Inequities compromise the well-being of families throughout the life cycle. The Family Health Nurse Specialist intervenes with sensitivity and a strong orientation toward equity. This report outlines the internship experience undertaken as part of the Master's Degree in Community Nursing, with a specialization in Family Health Nursing. The internship was conducted in two distinct healthcare settings, with the aim of acquiring and developing advanced clinical competencies to support a family-centered practice. In a Personalized Healthcare Unit, a case study was conducted with an immigrant nuclear family, resulting in measurable health gains by facilitating healthy transitions through culturally congruent interventions. In a Family Health Unit, a family health nursing intervention project was developed from an anticipatory perspective (primary prevention), focusing on the self-perception of ageism and the empowerment of elderly families. Objective: To contribute to the empowerment of elderly families in raising awareness of and preventing ageism. Methodology: A scoping review was carried out. The study followed a descriptive, cross-sectional, and quantitative design and involved 16 families (23 members). Data were collected using the Ageism Survey, the adapted Graffar Scale, and the Family APGAR, and were statistically analysed using Microsoft Excel©. The systemic interventions were grounded in Meleis’ Transitions Theory, the Family Nursing Process, and Figueiredo’s Dynamic Model of Family Assessment and Intervention. Discussion: The results suggest that ageism is frequently perceived as a recurrent phenomenon, with variations across social classes and levels of family functionality. Following the systemic interventions - which included therapeutic conversations - there was an observed increase in participants’ self-perception, indicating heightened awareness of the phenomenon. Participants expressed interest in discussing the topic with their extended families and within their communities. Conclusion: Specialized nursing practice proves effective in promoting health equity and strengthening health literacy in contexts of vulnerability, leading to improved health outcomes for the families under care.
