Abstract(s)
A crescente necessidade de artroplastia da anca tem resultado num aumento
dos tempos de espera, podendo exacerbar o declínio funcional das Pessoas com
osteoartrite e dificultar a recuperação pós-operatória. Esta recuperação está relacionada
com os cuidados de Enfermagem de Reabilitação ao nível da mobilidade, ensino e
preparação para o regresso ao domicílio. O sucesso da intervenção cirúrgica depende
da intervenção do Enfermeiro Especialista em Enfermagem de Reabilitação, a qual deve
ser precoce, planeada e individualizada, em concordância com as necessidades e os
objetivos da Pessoa. O objetivo geral deste Projeto foi elaborar o Programa de
Reabilitação e Capacitação para a transição segura para o domicílio da Pessoa
submetida a artroplastia da anca. Foram desenvolvidas as seguintes atividades:
conversas com a equipa de Enfermagem; análise SWOT do plano de cuidados do
serviço; revisão da literatura sobre artroplastia da anca e transição segura para o
domicílio; conceção, apresentação e implementação do guia de questões sobre as
características habitacionais, bem como o Programa de Reabilitação.
A avaliação das características habitacionais oferece contributos importantes na
adaptação do Programa de Reabilitação às necessidades da Pessoa/Família. Embora
este programa tenha sido implementado faltam evidências quanto à sua inclusão nos
registos de Enfermagem de Reabilitação: 100% dos processos têm identificados os
focos de atenção definidos com a avaliação do nível de dependência para cada
autocuidado e do potencial de reconstrução de autonomia para o autocuidado; mas 10%
dos processos têm a avaliação da capacidade para o uso de estratégias adaptativas
para cada autocuidado. O Programa de Reabilitação desenvolvido, conjuntamente com
os cuidados de Enfermagem de Reabilitação, proporciona: melhor controlo da dor;
diminuição de complicações; ganhos em amplitude de movimento; recuperação mais
rápida da força muscular, capacidade de marcha e funcionalidade; maior capacidade
para o autocuidado aquando da alta; menor risco de queda; maior qualidade de vida.
The increasing demand for hip arthroplasty has led to longer waiting times, which may exacerbate the functional decline in individuals with osteoarthritis and hinder postoperative recovery. This recovery is closely linked to Rehabilitation Nursing care in terms of mobility, education, and preparation for discharge home. The success of the surgical intervention depends on the involvement of the Specialist Nurse in Rehabilitation Nursing, which should be initiated early, planned, and individualized, in alignment with the person's needs and goals. The overall objective of this project was to develop a Rehabilitation and Empowerment Program for the safe transition home of individuals undergoing hip arthroplasty. The following activities were undertaken: discussions with the Nursing team; SWOT analysis of the care plan in the service; literature review on hip arthroplasty and safe transition home; design, presentation, and implementation of a guide with questions on housing characteristics, as well as the Rehabilitation Program. The assessment of housing characteristics offers important insights for adapting the Rehabilitation Program to the needs of the individual and family. Although this program has been implemented, there is a lack of evidence regarding its inclusion in the Rehabilitation Nursing records: 100% of the cases have identified the areas of focus based on the assessment of dependence level for each self-care activity and the potential for restoring autonomy in self-care; however, only 10% of the cases include an assessment of the capacity to use adaptive strategies for each self-care activity. The developed Rehabilitation Program, along with Rehabilitation Nursing care, provides: better pain management; reduced complications; improvements in range of motion; faster recovery of muscle strength, walking ability, and functionality; greater capacity for self-care at discharge; lower risk of falls; and improved quality of life.
