Abstract(s)
A estenose valvular aórtica caracteriza-se como uma patologia que provoca limitações físicas e emocionais que condicionam a funcionalidade e qualidade de vida. O tratamento desta condição passa pela cirurgia de substituição valvular. O sucesso cirúrgico e a recuperação da pessoa são frequentemente condicionados pelas complicações pós-operatórias que a evidencia demonstra serem preveníeis ou minimizadas pela Reabilitação Cardíaca (RC) pré-operatória.
A RC pré-operatória é uma estratégia terapêutica não farmacológica que é segura e efetiva na redução das complicações pós-operatórias. Contudo, pouco se sabe sobre estratégias que sejam facilitadoras da implementação de uma intervenção especializada de enfermagem de reabilitação no domínio da RC pré-operatória em pessoas propostas para cirurgia de substituição valvular aórtica.
Este estudo teve por finalidade definir uma estratégia para a sistematização da implementação de uma intervenção especializada de enfermagem de reabilitação no pré-operatório de pessoas submetidas a cirurgia valvular aórtica num centro de cirurgia cardiotorácico do norte de Portugal.
Desenvolvemos um estudo exploratório e transversal, utilizando como técnica de recolha de dados a entrevista semiestruturada com recurso a um focous group constituído por sete Enfermeiros especialistas em Enfermagem de Reabilitação a exercer funções no contexto do estudo. Na análise dos dados utilizamos a técnica de análise de conteúdo proposta por Bardin.
A análise de conteúdo permitiu identificar quatro unidades temáticas: necessidades de cuidados da pessoa no pós-operatório; intervenção do EEER para prevenir complicações pós-operatórias; estratégias para a implementação de uma intervenção especializada em enfermagem de reabilitação, e estratégias para sistematização da implementação das intervenções em enfermagem na pessoa em situação pré-operatória. Estas unidades temáticas complementadas pela literatura permitiram propor uma estratégia de intervenção para o contexto.
Esta proposta para a sistematização da intervenção do EEER apresenta potencial para influenciar positivamente os resultados em saúde das pessoas submetidas a cirurgia de substituição valvular, contribuindo também para a otimização da funcionalidade e qualidade de vidas das pessoas no pós-operatório reduzindo a incapacidade e contribuindo para uma rápida reintegração social.
Aortic valve stenosis is characterized as a pathology that causes physical and emotional limitations that affects functionality and quality of life. The treatment of this condition is the replacement of the valve through surgery. The surgical success and recovery are often conditioned by postoperative complications where the evidence shows can be prevented or minimized by preoperative Cardiac Rehabilitation (CR). Preoperative CR is a safe non-pharmacological therapeutic strategy that is effective in reducing postoperative complications. However, about this subject, important conclusions weren’t achieved about strategies that facilitate the implementation of a specialized rehabilitation nursing intervention in the field of preoperative CR in people that have been proposed for aortic valve replacement surgery. This study aimed to define a strategy to systematize the implementation of a specialized rehabilitation nursing intervention in the preoperative period of people undergoing aortic valve surgery in a cardiothoracic surgery center in the North of Portugal. An exploratory and cross-sectional study was conducted using a semi-structured interview as a data collection technique using a focus group composed for seven specialist nurses in Rehabilitation Nursing that were working in the context of the study. On the data analysis, we used the content analysis technique proposed by Bardin. The analysis of the content allowed the identification of four thematic units: the postoperative patient's care needs; interventions in rehabilitation nursing to prevent postoperative complications; strategies for the implementation of a specialized intervention in rehabilitation nursing, and strategies for the systematization of the implementation of nursing interventions in preoperative patients. These thematic units, complemented by the literature, allowed us to propose an intervention strategy for the context. This proposal for the systematization of interventions in rehabilitation nursing has the potential to positively influence the health outcomes of people that were submitted to the valve replacement surgery, contributing to the optimization of functionality and quality of life of people in the postoperative period, reducing disability and contributing to a rapid social reintegration.
