Este documento, sob a forma de relatório de estágio, surge no âmbito do término do curso de
Mestrado em Enfermagem Médico-Cirúrgica na área da Enfermagem à Pessoa em Situação
Crítica, da Escola Superior de Enfermagem do Porto. O presente relatório incorpora o que
emergiu das Unidades Curriculares “Estágio de natureza profissional com relatório, Módulo I e
II”, descrevendo de forma sistematizada as atividades e reflexões conducentes à aquisição de
competências, comuns e específicas, do Enfermeiro Especialista na área de Enfermagem à
Pessoa em Situação Crítica, tal como preconizado nos Regulamentos nº140/2019 e nº429/2018
da Ordem dos Enfermeiros.
Ao longo deste processo, procuramos evidenciar o desenvolvimento de competências
especializadas, a natureza dos contextos clínicos e a forma como as experiências vivenciadas
contribuíram para expandir e aprofundar essas competências. Considerando estas competências
como essenciais para a obtenção do grau académico de mestre e, no futuro, para a atribuição
do título profissional de Enfermeiro Especialista em Enfermagem Médico-Cirúrgica na área de
Enfermagem à Pessoa em Situação Crítica, conforme os requisitos estabelecidos pela Ordem
dos Enfermeiros.
Estruturalmente, o presente documento está dividido em três partes principais. A primeira,
centra-se na caracterização dos três contextos clínicos do estágio: um Serviço de Urgência
Polivalente, uma Unidade de Cuidados Intensivos Polivalente e uma unidade de cuidados
específicos, a Unidade de Cuidados Intensivos de Cirurgia Cardiotorácica. Na segunda parte, é
apresentado um “estudo de caso”, onde é detalhado o processo de conceção de cuidados, bem
como os seus fundamentos. A última parte consagra-se numa reflexão sobre o processo de
desenvolvimento das competências, comuns e especificas, do Enfermeiro Especialista na área
de Enfermagem à Pessoa em Situação Crítica. A elaboração deste relatório foi sustentada na
plataforma pedagógica da Escola Superior de Enfermagem do Porto, a "e4nursing", em vigor na
instituição.
A complexidade dos cuidados à Pessoa em Situação Crítica exige uma prática especializada,
alicerçada nas competências do Enfermeiro Especialista e Mestre em Enfermagem Médico-
Cirúrgica. Assumindo que o desenvolvimento de competências é um processo dinâmico e
contínuo, não se limitando ao estágio ou ao curso, considero que este percurso foi um marco
importante, permitindo-me adquirir ferramentas que serão essenciais no futuro para o meu
exercício profissional. A aprendizagem no contexto de estágio envolve, não só a organização de
conceitos teóricos e práticos, mas também a conceção da ação refletida e modificada, baseada em evidência científica. Este processo culmina na mobilização de saberes especializados,
orientando uma prática profissional consciente, segura e ética, com o objetivo de garantir a
qualidade do cuidar em enfermagem.
This report, developed as part of an internship, marks the completion of the Master's Degree in Medical-Surgical Nursing, in the area of Nursing for People in Critical Situations, at the Escola Superior de Enfermagem do Porto. This document integrates insights from the curricular units "Professional Internship with Report, Module I and II”, systematically describing the activities and reflections that contributed to the acquisition of both general and specific competencies required for a Specialist Nurse in Critical Care Nursing, as outlined in Regulations No. 140/2019 and No. 429/2018 of the Portuguese Nursing Association. Throughout this process, we seek to highlight the development of specialized competencies, the nature of the clinical settings and how the experiences lived contributed to expanding and deepening these competencies. These competencies are considered essential for obtaining a master's degree and, in the future, for the attribution of the professional title of Specialist Nurse in Critical Care Nursing, in accordance with the requirements established by the Portuguese Nursing Association. Structurally, this document is divided into three main parts. The first focuses on characterization of the three clinical internship settings: a Emergency Department, a Intensive Care Unit and a specialized care unit, the Cardiothoracic Surgery Intensive Care Unit. In the second part presents a “case study”, where the care planning process is detailed, as well as its foundations. The last part is dedicated to a reflection on the process of developing the common and specific competences of the Specialist Nurse in Critical Care Nursing. The preparation of this report was supported by the pedagogical platform, “e4nursing”, in force at the institution. The complexity of care for critical ill patients requires specialized practice, based on the competencies of the Specialist Nurse and the Master’s Degree in Medical-Surgical Nursing. Assuming that competency development is a dynamic and continuous process, not limited to the internship or the course, I consider this journey an important milestone, allowing me to acquire tools that will be essential in the future for my professional practice. Learning in the internship setting involves not only the organization of theoretical and practical concepts but also the design of reflected and modified action, based on scientific evidence. This process culminates in the mobilization of specialized knowledge, guiding a conscious, safe and ethical professional practice, with the aim of ensuring the quality of nursing care.
