A dor crónica é um problema frequente das pessoas idosas que tem grande impacto
na sua autonomia e independência, podendo contribuir para o aparecimento ou
agravamento das designadas grandes síndromes geriátricas. O conhecimento das
expetativas da pessoa idosa com dor crónica e os fatores que as influenciam é um
processo de avaliação contínuo ao longo do percurso de cuidados numa unidade de
dor. Este projeto decorreu num centro multidisciplinar dor e numa unidade de saúde
familiar de Lisboa e Vale do Tejo, com a finalidade do desenvolvimento de
competências como enfermeira mestre e especialista nos cuidados à pessoa idosa e
sua família, nomeadamente através da implementação de um projeto de intervenção
em enfermagem que permitiu o conhecimento das expetativas da pessoa idosa com
dor crónica na primeira consulta de enfermagem para a promoção do Cuidado-de-Si.
O modelo teórico de enfermagem norteador do projeto foi o modelo de parceria de
Gomes (2020), visando a prática de cuidados em parceria com a pessoa idosa com
dor crónica na promoção do Cuidado-de-Si. Como resultados constatámos nas
entrevistas que as pessoas idosas tinham a expetativa do alívio da dor e algumas
esperavam a cura total. Na perspetiva das enfermeiras é necessário envolver a
pessoa idosa no processo de avaliação e quantificação do alívio da dor e definirem
em conjunto expetativas realistas, que ajudam não só no controlo da dor, mas também
na aceitação da dor crónica. O confronto da equipa com a escassez de registo das
expetativas na avaliação inicial da primeira consulta de enfermagem à pessoa idosa
com dor crónica no Centro Multidisciplinar Dor expôs a dificuldade em identificar as
expetativas como um problema ao nível das intervenções de enfermagem o que
dificulta a realização de diagnósticos e de intervenções de enfermagem nesta área. O
percurso realizado promoveu o desenvolvimento de competências como enfermeira
especialista e mestre na área da intervenção à pessoa idosa e família, e o
desenvolvimento de competências na equipa de enfermagem nos cuidados à pessoa
idosa e família, para a promoção do Cuidado-de-Si, salientado a importância do
conhecimento das expetativas da pessoa idosa com dor crónica na primeira consulta
de enfermagem num centro multidisciplinar dor para melhoria da pratica clinica e
necessidade de investir em estudos de investigação nesta área.
Chronic pain is a common problem of older people which has a great impact on their autonomy and independence and can contribute to the onset or worsening of the so called major geriatric syndromes. The knowledge of the expectations of the elderly people with chronic pain and of the factors that influence them is a process of continuous assessment throughout the work of providing care in a pain clinic. This project took place at a multidisciplinary pain center and at a family healthcare unit in Lisbon and Tagus Valley region, with the aim of developing skills as a nurse with a master degree and as a specialist in healthcare for the elderly and their family, in particular through the implementation of an nursing intervention project which has allowed to collect knowledge about the expectations of the elderly with chronic pain at the first nursing consultation in order to promote care of the self. The nursing theoretical model guiding the project was Gomes partnership model (2020), aimed at providing care in partnership with the elderly person who suffers from chronic pain in promoting care of the self. As a result, we noted in the interviews that older people had the expectation of pain relief and some expected full cure. From the perspective of the nurses, it is necessary to involve the elderly in the evaluation and quantification of pain relief and in jointly defining realistic expectations, which helps not only in the control of pain, but also in the acceptance of chronic pain. The fact that the team was confronted with the scarcity of data on patient expectations in the initial evaluation of the first nursing consultation of the elderly person with chronic pain at the Multidisciplinary Pain Center exposed the difficulty in identifying the expectations as a problem at the level of nursing interventions making it difficult to perform diagnostics and nursing interventions in this area. The work undertaken promoted the development of skills as a specialist and as a nurse with a master degree in the area of intervention concerning the elderly and their relatives, as well as the development of skills in the nursing team in the care provided to the elderly person and his or her family, to promote the care of the self, underlying the importance of getting to know what are the expectations of the elderly with chronic pain during the first nursing consultation in a Multidisciplinary Pain Center in order to improve clinical practice and the need to invest in research studies in this area.
