Introdução: O edentulismo mandibular afeta severamente a qualidade de vida, e as próteses totais convencionais apresentam frequentes limitações de estabilidade e retenção. Embora a sobredentadura retida por dois implantes seja considerada o padrão de referência existem várias restrições - financeiras, sistémicas ou anatómicas - que inviabilizam a sua colocação.
Desenvolvimento: A utilização de um único implante colocado na linha média mandibular (sínfise) surge como uma alternativa minimamente invasiva e económica. Atuando como um fulcro central, o implante ajuda a resistir às forças de descolamento, garantido uma grande melhoria na eficiência mastigatória face às próteses convencionais. Esta reabilitação apresenta taxas de sobrevivência superiores a 95% e aumenta substancialmente a satisfação dos pacientes.
Contudo, devido aos movimentos rotacionais, existe uma maior concentração de forças, exigindo manutenção frequente devido ao rápido desgaste das matrizes de retenção e um maior risco de fratura da base protética.
Conclusão: As sobredentaduras retidas por um único implante representam uma solução terapêutica inclusiva, segura e com bom custo-benefício para populações mais vulneráveis. Apesar da necessidade de maior manutenção clínica, atenua o impacto do edentulismo e melhora a qualidade de vida geral dos pacientes.
Introduction: Mandibular edentulism severely affects quality of life, and conventional complete dentures often suffer from limitations in terms of stability and retention. Although the two-implant-retained overdenture is considered the gold standard, there are various restrictions – such as financial, systemic or anatomical factors – that make its placement unfeasible. Development: The use of a single implant placed in the mandibular midline (symphysis) emerges as a minimally invasive and cost-effective alternative. Acting as a central fulcrum, the implant helps resist dislodgement forces, ensuring a substantial improvement in masticatory efficiency compared to conventional dentures. This rehabilitation has survival rates exceeding 95% and substantially increases patient satisfaction. However, due to rotational movements, there is a greater concentration of forces, requiring frequent maintenance due to the rapid wear of the retention matrices and an increased risk of fracture of the prosthetic base. Conclusion: Overdentures retained by a single implant represent an inclusive, safe and cost-effective therapeutic solution for more vulnerable populations. Despite the need for greater clinical maintenance, they mitigate the impact of edentulism and improve patients overall quality of life.
