Abstract(s)
A elaboração do presente relatório surge no âmbito do desenvolvimento do Estágio de Natureza Profissional com relatório- Módulo I e Módulo II do 1º Curso de Mestrado em Enfermagem Médico-cirúrgica na área de Enfermagem à Pessoa em Situação Paliativa da Escola Superior de Enfermagem do Porto. Tem como objetivo descrever e analisar as atividades desenvolvidas, refletir acerca das aprendizagens e do processo de desenvolvimento das competências. O método utilizado para a elaboração deste documento foi o descritivo e reflexivo, recorrendo-se à melhor evidência disponível, à contextualização dos factos e ao desenvolvimento de competências, tendo por base o disposto nos regulamentos, bem como as experiências e oportunidades vividas em cada contexto clínico.
No que respeita à identificação das necessidades das pessoas significativas em situação de últimos dias ou horas de vida é consensual que deve ser feita o mais precocemente possível e, para isso, é obrigatório conhecer o doente/família e incluí-los no processo de tomada de decisão. A elaboração do plano individual de cuidados deve ter como objetivo dar resposta às necessidades dos doentes e das pessoas significativas. A comunicação em cuidados paliativos é um dos pilares dos cuidados e a principal ferramenta que os profissionais utilizam para diagnosticar as necessidades do doente e das pessoas significativas. Salientam-se as entrevistas clínicas e as conferências familiares como meios indispensáveis para identificação de necessidades e mediação do processo de doença. Como principais necessidades das pessoas significativas em situação de últimos dias ou horas de vida, salientamos: necessidade de informação, necessidade de estar junto do doente e de continuar a cuidar; necessidade de suporte/apoio emocional. A intervenção junto das pessoas significativas em situação de últimos dias ou horas de vida deve seguir uma abordagem multidimensional (espiritual, física, educativa e psicossocial).
A componente critico-reflexiva dos casos clínicos e de todo o percurso formativo foi preponderante para a aquisição de novas competências no que respeita ao processo de tomada de decisão, pelo que o presente relatório foi um instrumento fundamental no processo de aprendizagem e para o desenvolvimento de competências comuns e específicas do enfermeiro especialista em enfermagem médico-cirúrgica em situação paliativa.
This report was drawn up as part of the development of the Professional Internship with report - Module I and Module II of the 1st Master's Degree in Medical-Surgical Nursing in the area of Palliative Care Nursing at the Nursing School of Porto. It aims to describe and analyse the activities carried out; to reflect on learning and the process of developing skills;. The method used to prepare this document was the descriptive and reflective one, using the best available evidence, contextualizing the facts and developing skills, based on regulations, as well as the experiences and opportunities lived in each clinical context. With regard to identifying the needs of significant persons in the last days or hours of life, there is a consensus that this should be done as early as possible and, to this end, it is essential to get to know the patient/family and include them in the decision-making process. Drawing up the individual care plan should aim to meet the needs of patients and their significant persons. Communication was recognized as the main tool professionals use to diagnose the needs of patients and their significant persons. Clinical interviews and family conferences are highlighted as essential means of identifying needs and mediating the illness process. The main needs of significant persons in a situation of last days or hours of life are: The need for information, the need to be close to the patient and to continue caring; the need for emotional support. Intervention with significant persons in the last days or hours of life should follow a multidimensional approach (spiritual, physical, educational and psychosocial). The critical-reflective component of the clinical cases and of the entire training course was preponderant for the acquisition of new skills when it comes to the decision-making process, therefore this report was a fundamental tool in the learning process and for the development of general and specific skills for nurses specializing in medical-surgical nursing in an area of palliative care.
