Abstract(s)
Introdução: A OMS define Cuidados Paliativos (CP) como cuidados de saúde que procuram
melhorar a qualidade de vida dos doentes em situação de sofrimento decorrente de doença
incurável, em fase avançada ou progressiva e às suas famílias, através da prevenção e alívio do
sofrimento, pela identificação precoce, avaliação correta e tratamento da dor e outros
problemas, sejam eles físicos, psicossociais ou espirituais. A exigência e complexidade dos
cuidados, torna imperativo a necessidade de profissionais formados e especializados nesta área.
A dor sendo um dos sintomas mais prevalentes e causadores de sofrimento nos doentes em
situação paliativa, faz com que a abordagem multidisciplinar da sua gestão, seja fundamental.
Objetivo: Pretende-se apresentar o percurso de aquisição e desenvolvimento de competências
comuns de enfermeiro especialista e competências específicas em enfermagem médico
cirúrgica na área da pessoa em situação paliativa, através de uma análise critico-reflexiva das
atividades desenvolvidas em ensino clínico em dois contextos da prática de CP.
Metodologia: Metodologia de projeto, considerando as aptidões enumeradas na legislação
portuguesa para a obtenção de grau de mestre, as competências comuns e específicas
preconizadas pela Ordem dos Enfermeiros para enfermeiro especialista em enfermagem médico
cirúrgica à pessoa em situação paliativa, destacando a intervenção especializada de
enfermagem face ao foco dor, tendo em conta a tomada de decisão do enfermeiro na gestão da
dor da pessoa em situação paliativa.
Resultados: Realizada a descrição das atividades concretizadas na prática clínica que
contribuíram para o desenvolvimento de competências comuns e específicas em enfermagem
médico cirúrgica à pessoa em situação paliativa, com a caracterização dos contextos clínicos e a
descrição de dois estudos de casos onde decorreu o estágio de natureza profissional (ENP).
Realizada ainda uma revisão da literatura, relativamente a fatores que influenciam o processo
de tomada de decisão dos profissionais de saúde, incluindo enfermeiros, na gestão da dor da
pessoa em situação paliativa.
Conclusão: Com o presente relatório pretendeu-se descrever todo o trajeto desenvolvido
focando as atividades que contribuíram para o desenvolvimento de competências, suportando a
prática como enfermeira especialista na melhor evidência disponível de modo a promover a
melhoria da qualidade dos cuidados prestados, o alívio do sofrimento, promoção da dignidade e
melhoria da qualidade de vida do doente em situação paliativa, seus cuidadores e família.
Introduction: The WHO defines Palliative Care (PC) as health care that seeks to improve the quality of life of patients suffering from incurable, advanced or progressive disease and their families, through the prevention and relief of suffering, early identification, correct assessment and treatment of pain and other problems, whether physical, psychosocial or spiritual. The demand and complexity of care makes it imperative to have trained and specialized professionals in this area. Pain is one of the most prevalent symptoms, causing suffering in palliative care patients, making a multidisciplinary approach to its management essential. Objective: It is intended to present the process of acquisition and development of common competences of a specialist nurse and specific competences in medical-surgical nursing in the area of the person in a palliative situation, through a critical-reflective analysis of the activities developed in clinical teaching in two contexts of PC practice. Methodology: Project methodology, considering the skills listed in Portuguese legislation for obtaining a master's degree, the common and specific competences recommended by the Portuguese Order of Nurses for nurses specializing in medical surgical nursing for people in palliative situations, highlighting the specialized nursing intervention in the face of the pain focus, taking into account the nurse's decision making in the management of pain in people in palliative situations. Results: The description of the activities carried out in clinical practice that contributed to the development of common and specific competences in medical surgical nursing for the person in a palliative situation, with the characterization of the clinical contexts and the description of two case studies where the professional internship took place. A review of the literature was also carried out, regarding factors that influence the decision-making process of health professionals, including nurses, in the management of pain in the palliative situation. Conclusion: The present report aimed to describe the entire path developed focusing on the activities that contributed to the development of skills, supporting the practice as a specialist nurse in the best available evidence in order to promote the improvement of the quality of care provided, the relief of suffering, promotion of dignity and improvement of the quality of life of the palliative patient, their caregivers and family.
