O modelo de governação do Subsistema Público de Saúde – Assistência na Doença
aos Militares, configura um tema de grande atualidade e importância, para o seu universo de
beneficiários. Todavia, análises recentes provenientes de estudos académicos e auditorias
temáticas têm destacado a relevância de reajustar o modelo de governação, face à deteção
de ineficiências que comprometem a eficácia da gestão e a qualidade da resposta na
assistência na doença à família militar. Neste sentido, o presente trabalho teve como
propósito analisar e delinear orientações estratégicas suscetíveis de mitigar os
constrangimentos mais relevantes identificados, com vista a promover a implementação de
um novo modelo de governação no subsistema.
Nesse sentido, com base num modelo de raciocínio indutivo, suportado numa
estratégia de investigação qualitativa e num desenho de estudo de caso, procedeu-se à
revisão de literatura, complementada com a realização de entrevistas a entidades detentoras
de conhecimento neste âmbito. Concomitantemente, foram analisados modelos de
governação de subsistemas públicos de saúde congéneres no contexto nacional, e o que serve
as Forças Armadas espanholas, com o propósito de identificar práticas potencialmente
configuradoras de oportunidades de melhoria no quadro da modernização e aperfeiçoamento
do modelo de governação do Subsistema Público de Saúde – Assistência na Doença aos
Militares.
A investigação evidenciou a necessidade de implementar um novo modelo de
governação no subsistema, capaz de respeitar a sua natureza e as especificidades inerentes a
um Subsistema Público de Saúde destinado servir os seus beneficiários. Tal transformação
revela-se viável e desejável, desde que acompanhada das devidas adaptações e precauções,
podendo inspirar-se em práticas já consolidadas e eficazes noutros Subsistemas Públicos de
Saúde congéneres.
The governance model of the Public Health Subsystem - Military Health Care is a highly topical and important issue for its beneficiaries. However, recent analyses from academic studies and thematic audits have highlighted the importance of recalibrating the governance model, as inefficiencies have been identified that affect the effectiveness of management and the quality of response in the provision of health care to military families. In this sense, the aim of this study was to analyse and outline strategic guidelines to mitigate the main constraints identified, with a view to promoting the implementation of a new governance model in the subsystem. To this end, based on an inductive reasoning model, supported by a qualitative research strategy and a case study design, a literature review was carried out, complemented by interviews with entities with knowledge in the field. At the same time, governance models of similar public health subsystems in the national context and those serving the Spanish Armed Forces were analysed to identify practices that could potentially lead to opportunities for improvement in the modernisation and refinement of the governance model of the Public Health Subsystem - Medical Assistance to Military Personnel. The research highlighted the need to implement a new governance model in the subsystem, capable of respecting its nature and the specific characteristics inherent to a public health subsystem designed to serve its beneficiaries. Such a transformation is feasible and desirable, if it is accompanied by the necessary adaptations and precautions and can draw inspiration from practices already well established and effective in other similar public health subsystems.
