Abstract(s)
O presente relatório, inserido na unidade curricular “Estágio de natureza profissional Módulo II”,
foi elaborado no âmbito da segunda edição do Mestrado em Enfermagem Médico-Cirúrgica, na
área da Enfermagem à Pessoa em Situação Crítica, no ano letivo 2022/2024, da Escola Superior
de Enfermagem do Porto e descreve o percurso de desenvolvimento e aquisição de
competências comuns e específicas do Enfermeiro Especialista na área de Enfermagem à
pessoa em situação crítica, tal como indicado nos Regulamentos nº140/2019 e nº429/2018.
O estágio de natureza profissional, inserido no referido Mestrado decorreu em contextos da
prática clínica dirigidos à pessoa em situação critica, sendo eles um serviço de urgência
polivalente, um serviço de medicina intensiva polivalente e uma unidade de cuidados
intermédios polivalente. Este percurso teve como objetivos desenvolver competências de
conceção e prestação de cuidados de enfermagem à pessoa em situação crítica; a
implementação e gestão de protocolos terapêuticos de especial complexidade e particular
enfoque na gestão e controlo da dor na pessoa em situação critica incapaz de verbalizar a
mesma.
O reconhecimento da presença de dor na pessoa em situação crítica, seja pela causa subjacente
da patologia ou pelas atitudes terapêuticas necessárias, é muitas vezes subvalorizado. A
experiência dolorosa pode ainda ser agravada pela dificuldade de comunicação, medo e
ansiedade, repercutindo-se num aumento do risco de complicações, aumento da instabilidade
clínica e diminuição do conforto e bem-estar do doente (Nordness et al., 2021). Assim, e
partindo da premissa de que a incapacidade de reportar a dor não anula a sua existência, tornase
fundamental a sua avaliação, monitorização e posterior controle (Ferreira et al., 2014).
A estrutura deste relatório inclui a caraterização dos contextos da prática clínica onde estagiei,
a apresentação da conceção de cuidados de três casos clínicos fundamentados no processo de
tomada de decisão e baseados na ontologia em enfermagem e a análise crítico-reflexiva sobre o
contributo das diferentes atividades para o desenvolvimento das competências comuns e
específicas na área de Enfermagem à Pessoa em Situação Crítica.
Em suma, a experiência de estágio e o processo de elaboração do relatório daí decorrente,
foram os catalisadores para o meu desenvolvimento pessoal e profissional, despertando um
olhar focado no exercício de um papel mais ativo na melhoria das práticas clínicas em
enfermagem.
The present report, part of the curricular unit "Professional Internship Module II", was developed within the scope of the second edition of the Master's Degree in Medical-Surgical Nursing, in the Critical Care Nursing area, in the academic year 2022/2024, at the Porto Nursing School. It describes the journey of development and acquisition of common and specific competencies of the Specialist Nurse in the field of Nursing for critically ill patients, as indicated in Regulations No. 140/2019 and No. 429/2018. The professional internship, included in the mentioned Master's Degree, took place in clinical practice contexts focused on critically ill patients, including a versatile emergency service, a versatile intensive care unit, and an optional context, a versatile intermediate care unit. The objectives were to develop skills for designing and providing nursing care for critically ill patients, implementing and managing therapeutic protocols of special complexity, with particular emphasis on managing and controlling pain in critically ill patients unable to verbalize it. Recognition of pain in critically ill patients, whether due to the underlying cause of the pathology or the necessary therapeutic attitudes, is often undervalued. The painful experience can be exacerbated by communication difficulties, fear, and anxiety, leading to an increased risk of complications, clinical instability, and decreased patient comfort and well-being (Nordness et al., 2021). Therefore, assuming that the inability to report pain does not negate its existence, its assessment, monitoring, and subsequent control become fundamental (Ferreira et al., 2014). The structure of this report includes the characterization of the clinical practice contexts where I interned, the presentation of the care conception of three clinical cases based on the decisionmaking process and grounded in nursing ontology, and a critical-reflexive analysis of the contribution of different activities to the development of common and specific competencies in the area of Nursing for Critically Ill Patients. In summary, the internship experience and the subsequent report writing process were catalysts for my personal and professional development, awakening a focused look at playing a more active role in improving clinical nursing practices.
