O presente relatório surge na sequência da conclusão do Estágio de Natureza Profissional
incluído no ciclo de estudos do 2º Mestrado em Enfermagem Médico-Cirúrgica na área de
Enfermagem à Pessoa em Situação Paliativa da Escola Superior de Enfermagem do Porto.
Conforme as orientações da Ordem dos Enfermeiros, o Estágio de Natureza Profissional
decorreu em dois contextos clínicos distintos da prática especializada de Cuidados Paliativos:
uma Equipa Intra-Hospitalar de Suporte em Cuidados Paliativos e uma Unidade de Cuidados
Paliativos. A concretização do estágio pressupôs a definição prévia de objetivos que visassem a
aquisição de competências especializadas nesta área profissional, pelo que finda a sua
realização, se procede à elaboração do relatório. Para o efeito, este documento pretende
realizar uma análise crítico-reflexiva das experiências clínicas adquiridas ao longo deste
percurso formativo e refletir sobre o contributo das experiências profissionais, como recursos
fundamentais para a consolidação do desenvolvimento de competências. Assim, a articulação e
aquisição de conhecimentos teóricos, baseados na melhor evidência científica disponível, foram
fundamentais para a sua concretização.
Perante circunstâncias de doença avançada, o doente e familiares cuidadores são confrontados
com múltiplas perdas que provocam mudanças e induzem sofrimento nos sistemas familiares.
Ao longo destes percursos, o enfermeiro especialista assume-se como um agente facilitador dos
processos de transição e adaptação à perda, concebendo e implementando intervenções,
autónomas e interdependentes, que mitigam o sofrimento. O acompanhamento de dois casos
clínicos, recorrendo à plataforma e4nursing da Escola Superior de Enfermagem do Porto,
pretendeu o aperfeiçoamento do planeamento e execução de cuidados em Enfermagem,
através da análise reflexiva da intervenção profissional dos enfermeiros especialistas.
O tema central deste relatório foi o luto antecipatório dos familiares cuidadores. O apoio à
família é um dos pilares da abordagem dos CP. O acompanhamento dos processos de doença e
a proximidade do momento da morte, expõe os familiares cuidadores a altos níveis de
sofrimento. Conhecendo-se a relação existente entre o sofrimento vivenciado, neste período, e a
vulnerabilidade para o desenvolvimento de sintomas de luto complicado após a morte,
considerou-se que a sua abordagem, no âmbito da prática profissional especializada do
enfermeiro, seria relevante, para melhor compreensão das respostas humanas a estes desafios.
Não existindo uma formalização da abordagem a esta área de atuação, procedeu-se à
realização de uma revisão da literatura, baseada na metodologia scoping review, para uma
melhor compreensão do tema.
The present report arises from the Professional Internship included in the 2nd Master in Medical- Surgical Palliative Care Nursing at the Nursing School of Porto. According to the guidelines of the Nursing Order, the Professional Internship took place in two distinct clinical contexts of specialized practice in Palliative Care:: an Intra-Hospital Palliative Care Support Team and a Palliative Care Unit. The realization of the internship presupposed the prior definition of objectives aimed at acquiring specialized skills in this professional area, so upon its completion, this report is elaborated. For this purpose, this document intends to carry out a critical-reflective analysis of the clinical experiences acquired throughout this formative journey and reflect on the contribution of professional experiences, as fundamental resources for the consolidation of skills development. Thus, the articulation and acquisition of theoretical knowledge, based on the best available scientific evidence, were fundamental for its realization. In the face of advanced illness circumstances, the patient and family caregivers are confronted with multiple losses that provoke changes and induce suffering in family systems. Throughout these journeys, the specialist nurse assumes the role of a facilitator of transition and adaptation processes to loss, conceiving and implementing autonomous and interdependent interventions that mitigate suffering. The follow-up of two clinical cases, using the e4nursing platform of the Nursing School of Porto, aimed at improving the planning and execution of Nursing care, through the reflective analysis of the professional intervention of specialist nurses. The central theme of this report was anticipatory grief of family caregivers. Family support is one of the pillars of the PC approach. The monitoring of illness processes and the proximity of the moment of death, exposes family caregivers to high levels of suffering. Knowing the relationship between the suffering experienced, during this period, and the vulnerability to the development of complicated grief symptoms after death, it was considered that its approach, within the scope of the specialized professional practice of the nurse, would be relevant, for a better understanding of human responses to these challenges. As there is no formalization of the approach to this area of action, a literature review was carried out, based on the scoping review methodology, for a better understanding of the topic.
