Abstract(s)
O presente trabalho é a etapa final de um percurso formativo inserido no curso de Mestrado
em Enfermagem Médico-Cirúrgica na área de especialização em Enfermagem no cuidado à
Pessoa em Situação Crítica.
Neste, será realizada uma análise crítica, reflexiva e fundamentada do percurso realizado,
tendo em conta as vivências e atividades desenvolvidas, ao longo de três estágios, e o seu
contributo para a aquisição e desenvolvimento de competências especializadas e de Mestre.
Foi realizado um projeto de intervenção, com implementação nos diferentes campos de
estágio, que teve como base a Metodologia de Projeto e enquadrado no Modelo para a
Mudança da Prática Baseada na Evidência. Este teve como objetivo sensibilizar, os
profissionais de saúde, para a importância da aplicabilidade da Metodologia ISBAR durante
a transição de cuidados garantido segurança e qualidade nos cuidados prestados.
Na saúde, a comunicação eficaz, é fundamental para garantir uma prestação de cuidados
humanizada, segura e personalizada com resultados positivos para o doente e para os
profissionais de saúde. Atualmente à luz da evidência, 70% dos eventos adversos resultam
de uma comunicação fraca e ineficaz entre profissionais de saúde durante a transição de
cuidados. A utilização de metodologias padronizadas de comunicação, pode reduzir esta
percentagem, melhorando o processo de transferência de informação como é exemplo a
metodologia ISBAR.
Trata-se de uma ferramenta que padroniza a comunicação em saúde, durante os momentos
de transferências dos doentes, e é caraterizada por facilitar a transmissão de informação
essencial de forma rápida e eficaz. Esta é reconhecida internacionalmente, e em Portugal
existe uma norma reguladora (Norma nº001/2017), criada pela Direcção Geral da Saúde,
que padroniza esta metodologia em todas as instituições de saúde.
De modo a garantir uma boa base de sustentação e fundamentação de todas as atividades
realizadas durante o estágio foram, previamente, efetuadas diversas pesquisas em diferentes
bases de dados científicas com o intuito de garantir informação fidedigna.
This work is the final stage of a training path included in the Master's degree in Medical-Surgical Nursing in the area of specialization in Nursing in the care of People in Critical Situations. In this, a critical, reflective and well-founded analysis of the path taken will be carried out, taking into account the experiences and activities developed, throughout three stages, and their contribution to the acquisition and development of specialized and Master's skills. An intervention project was carried out, with implementation in the different internship fields, which was based on the Project Methodology and framed in the Evidence-Based Practice Change Model. This aimed to raise awareness among health professionals about the importance of the applicability of the ISBAR Methodology during the transition of care, guaranteeing safety and quality in the care provided. In healthcare, effective communication is essential to guarantee a humanized, safe and personalized provision of care with positive results for the patient and healthcare professionals. Currently in light of evidence, 70% of adverse events result from weak and ineffective communication between healthcare professionals during the transition of care. The use of standardized communication methodologies can reduce this percentage, improving the information transfer process, such as the ISBAR methodology. It is a tool that standardizes health communication during patient transfers, and is characterized by facilitating the transmission of essential information quickly and effectively. This is internationally recognized, and in Portugal there is a regulatory standard (Norm nº001/2017), created by the General Directorate of Health, which standardizes this methodology in all health institutions. In order to guarantee a good basis of support and justification for all activities and actions carried out, bibliographical research was carried out in different scientific databases in order to guarantee reliable information.
