O relatório de estágio de natureza profissional, realizado no âmbito do Mestrado em
Enfermagem Médico-Cirúrgica, na vertente de Enfermagem à Pessoa em Situação Crítica, da
Escola Superior de Enfermagem do Porto, apresenta uma síntese do percurso realizado focado
no desenvolvimento de competências comuns e específicas do Enfermeiro Especialista. Este
relatório emergiu das Unidades Curriculares Estágio de natureza profissional com relatório
(Módulo I – 15 ECTS e Módulo II – 30 ECTS) que teve como foco particular a abordagem à pessoa
em situação crítica com compromisso da limpeza da via aérea. Desta forma, foi delineado e
consolidado um projeto de desenvolvimento profissional com a temática: a Pessoa em Situação
Crítica, com compromisso da limpeza da via aérea - aspiração da via aérea segura.
O compromisso da limpeza da via aérea é frequentemente um domínio de atenção para a
prática de Enfermagem, especialmente no contexto da pessoa em situação crítica. A sedação, a
presença de uma via aérea artificial e disfunções cardiorrespiratórias ou neurológicas
comprometem a mobilização das secreções e o reflexo da tosse.
A aspiração da via aérea é um procedimento recorrente e essencial, mas apresenta riscos, tais
como dessaturação, alterações hemodinâmicas, lesões na mucosa e infeções. A gestão
inadequada da limpeza da via aérea pode prolongar a ventilação mecânica e aumentar o tempo
de internamento hospitalar. Torna-se imperativo a adequação de intervenções que assegurem
uma aspiração da via aérea segura no contexto da pessoa em situação crítica, através da
orientação de guidelines e implementação de protocolos.
Estruturalmente este relatório está organizado em três partes. A primeira, centra-se na
caracterização dos quatro contextos clínicos onde foram realizados os estágios. Na segunda
parte é apresentado um plano de conceção de cuidados, onde é detalhado o processo de
conceção e tomada de decisão clínica. Por último, é apresentado um capítulo crítico e reflexivo
da aquisição de conhecimentos e desenvolvimento de competências especializadas no âmbito
da Enfermagem.
The professional internship report, conducted within the scope of the Master’s Degree in Medical-Surgical Nursing, with a specialization in Nursing Care for the Critically Ill Person, at the Nursing School of Porto, presents a synthesis of the trajectory undertaken, focused on the development of both common and specific competencies of the Specialist Nurse. This report results from the curricular units "Professional Internship with Report" (Module I – 15 ECTS and Module II – 30 ECTS), which specifically targeted the approach to critically ill patients with compromised airway clearance. Accordingly, a professional development project was designed and consolidated under the theme: The Critically Ill Person with Compromised Airway Clearance – Safe Airway Suctioning. Airway clearance impairment is frequently a focal area in nursing practice, particularly in the context of critically ill patients. Sedation, the presence of an artificial airway, and cardiopulmonary or neurological dysfunctions compromise secretion mobilization and the effectiveness of the cough reflex. Airway suctioning is a recurrent and essential procedure; however, it carries inherent risks, including oxygen desaturation, hemodynamic alterations, mucosal injury, and infection. Inadequate airway clearance management may prolong mechanical ventilation and increase hospital length of stay. Therefore, it is imperative to adopt interventions that ensure safe airway suctioning in critically ill patients, guided by evidence-based guidelines and the implementation of standardized protocols. Structurally, this report is organized into three sections. The first focuses on the characterization of the four clinical settings where the internships were conducted. The second section presents a nursing care planning framework, detailing the clinical reasoning and decision-making process. Finally, a critical and reflective chapter is presented, addressing the acquisition of knowledge and the development of specialized nursing competencies.
