effectiveness even in small spaces in the dental arch. The combination of aligners and micro implants thus represents a significant advance in contemporary orthodontic therapeutic approaches. This synergy makes it possible to perform complex and highly controlled tooth movements that would be difficult to perform with aligners alone. Their combination with aligners allows for less discomfort for the patient and a more personalized approach. In clinical cases that require extensive movements, anterior retractions, and intrusions, this combination has proven to be particularly advantageous, contributing to more effective, predictable, and satisfactory treatments. The objective of this narrative review is to evaluate how skeletal anchorage mechanisms using micro implants, when associated with orthodontic treatment with clear aligners, can allow for more effective and predictable clinical results.
