O presente relatório de estágio de natureza profissional apresenta como temática transversal “A Esperança da Pessoa em Situação Paliativa: Contributos para uma Avaliação Sistematizada”, refletindo o desenvolvimento das competências comuns e específicas do enfermeiro especialista em Enfermagem Médico-Cirúrgica na área de Especialização em Enfermagem à Pessoa em Situação Paliativa, em contexto da prática clínica. Para além de descrever e refletir sobre as atividades realizadas em contexto de estágio, bem como sobre as competências desenvolvidas, este projeto tem também por finalidade contribuir para a avaliação da esperança a clientes adultos em situação paliativa, em contexto hospitalar.
A pessoa em situação paliativa é confrontada, não raras vezes, com notícias e experiências promotoras de sofrimento e angústia a vários níveis. A esperança é um domínio de enfermagem que integra várias dimensões do ser humano, seja físico, psicossocial, emocional e espiritual, pelo que a sua avaliação ao longo de todo o processo de doença é primordial.
Neste documento foi utilizada uma metodologia descritiva, através de uma análise crítica e reflexiva, suportada na literatura científica disponível. O projeto de desenvolvimento de competências, centrada na temática da Esperança, teve por base uma revisão narrativa da literatura. Este projeto foi desenvolvido durante o Estágio de Natureza Profissional com relatório – Módulos I e II, realizado numa Unidade de Cuidados Paliativos e numa Equipa Intra-Hospitalar de Suporte em Cuidados Paliativos.
Os principais resultados foram o desenvolvimento das competências comuns, bem como das competências específicas do enfermeiro especialista em Enfermagem Médico-Cirúrgica na área da Enfermagem à Pessoa em Situação Paliativa, já que este é um processo dinâmico. Estes resultados estão espelhados ao longo do presente relatório através das atividades desenvolvidas em contexto da prática clínica, nomeadamente a revisão narrativa da literatura, da qual emergiram alguns instrumentos importantes para avaliação da esperança e indicadores clínicos.
O projeto inicialmente delineado não viu alguns dos seus objetivos implementados. Apesar disso, com o apoio na evidência científica e através do levantamento das três tipologias de indicadores foi possível avaliar a esperança, de forma mais sistematizada e acurada, nomeadamente nos estudos de caso sobre os quais nos debruçamos durante o estágio, bem como de outros clientes, em ambos os contextos da prática clínica.
This professional internship report focuses on the transversal theme of “ Hope of Adult Patients in a Palliative Situation: Contributions Towards a Systematized Assessment”. It reflects the development of common and specific competencies of a specialist nurse in Medical-Surgical Nursing in the area of Specialization in Palliative Care, within the context of clinical practice. In addition to describing and reflecting on the activities carried out during the internship, as well as on the competencies developed, this project also aims to contribute to the assessment of hope in adult patients in palliative care, in a hospital setting. People in palliative care are often faced with news and experiences that promote suffering and distress at various levels. Hope is a nursing domain that integrates several dimensions of the human being, whether physical, psychosocial, emotional, or spiritual. Therefore, its assessment throughout the disease process is essential. This document uses a descriptive methodology, through a critical and reflective analysis, supported by the available scientific literature. The competency development project, focused on the theme of Hope, was based on a narrative literature review. This project was developed during the Professional Internship with Report – Modules I and II, carried out in a Palliative Care Unit and an Intra-Hospital Palliative Care Support Team, for a total of 840 hours. The main results were the development of common competencies, as well as the specific competencies of a specialist nurse in Medical-Surgical Nursing in the area of Palliative Care, since this is a dynamic process. These results are reflected throughout this report through the activities developed in the context of clinical practice, namely the narrative literature review, from which emerged some important instruments for the assessment of hope and clinical indicators. The initially outlined project did not see some of our objectives being executed. However, with the support of scientific evidence and through the identification of the three types of indicators, it was possible to assess hope in a more systematized and accurate way, namely in the case studies that we focused on during the internship, as well as in other patients, in both contexts of clinical practice.
