Doença de Parkinson, as disfunções temporomandibulares e o bruxismo : estudo piloto

Abstract(s)
Introdução: A relação entre o bruxismo e a doença de Parkinson (DP) ainda não é clara e está pouco estudada. A DP, doença crónica relacionada com a diminuição de dopamina, e o bruxismo regulado a nível central com desempenho importante na rede dopaminérgica, podem levar a movimentos involuntários na região orofacial. Além disso, pessoas com DP, bem como pessoas com bruxismo relatam sintomas de disfunção temporomandibular (DTM).
Objetivo: Analisar a relação entre a DP, a perceção de bruxismo e sinais e sintomas de DTM.
Materiais e métodos: Foram convidados a participar no estudo indivíduos com e sem diagnóstico de DP, com características cognitivas e técnicas para responder a um questionário online, com idade superior ou igual a 18 anos. Após obtenção do consentimento completaram um questionário constituído por questões sobre demografia, diagnóstico de DP, perceção de bruxismo (questionário de auto-perceção), nível de ansiedade (generalized anxiety disorder – GAD-7), sinais e sintomas de DTM (DC/TMD) (questionário de sintomas), Graded Chronic Pain Scale, e localizações de dor (diagramas de desenho de dor). Utilizou-se o teste qui-quadrado para a avaliação da associação entre as variáveis, e estabelecido um nível de significância de 5% (p < 0,05).
Resultados: A amostra integrou 65 (18,8%) indivíduos com diagnóstico de DP, e 281 (81,2%) indivíduos sem diagnóstico de DP. Os resultados revelaram que a prevalência de sinais e sintomas de DTM nos indivíduos sem diagnóstico de DP foi maior e estatisticamente significativa (p = 0,001). Indivíduos com sinais e sintomas de DTM apresentaram maior perceção de bruxismo.
Conclusões: Verificou-se a relação entre a presença de sinais e sintomas de DTM e a perceção de bruxismo. No entanto, as relações entre a presença de sinais e sintomas de DTM e o diagnóstico de DP, e entre a perceção de bruxismo e o diagnóstico de DP necessitam de mais estudos.

In addition, patients with PD as well as patients with bruxism report symptoms of temporomandibular dysfunction (TMD). The chi-square test was used to assess the association between variables, and a significance level of 5% (p < 0.05) was established. Results: The sample comprised 65 (18.8%) participants diagnosed with PD, and 281 (81.2%) participants without a diagnosis of PD. The results revealed that the prevalence of TMD signs and symptoms in people not diagnosed was higher and statistically significant (p = 0.001). People with TMD signs and symptoms had a higher perception of bruxism. Conclusion: There was a relationship between the presence of TMD signs and symptoms and the perception of bruxism. However, the relationship between the presence of TMD signs and symptoms and the diagnosis of PD, and between the perception of bruxism and the diagnosis of PD requires further studies.
