O presente relatório de estágio descreve o processo de aprendizagem no desenvolvimento de competências durante o Curso de Mestrado em Saúde Materna e Obstétrica, realizado no Centro Materno-Infantil do Norte, entre 6 de fevereiro de 2023 e 30 de novembro de 2023. Esta análise retrospetiva aborda a aquisição e o desenvolvimento de competências relacionadas à prática profissional do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica, conforme o Regulamento de Competências do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstetrícia, estabelecido e aprovado pela Ordem dos Enfermeiros, em conformidade com a Mesa de Colégio da Especialidade de Saúde Materna e Obstétrica. Este relatório de estágio contempla ainda uma abordagem à componente de investigação em Enfermagem de Saúde Materna e Obstétrica.
O estágio profissional abrangeu três áreas distintas de intervenção em enfermagem, especificamente a Gravidez com Complicações, Trabalho de Parto e Parto, e Cuidados Pós-Parto e Parentalidade. Durante esses três contextos de aprendizagem, foram implementadas intervenções de enfermagem baseadas na evidência científica mais recente, visando o desenvolvimento de competências no domínio dos cuidados especializados de enfermagem para todas as grávidas, parturientes, puérperas e seus recém-nascidos. Este relatório documenta as experiências vivenciadas em todos os locais de estágio, seguidas de uma análise reflexiva das práticas hospitalares.
Para o desenvolvimento das competências de investigação, foi elaborada uma Revisão Integrativa da Literatura, abordando as técnicas "Hands-On" versus "Hands-Off" durante o período expulsivo, com o intuito de mitigar lacerações perineais e minimizar as sensações adversas durante o pós-parto. Este estudo pretendeu destacar o papel do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica na promoção de resultados obstétricos mais favoráveis para a puérpera.
Os protocolos atuais e a prática clínica sugerem a adoção da técnica “Hands-On” como a mais eficaz, contradizendo com os estudos desta Revisão. Assim, poderá ser importante ou elaborar uma revisão sistemática da literatura, permitindo sintetizar toda a evidência disponível sobre a prática ou efetuar estudos primários nomeadamente estudos randomizados apenas destas duas técnicas fornecendo uma maior evidência científica atual.
The present internship report describes the learning process of developing skills in the Master's Degree on Maternal and Obstetric Health Nursing, carried out at the Centro Materno-Infantil do Norte, between the 6th of February 2023 and the 3rd of November 2023. This retrospective analysis addresses the acquisition and development of skills related to the professional practice of a Specialist Nurse in Maternal and Obstetric Health Nursing, according to the Regulation of Competencies for the Maternal and Obstetric Nurse Specialist, established and approved by the Ordem dos Enfermeiros, in accordance with the Mesa do Colégio da Especialidade de Saúde Materna e Obstétrica. This internship report also includes a research component in Maternal and Obstetric Health Nursing. Professional internship covered three distinct areas of nursing intervention, namely High-risk Pregnancy, Labor and Delivery, as well as Postpartum Care and Parenthood. During these three learning scenarios, nursing interventions were implemented based on the most recent scientific evidence, aiming to develop skills in specialized nursing care for all pregnant women, parturients, postpartum women, and their newborns. This report documents occurrences experienced in all internship locations, followed by a reflexive analysis of hospital practices. For the development of research skills, an integrative literature review was elaborated, addressing "Hands-On" versus "Hands-Off" techniques during the expulsion phase, aimed at mitigating perineal lacerations and minimizing adverse sensations during the postpartum period. This study aimed to highlight the role of a Specialist Nurse in Maternal and Obstetric Health in promoting positive obstetric outcomes for postpartum women. Current protocols and clinical practice suggest that the "Hands-On" technique is the most effective, contradicting the studies in this review. It may therefore be important to either carry out a systematic review of the literature, making it possible to synthesise all the available evidence on the practice, or to carry out primary studies, namely randomised studies of just these two techniques, providing more current scientific evidence.
